1.Response and Toxicity of Chemotherapy in 78Cases of Malignant Ovarian Tumors.

For evaluation of the response and toxicity of the combination chemotherapy, forty six patients with malignant ovarian tumors who had prior surgery were treated with combination chemotherapy from January 1985 to March 1991 at the Department of Obstetrics and Gynecology, Kyung-pook National University Hospital. The results were as follows : 1. The responses were in complete 20 cases(43.5%), in partial 9 cases(19,6%), in stable 6 cases(13.0%), in progressive 11 cases(23.9%) among 46 patients. 2. By the response rates of various combination chemotherapy regimens, the response rate of CAP was 64%(16/25), CP 57.1%(8/14), VAC 100%(3/3), VBP 50%(1/2), FAM 0%(0/1), and Melphalan 100%(1/1) respectively. 3. As the chemotoxicities of combination chemotherapy, leukopenia 20 cases(46.5%), thrombocytopenia 2 cases(4.7%), anemia 20 cases(46.5%), nephrotoxity 6 cases(14.6%), hepatotoxicity 7 cases(18.4%) were observed.

4.Emergency ultrasonography of the gastrointestinal tract of children.

Ultrasonography (US) is widely used to evaluate most body parts in pediatric patients because it is simple, noninvasive, easy to use, and applicable in a wide variety of clinical situations. US is the modality of choice for the initial evaluation of acute abdominal pain in pediatric patients because of their small body habitus and the presence of less fat tissue in the abdominal wall and peritoneal cavity. The most important factor in performing pediatric emergency US is the graded compression technique and selection of the proper transducer. The careful evaluation of bowel wall thickening combined with ancillary findings such as fluid collection, increased echogenicity of the mesenteric fat tissue, enlarged lymph nodes, hyperemic bowel changes, and abnormal bowel peristalsis can increase the diagnostic capability of US.

Purpose:
This study aimed to determine whether zero echo time magnetic resonance imaging (ZTE-MRI), as an alternative imaging modality, and conventional computed tomography (CT) have similar diagnostic qualities for assessing pediatric osseous pathologies.
Materials and Methods:
Twenty-six sets of pediatric musculoskeletal CT and MRI scans (15 boys and 11 girls; mean age, 12 ± 4 years; range, 5–23 years) acquired at Seoul National University Children’s Hospital (January 2021 to November 2023) were retrospectively evaluated. CT-like images from ZTE-MRI were generated using grayscale inversion. Two radiologists independently assessed ZTE-MRI image quality (S anat) on a 5-point scale (1 = nondiagnostic, 5 = excellent) and a comparative scale (–2 = CT greater, 0 = same, 2 = ZTE-MRI greater) for lesion delineation (Scomp). The confidence interval of proportions and intraclass correlation coefficient were calculated to assess inter-rater agreement, and Wilcoxon rank-sum test, Mann–Whitney U test, or paired t-test was used to compare image quality or cortical thickness between the modalities.
Results:
ZTE-MRI demonstrated diagnostic quality (S anat ≥ 3) in 85%–96% of the cases, 89%–96% for cortical delineation, 92%–100% for intramedullary cavity (IMC) delineation, and 92% for lesion delineation. Compared with conventional CT, ZTE-MRI showed comparable diagnostic power (Scomp ≥ –1) in 92%–96% of the cases, with Scomp scores indicating no significant difference in lesion delineation (p = 0.53 in reader 1 and p = 0.25 in reader 2). There was a preference for CT over ZTE-MRI in terms of overall image quality and delineation of the cortex and IMC (p < 0.001). Cortical thickness was not significantly different (p = 0.11) between ZTE-MRI and CT.
Conclusion
ZTE-MRI demonstrated diagnostic quality comparable to that of CT, particularly in lesion delineation. In addition to the unique information that conventional MRI can provide, ZTE-MRI can provide additional information about osseous structures similar to that provided by CT, which we believe will be valuable in the future.

8.Relapse Rates of Ulcerative Colitis in Remission and Factors Related to Relapse.

BACKGROUND/AIMS: Ulcerative colitis (UC) is a chronic inflammatory bowel disease with heterogeneous clinical features. Data on the disease course and prognosis of UC patients who have been regularly treated are lacking. We aimed to investigate relapse rates of UC in remission and factors related to relapse. METHODS: We retrospectively analyzed clinical courses of 84 patients (43 males, median age 43 years, ranged 20-73 years) diagnosed as UC at Ajou University Hospital between January 1997 and December 2005 based on clinical, endoscopic and pathologic findings, and who were regularly followed for at least one year after the remission. RESULTS: Study subjects consisted of 32 proctitis (38%), 21 left-sided colitis (25%), and 31 subtotal or total colitis (37%). Of 84 patients, relapse was observed in 52 patients (62%) during the follow-up period (ranged 1-9 years). The relapse rate was 24%, 41%, 51%, 65%, 71%, and 79% at 1 year, 2 years, 3 years, 4 years, 5 years and 6 years, respectively. Among sex, age, hemoglobin, ESR, and the extent of disease on admission, decrease of hemoglobin level was the only independent factor related to relapse (odds ratio=2.67, 95% CI (1.32-5.42), p<0.01). CONCLUSIONS: In Korea, relapse of UC in remission is not rare. Decrease of hemoglobin level is an independent risk factor related to its relapse, while the extent of disease is not.

Intramural pregnancy is one of the rarest form of ectopic pregnancy that only 20 cases have been reported in the world so far. Pathologically, the location of the gestational sac is completely isolated from the uterine cavity and entirely surrounded by myometrium without any contact of endometrium. Many reports noted that there would be possible relationship between the intramural pregnancy and prior uterine traumatic factor- curettage, cesarean section, myomectomy, salpingectomy, and manual removal of the placenta and so on. We have recently experienced a case of intramural pregnancy following curettage, managed completely by the laparoscopic excision and additive single dose intramuscular MTX treatment. So, we report this case with a brief review of the literature.

10.The Change of Cardiothoracic Ratio after Ligation of Patent Ductus Arteriosus in Adult: Report of 30 cases.

BACKGROUND: Treatment of adult patent ductus arteriosus(PDA) has many difficulty such as pulmonary hypertension, arterial wall calcification, aneurysmal chage of ductus. We tried to evaluate the effect of surgical interruption of PDA on postopeative change of heart size. MATERIAL AND METHOD: From 1987 to 1997 we experienced 30 cases of the ligation of patent ductus arteriosus in adult at the department of cardiovacular surgery in Masan Samsung General Hospital. RESULT: There were 9 males and 21 females and their ages ranged from 16 to 44 years, with a mean age of 26.1 years. 15 patients had pulmonary hypertension and 9 patients complained of dyspnea with a degree of functional NYHA class III. Operation method was double or triple ligation using Teflon felt. Postoperative complications were wound dehiscence in 3 patients and transient hoarseness in 1 patient. The mean preoperative cardiothoracic ratio was 54.7%, and mean postoperative cardiothoracic ratio was 51.9%. The change of cardiothoracic ratio was more typical in the cases who had congestive heart failure. Their mean preoperative cardiothoracic ratio was 64.8% and the mean postoperative cardiothoracic ratio was 58.5% there was no postoperative deaths. CONCLUSION: We canclude that the improvement in cardiothoracic ratio may result from surgical intervention of PDA.
