5.The Frequency of Serum IgA autoantibody in Patients with Pemphigus.

BACKGROUND: Pemphigus is an autoimmune bullous disease with circulating desmosomal autoantibodies of IgG. In direct IF studies with perilesional tissue, IgA or IgM antibodies can be seen in addition to IgG. OBJECTIVE: We examined sera of patients with pemphigus for the presence/frequency of IgA autoantibodies as well as IgG by indirect IF and immunoblot assay. Patients: Twenty patients of pemphigus (PV 10, PF 10) who showed positive findings in indirect IF examinations. METHODS: Indirect IF study with normal human skin substrates and immunoblot analysis using A431 cell extracts (with multi-step immunostaining) were performed with patients sera. RESULTS: In indirect IF, IgA autoantibodies that bind to the epidermal keratinocyte antigens were detected in 4 cases among the 20 patients (PV 2 and PF 2). In immunoblot analysis IgA bands reacting to PV/PF antigens were observed in 7 cases from the 20 patients with pemphigus (PV 3, PF 4). The serum titers of IgA autoantibodies were lower than those of IgG in every single case. CONCLUSION: In patients with pemphigus (PV/PF), 35% of cases have serum IgA autoantibodies as well as IgG autoantibodies specific to the pemphigus antigens (Dsg 1/Dsg 3). However, pathogenic roles of the associated IgA autoantibody are not clear.

6.Gynecologic Application of the Pelviscopic Surgery.

The aim of this study was to identify the usefulness of pelviscopy in treatment besides its diagnostic value. The advantages of pelviscopic surgery are low cost, rapid recovery, good cosmetic effect, low incidence of complication and postoperative adhesion. So most of the pelvic exploration in gynecologic patients are replaced by the pelviscopic surgery these days. Pelviscopic surgery was performed on 136 patients at the Department of Obstetrics and Gynecology, College of Medicine, Yenungnam University from May 1991 to July 1993. The results obtained were as follows : The age distribution of the patients was from 19 to 55 with age of 31.2 years, and the mean parity was 0.96, the most common indication of pelviscopic surgery was tubal pregnancy(66.9%), the second most common indications was ovarian cyst(10.3%) and the other indications were endometriosis, corpus iuteum cyst rupture, parovarian cyst, foreign body, tubal ligation, hydrosalpinx, uterine myoma and in 16.3%. The mean duration of hospitalization was 2.1 days without specific complications. According to these results, it was postulated that the pelviscopic surgery was a useful operative tool in gynecologic treatment and its application could be extended to many other areas of gynecology with safety by the development of surgical techniques and instruments.

7.Factors Affecting the Complaints of Subjective Symptoms in VDT Operators.

In order to investigate symptoms prevalence related to visual display terminal (VDT) syndrome, and to evaluate the variables affecting the development of the VDT syndrome, a questionnaire survey was conducted on 119 (92 men and 27 women) VDT operators who had been engaged in one plant design company in Seoul. The results were as follows : The average age of questionnaire respondents was 30. The average years of service were 4.3 years, and the working hours per day with VDT for half of them were more than 4 hours with the average being about 4.7 hours. The majority of subjects (71 persons) were working at computer aided design (CAD) or computer programming tasks. Subjective symptoms of VDT syndrome were divided into 4 groups with musculoskeletal, eye, skin, and industrial fatigue according to their major affected organ. 4 high incidence of complaints of eye discomfort was observed. In the 3 subgroups of industrial fatigue symptom, the highest average group score was accounted for by 'dullness and sleepiness', followed by 'difficulty in concentration', 'bodily projection of fatigue' in the order of sequence, suggesting the heavier mental stress of VDT work rather than physical burden. Relationship among these 4 groups of symptoms showed statistically significant correlations one another. This result suggests that VDT syndrome is a multi-organ disease composed of these 4 symptom groups which were commonly caused by the use of VDT. Multiple regression analysis were used to examine the relationship between risk factors and these symptom groups of VDT syndrome. As the result of analysis, the most important risk factor for VDT syndrome was the average daily VDT working hours. The longer was VDT working hours, the higher was symptom prevalence of VDT syndrome. In addition, daily total working hours including VDT working hours and shift of work also affected the health of VDT users. In order to protect workers from VDT syndrome, it is urgently required to implement standard management recommendations including restriction of VDT working hours and allowance of more sufficient resting time for VDT workers.

8.Effects of Nitric Oxide Synthesis Inhibition on the Blood Pressure Response to Bradykinin in Rats.

Bradykinin has been known to elicit a pressor effect when administered centrally, and a depressor effect when administered peripherally. The present study was aimed at investigating whether the blood pressure response to bradykinin is dependent on the endogenous generation of nitric oxide (NO). Effects of NG-nitro-L-arginine methyl ester (L-NAME) on the pressor and depressor responses to intracerebroventricularly and intravenously injected bradykinin (5nmol/rat), respectively, were examined in anesthetized rats. Neither the pressor response nor the depressor response was affected by acute parenteral treatment with L-NAME. The pressor and depressor effects of bradykinin were also noted in rats chronically supplemented with L-NAME in drinking water for 4 weeks. Bradykinin caused a relaxation of the isolated thoracic aorta in vitro, which was not affected in the presence of L-NAME. However, bradykinin failed to cause a relaxation of the aorta isolated from rats chronically treated with L-NAME. These findings suggest that endogenous generation of NO may not completely account for the blood pressure responses to bradykinin in rats.

9.Transient Receptor Potential Canonical 4 and 5 Channel Antagonist ML204 Depolarized Pacemaker Potentials of Interstitial Cells of Cajal

Background/Aims:
To investigate an effect of ML204 (an inhibitor of transient receptor potential canonical 4 and 5 [TRPC4/5] channels) on interstitial cells of Cajal (ICCs) and therefore determine whether TRPC4/5 channels act on ICC-generated pacemaker activity.
Methods:
We enforced whole cell patch clamp analysis, measurements of the intracellular Ca2+ concentration, and reverse transcription polymerase chain reaction to determine the effect of ML204 (10 μM) or englerin A (a selective activator of TRPC4/5 channeles, 10 μM) and the existence of TRPC4/5 in mouse small intestinal ICC.
Results:
Treatment of ICCs with ML204 or englerin A caused the membrane potentials to depolarize. This depolarization effect of membrane potentials by ML204 in ICCs was observed to be concentration-dependent. After treating Ca 2+ - and Na + -free solutions or flufenamic acid (a non-selective cation channel blocker), the pacemaker potentials in the ICCs were abolished. A specific anoctamin 1 channel blocker did not have any effect on the pacemaker activity in ML204-untreated control cells; however, they blocked ML204-induced pacemaker activity in ICCs. Specific primers designed against TRPC4 and TRPC5 detected the presence of TRPC4/5 in small intestinal ICCs, and the application of ML204 increased raise the frequency of Ca2+ oscillations in ICCs, as assessed using Fluo-4 AM.
Conclusion
The results implied that ML204 could not inhibit the pacemaker activity but depolarized the membrane potential of ICCs by regulating intracellular Ca2+oscillations and anoctamin 1 channels.
10.Neurotensin enhances gastric motility in antral circular muscle strip of guinea-pig..

Many reports suggest that neurotensin (NT) in the gastrointestinal tract may play a possible role as a neurotransmitter, a circulating hormone, or a modulator of motor activity. NT exerts various actions in the intestine; it produces contractile and relaxant responses in intestinal smooth muscle. This study was designed to investigate the effect of NT on motility of antral circular muscle strips in guinea-pig stomach. To assess the role of Ca2+ influx in underlying mechanism, slow waves were simultaneously recorded with spontaneous contractions using conventional intracellular mircoelectrode technique. At the concentration of 10-7 M, where NT showed maximum response, NT enhanced the magnitude (863 +/- 198%, mean +/- SEM, n = 13) and the frequency (154 +/- 10.3%, n = 11) of spontaneous contractions. NT evoked a slight hyperpolarization of membrane potential, tall and steep slow waves with abortive spikes (278 +/- 50%, n = 4). These effects were not affected by atropine (2 micrometer), guanethidine (2 micrometer) and tetrodotoxin (0.2 micrometer). NT-induced contractile responses were abolished in Ca2+-free solution and reduced greatly to near abolition by 10 micrometer of verapamil or 0.2 mM of CdCl2. Verapamil attenuated the effects of NT on frequency and amplitude of the slow waves. Taken together, these results indicate that NT enhances contractility in guinea-pig gastric antral circular muscle and Ca2+ influx through the voltage-operated Ca2+ channel appears to play an important role in the NT-induced contractile mechanism.
