3.Cytokine production of peripheral blood mononuclear cells from atopic asthmatics.

To investigate the imbalance of the cytokine production profile of T cells from atopic asthmatics, we measured concentrations of IL-4, IL-5 and IFN-y by ELISA method in the culture supernatants of peripheral blood mononuclear cells(PBMCs) and Derrnato-phagoides pteronyssinus(Der p) J-stimulated PBMCs from Der p-sensitized atopic asthmatics, Der p-sensitized healthy atopits, non-atopic asthmatics and healthy non-atopics. The suppressive effect of IFN-y on cytokine production of Der p J-stimulated PBMCs was also examined. The PBMCs from atopics showed higher IL-4 and IL-5 production in response to PHA +TPA and higher IFN-gamma production in response to Der p Jq compared with non-atopits. The Der p J-stimulated PBMCs from atopics showed a tendency of increased IL-5 production in response to Der p J and higher IL-4 and IL-5 production in response to PHA+TPA compared with non-atopics. IL-5 production of Der p J-stimulated PBMCs from atopics was suppressed by IFN It is suggested that an imbalance in IL-4, IL-5 and IFN-y production is a feature of the atopic state. The TH2 characteristics of allergen-stimulated PBMCs could be regulated by IFN-y.

4.The Studies of bcl-2 Oncoprotein and Epstein-Barr Virus Expression in Malignant Lymphomas: Immunohistochemical and in situ hybridization analysis on 66 cases.

Bcl-2 oncoprotein is being localized to mitochondria and interfering with programmed cell death (apoptosis) independent of promoting cell division in the lymphoid and nonlymphoid cells. The bcl-2 oncoprotein expression has been reported in follicular lymphomas as well as in diffuse non-Hodgkin's lymphoma, leukemia and a variable propotion of Hodgkin's lymphoma cases. Recent evidence suggests that some lymphomas protected from apoptosis is conferred through expression of Epstein-Barr virus(EBV) latent membrane protein which turn to cause upregulation of bcl-2. To define the role of the bcl-2 oncoprotein and EBV in lymphoid malignancy, we tried immunohistochemical studies with anti-bcl-2 antibody and In situ hybridization (ISH) with EBV-encoded small nuclear RNAs(EBER) in the paraffin embedded sections of 46 non-Hodgkin's lymphoma (NHL) cases and 20 Hodgkin's lymphoma (HL) cases. Bcl-2 oncoprotein expression was found in 37 of 46 cases (80%) of NHL with relatively strong cytoplasmic staining, and in 14 of 20 cases (70%) of HL with weak cytoplasmic staining in limited small numbers of RS, Hodgkin and lacunar cells. The widespread presence of bcl-2 oncogene in many different types of both NHL and HL supports that the extended cell survival through overexpression of bcl-2 gene protein may be a growth advantage of neoplastic lymphoid cells. In the ISH analysis for EBV, the presence of EBV was detected in 17 of 20 cases (85%) of HL, compared to 6 of 44 cases(13.6%) of NHL. It appears to be no direct correlation between overexpression of bcl-2 oncoprotein by neoplastic lymphoid cells and the presence of EBV in NHL but it seems to be a definite association between EBV and HL.

6.A Study of Korean Literature Review Related to Speech Characteristics and Speech Therapy in Patients with Parkinson Disease

The purpose of this study was to investigate the speech characteristics and speech therapy of Parkinson disease (PD). This study selected 28 papers published in Korea from 1998 to 2018 after searching the terms ‘Parkinson voice’ and ‘Parkinson speech therapy.’ Literature review had been conducted in the two aspects of speech characteristics and speech therapy. The speech characteristics were divided into respiration, phonation, articulation, prosody, vowel production, and voice questionnaire. Speech therapy was divided into Lee Sliverman voice treatment (LSVT) and other voice therapy. PD patients did not differ in respiration function compared to normal elderly people, but their speech and articulation function were poorer. There was also a difference in the speech rate, frequency of pause, and accuracy of vowel production compared with normal elderly people. PD had a lower VHI score and their voice related quality of life was a little poorer. The LSVT was typically used in speech therapy for PD. The methods of speech therapy for PD have been shown to improve respiration and phonation. It is necessary to establish voice norms in PD patients and develop effective speech therapy in the following study.
7.Fracture of the Ipsilateral Femur after Total Hip Arthroplasty.

PURPOSE: Although considered uncommon, periprosthetic fracture of the femur after hip arthroplasty is a serious complication that can be difficult to treat. Authors analyzed the types of fracture and modality of treatment. MATERIALS AND METHODS: Between Jan. 1983 and Dec. 1997, 17 cases of postoperative periprosthetic fractures had been treated at our hospital. These cases were followed up for an average of 58 (13-123) months. The fractures were classified according to Duncan and Masri classification. Three cases of type A, and 6 cases of type B1, 3 cases of type B2, 3 cases of type B3 and 2 cases of type C. The 3 cases of type A and 2 cases oftype B1 were conservative treatments. The 4 cases of type B1 were treated with open reduction and internal fixation with bone graft. The 3 cases of type B2 and 3 cases in type B3 were treated with long stem revision. The 2 cases of type C were treated with open reduction and internal fixation with bone graft. RESULTS: Bony union happened in all cases. The results according to Beals and Tower criteria were excellent in 12 cases, good in 3 cases and poor in 2 cases. CONCLUSIONS: The most frequent cause of periprostic fracture was trauma and other causes were loosening and osteolysis. Intramural pregnancy is one of the rarest form of ectopic pregnancy that only 20 cases have been reported in the world so far. Pathologically, the location of the gestational sac is completely isolated from the uterine cavity and entirely surrounded by myometrium without any contact of endometrium. Many reports noted that there would be possible relationship between the intramural pregnancy and prior uterine traumatic factor- curettage, cesarean section, myomectomy, salpingectomy, and manual removal of the placenta and so on. We have recently experienced a case of intramural pregnancy following curettage, managed completely by the laparoscopic excision and additive single dose intramuscular MTX treatment. So, we report this case with a brief review of the literature.

9.Can the Acoustic Rhinometry Reflect the Real Volume Change of the Nasal Cavity?.

BACKGROUND AND OBJECTIVES: Acoustic rhinometry (AR) measures nasal cavity geometry by analyzing reflected acoustic impulses. The authors aimed to find out whether AR could reflect the volume change developed from conchotomy. MATERIALS AND METHODS: To establish the test-retest reliability of the AR, 20 normal nasal cavities were tested with AR before conducting main study. The volumes of the 31 conchotomy specimens were measured with water displacement method (WDM). The nasal volume changes in accordance with conchotomy operations were measured with AR, and the paired values were compared. RESULTS: AR revealed highly consistent results as there was statistically significant correlation between test and retest values (r=0.98, p<0.0001). The volume of the conchotomy specimens measured with WDM was 1.40+/-0.63 cm3 (mean+/-SD) and the volume change measured with AR was 1.49+/-1.48 cm3 (mean+/-SD). There was statistically significant correlation between the two values (r=0.55, p<0.01), though they were not so consistent with each other. CONCLUSION: The nasal volume change after conchotomy measured with AR correlates with the conchotomy specimen volume with statistical significance, though the correlation between them does not always show consistency.

10.Congenital Heart Disease and Extracardiac Anomalies.

Purpose: The diagnostic usefulness of fetal echocardiography in congenital heart disease is well known. One of the indications for fetal echocardiography is presence of extracardiac anomalies and such coexistent congenital heart disease may have important implications for obstetric and neonatal management. MATERIALS AND METHODS: 496 women with singleton pegnancies with amgenital anomalies that underwent penatal care and follow-up in Severance Hospital for 5 years from 1991 to 1995 were evaluated for the incidence of congenital heart disease. RESULTS: The incidence of congenital heart disease in fetuses with the associated extracardiac anomalies were as follows; central nervous system 6.9%(11/160), gastro-intestinal system 10.7%(8/75), genito-urinary sysem 4.7%(4/85), others 5.6%(4/72). Thirty-one of eighty-ane fetuses with congenital heart disease were found to have the associated extracardidac anomalies and ten of them chenoscenal abnormalities. CONCLUSION: We recommend that fetuses with congenital extracardiac anomalies sbould be checked for the presence of congenital heart disease and chmmosomal abnormalities, if needed.
