3.Nutrient Intakes and Physical Fitness by BMI among Middle School Students in Seoul.

Proper nutrient intakes and physical fitness focus one's attention on health promotion. This study was conducted to investigate nutrient intakes and physical fitness by BMI in middle school students. Health-related questionnaire, dietary intakes, food frequency questionnaire and physical fitness were studied. There were no significant differences in health-related questionnaire, food frequency questionnaire and physical fitness by BMI, but several nutrition intakes, preference of salty tastes and the proportion of iron deficiency of female students were significantly different by BMI. Although the female obese group (BMI 85 - 100 percentile) had decreased the proportion of iron deficiency and increased fish intake frequency than other groups, obese females did not show better physical fitmess than other groups. And the average Ca intake was below 75% of Korean RDA in both male and female students. Significantly positive correlation was found between calorie intake and push-up of normal (BMI 15 - 84 percentile) male students (r = 0.29), also significantly negative correlation was found between protein intake and 50m dash of normal female students (r = 0.22). And significantly positive correlation was found between beans and bent knee sit-up of normal male students (r = 0.25), and significantly positive correlations were found between beans, meat, eggs, green & yellow vegetable and sit and reach (r = 0.20, r = 0.28, r = 0.21, r = 0.19) and significantly negative correlation was found between beans and long distance race of normal female students. Therefore, this study suggests that proper nutrition and non-obese states should be achieved for health promotion in middle school students.

4.Assessment of nutritional Status and Factors Related to Smoking in Adolescent Males: 1.dietary Intakes and Nutritional Assessment of Serum Lipids and Antioxidant Vitamins in Adolescent male Smokers.

This study was done to investigate the effect of adolescent smoking on dietary intakes and nutritional status of serum lipids and antioxidant vitamins. Subjects were 82 somkers whose average pack-year was 0.73, and 85 nonsmokers of male technical high school students in Seoul. Anthropometric measurement was performed and % body fat was also analyzed by Bioelectrical Impedance Fatness Analyzer(GIF-891). Dietary intakes and habits were examined through questionnaires and nutrient intakes were analyzed by Computer Aided Nutritional (CAN) analysis programs. Serum TG and total cholesterol levels were measured by Spotchem sp-4410 and serum HDL-cholesterol levels were measured by test kit. serum vitamin C level was measured by 2,4-dinitrophenylhydrazine method and serum levels of vitamin A and vitamin E were measured by HPLC. All data were statistically analyzed by SAS PC package program. There was a significant difference in body fat percentage and systolic blood pressure while other anthropometric measurements were not different between smokers and monsmokers. Caloric intakes(2335 kcal) in adolescent smokers tended to be higher than that of nonsmokers (2,175 kcal) but the difference was not statistically significant. Intakes of protein(76.67g) and niacin(16.49 mg) in adolescent male smokers were significantly higher(P<0.05) than those of nonsmokers although other nutrient intakes were not significantly different. Analysis of serum lipids showed that serum HDL-cholesterol level of smokers was significantly different. Analysis of serum lipids showed that serum HDL-cholesterol level of smokers was significantly lower(p<0.05) than that of nonsmokers, whereas other lipid levels were not significantly different. Serum vitamin C level was also significantly lower(p<0.05) in adolescent smokers than in nonsmokers. In addition, serum vitamin E level, which was 7.85 mg/1 in smokers, was lower than that of 9.20 mg/1 in nonsmokers(p<0.05) while serum vitamin A level was not significantly different between the two groups. These results indicate that cigarette smoking in adolescence decreases serum levels of HDL-cholesterol, vitamin C and vitamin E even thoughth their smoking history is very short.

5.The Value of Tc-99m DTPA Diuretic Renography for Assessment of Dilated Upper Urinary Tract in Children.

PURPOSE: The purpose of this study was to evaluate the accuracy of Tc-99m DTPA diuretic renal scans in children upper urinary tract. MATERIALS AND METHODS: We reviewed diuretic renal scans of 14 pediatric patients (age range: 3 days to 4 years) with unilateral hydronephrosis diagnosed by ultrasonography. Diuretic renal scan was done using Tc-99m DTPA and standardized protocol. In 3 neonates, diuretic renal scans were performed within 1 week and 3-7 months after birth. RESULTS: Six patients required pyeloplasty and eight were managed conservatively. All 6 patients requiring pyeloplasty were diagnosed as having ureteropelvic junction obstruction in the diuretic renal scan. In these 6 patients, post-operative renal scans at 3-12 months after surgery were converted to nonobstructive pattern in 5 and a nonobstructive patterns in 1. In 3 patients who underwent diuretic renal scan within 1 week after birth., nonobstructive patterns of initial scan were coverted to obstructive patterns in the follow-up scan. However, all patients with nonobstructive diuretic renal scans performed after the neonatal period did well on serial ultrasonography and showed favorable clinical outcome without progression to obstruction. CONCLUSION: Tc-99m DTPA diuretic renal scan with standarized protocol is useful in assessing suspected ureteropelvic junction obstruction in children as an initial diagnostic or post-operative follow-up modality. Nonobstructive or indeterminate scan RESULTS in the neonatal period requires follow-up scan to monitor development of the obstructive pattern.

6.Infection Control Preparedness for Influenza A Pandemic (H1N1) 2009 in Healthcare Settings.

BACKGROUND: This study aimed to assess the status of infection control during influenza A pandemic (H1N1) 2009. METHODS: A questionnaire survey was conducted in November 2009 at the 100 hospitals designated by the Korea Centers for Disease Control and Prevention (KCDC) as influenza A pandemic (H1N1) 2009-treatment institutions. RESULTS: In 32.3% of the hospitals, the ventilation system of the influenza A pandemic (H1N1) 2009 isolation ward was separated from the hospital's ventilation system. With regard to wearing personal protective equipment while caring for patients with H1N1 infection, during usual patient contact, masks were always worn by all medical staff at all the hospitals; however, medical staff at 38.7% and 51.6% of the hospitals did not wear gloves and gowns, respectively. During aerosol-generating procedures, some medical staff wore surgical masks, whereas medical staff at 10% and 23.3% of the hospitals did not wear gowns and protective goggles, respectively. In all, 64.5% of the hospitals responded that the contents of the guidelines established by the KCDC were insufficient for reference purposes in actual practice. CONCLUSION: Some of influenza A pandemic (H1N1) 2009-treatment institutions are believed to be inadequate in facilities, infection control during patient treatment, and administrative measures among their efforts to prevent transmission in hospital. In preparation against the outbreak of similar diseases in the future, the government needs to establish guidelines highly applicable by medical staff, to secure exclusive spaces and personnel for treating infectious disease patients, and to develop personal protective gear support and management systems.

7.The Effects of Vit C Supplementation and Nutrition Education on Nutrition Knowledge, Food Habits and Antioxidative Enzyme Activity in Male Adolescent Smokers.

The study evaluated the effectiveness of intervention for male adolescent smokers by making an assessment in terms of changes in food habits, nutrition knowledge, plasma catalase, superoxide dismutase(SOD), glutathione peroxidase peroxidase(GSH-px) activities and thiobarbituric acid reactive substance(TBARS) after Vit C supplementation and nutrition education. The subjects, male adolescent smokers, were assigned into four groups : Control group(19 students), education(Educ.) group(19 students), Vit C supplementation (suppl) group(19 students), and Educ. + Vit C suppl. group(19 students). The Educ. group and Educ. + Vit C suppl. group received nutrition education once a week for 2 weeks. The Vit C suppl. group Educ. + Vit C suppl. group received 500 mg ascorbic acid for 35 days. All data were collected before intervention and after intervention. Nutrition knowledge of those who received education increased, and the frequency of fruit and yellow-green vegetable consumption also increased. Plasma antioxidant enzyme activities were not different except for the SOD activity in the Educ. + Vit C suppl. group, which was significantly increased. The plasma ceruloplasmin level of groups that received Vit C supplementation was reduced more than any other groups, and the specific ceruloplasmin ferroxidase activity of groups that received Vit C supplementation was elevated more than other groups. These intervention programs had an impact on food habits, nutrition knowledge, plasma antioxidant enzyme activities, and plasma TBARS in male adolescent smokers. Various nutrition education programs must be implemented for adolescent smokers, and further studied are needed regarding sorts and amount of antioxidant nutrients and supplementation duration.

8.Influence of Professionalism, Role Conflict and Work Environment in Clinical Nurses with Expanded Role on Job Enbeddedness.

PURPOSE: The aim of this study was to identify the influence of professionalism, role conflict and work environment in clinical nurses with expanded role (CNE) on job embeddedness. METHODS: The participants in this study were 136 CNE working in general hospitals in Seoul, the wider metropolitan area and Gangwon Province. RESULTS: Job embeddedness, work environment and professional performance of the participants showed positive correlation with each other. Significant predictors of embeddeness were belief in public service and sense of calling in the professional subcategory and participation in hospital affairs and nurse-doctor relationship in the work environment subcategory. CONCLUSION: Finding from this study indicate the need to evaluate and improve the significant predictors of job embeddedness for CNE.

9.Report of two cases of double outlet left ventricle

Double outlet left ventricle (DOLV), even defined as origins of both great vessels from left ventricle,implies varieties of morphology in concern with loop pattern, location of ventricular septal defect, conotruncalanatomy, and presence or adsence of pulmonary stenosis. With the improvement of surgical tenchnique, correctivesurgery of DOLV became possible, and preoperative diagnosis and determination of the accurate anatomicalrelaltionship between great vessels, ventricular septal defect, and ventricles is very important in surgicalimplication. Authors have recently experienced two cases of DOLV. The one is with subpulmonic conus, subpulmonicventricular septal defect, patent ductus arteriosus, and preductal type coarctation of aorta. The other is withbilateral coni and subaortic ventricular septal defect. Loop patterns are [S,D,D] in both cases.
