1.Myxoma of the Breast: A case report.

A case of myxoma of the breast was reported. A 25-year-old woman showed a lump in the left breast which was suddenly enlarged after the delivery of a baby. A tumor measuring 73.53cm was located in the upper outer quadrant of the breast. Fine needle aspiration cytology revealed scattered benign spindle shaped mesenchymal cells with abundant myxoid material. On excision, the tumor was partly solid, partly cystic and multilocular. Each nodule consisted of an abundant mucoid material with a few spindle or stellate mesenchymal cells. Pleomorphism and mitosis were absent. The mesenchymal cells showed immunoreactivities for S-100 protein, smooth muscle actin, and CD34. The myxoid stroma was positively stained with alcian blue and mucicarmine. These findings suggest that constituent cells derive from totipotential primitive mesenchymal cells.

2.Radiological evaluation of ventricular septal defect with aortic insufficiency: an analysis of cineangiographyin 15 cases

Fifteen cases of ventricular septal defect with aortic insufficiency were diagnosed radiographically andconfirmed after operation at Seoul National University Hospital in recent two and half years since 1979.Cineangiographies of ascending aorta and left ventricle were done in those cases and revealed some characteristicfindings. The resuls of the analysis are as follow; 1. Among the 15 cases, 14 cases were male and 1 case wasfemale. Age distribution was from 7 years to 23 years. 2. Those 15 cases were corresponded to 8% among total 193cases of ventricular septal defect, to 11% among total 135 cases of aortic insufficiency and especially to 48%among 48 cases of aortic in sufficiency below age of 20 years. 3. After operation, 11 cases were confirmed assubpulmonary type ventricular septal defect and 4 cases as subcristal type. The sizeds of the ventricular septaldefects were ranged between 0.6 and 2.5cm in diameter. 4. Regurgitation of contrast media was noticed in cineaortography of all cases, and the grades of regurgitation were II-III/IV in 13 cases. 5. Various types ofherniated aortic cusp through ventricular septal defect were seen. In the cases of subpulmonary ventricular septaldefect characteristic saccular aneurysm was found in 7 cases. Asymmetry or mild bulging of aortic sinus was foundin the cases of subcristal ventricular septal defect. 6. Infundibular stenosis was found in 3 cases with rightventriculography and those were caused by the herniated saccular aneurysm of aortic cusp. 7. It is essential forthe diagnosis of ventricular septal defect with aortic insufficiency to undertake biplane cineangiography ofascending aorta and left ventricle in long axial view and right ventriculograhy should be done in suspicion ofinfundibular pulmonary stenosis.

3.Plain chest PA evaluation of left atrial thrombosis in mitral valvular disease

Conventional posteroanterior chest radiographs of 73 patients with mitral valve disease who had surgery were analyzed, and particular attention was directed to the absence of the convexity of the left lower mid cardiacborder (left atrial segment) and to the relation with duration of illness, atrial fibrillation, pulmonary capillary wedge pressure (PCWP) as well as surgical findings. The results obtained are as follows; 1. The flatness or concavity of this segment, despite other evidence of left atrial enlargement, was observed in eleven (64.7%) of17 patients who had left atrial thrombosis and six(10.7%) of 56 patients who did not have thrombosis(false positive diagnosis). Six (35.3%) of the 17 patients who had left atrial thrombosis did not show this finding on radiographs(false positive diagnosis). Six(35.3%) of the 17 patients who had left atrial thrombosis did not showthis finding on radiographs (false negetive diagnosis). Therefore, the accuracy in the diagnosis of thrombosis of left atrium was 64.7%(eleven of seventeen) for patients who had thrombosis of left atrium. 2. The atrial fibrillation was observed in sixteen (94.1%) of 17 patients who had left atrial thrombosis. So, correlation between the presence of left atrial thrombosis and atrial fibrillation was evident. But there was no correlationbetween the duration of illness or the PCWP of patients and the presence of left atrial thrombosis.

4.Hepatic and postrenal segment anomalies of inferior vena cava

Postrenal segment anomalies of inferior vena cava such as bilateral inferior vena cava and left-sided inferiorvena cava can simulate lymphadenopathy on CT scan and these anomalous veins need consideration in retroperitonealoperations such as procedures for prevenetion of venous embolism, splenorenal shunt operation and aorticprosthetic repalcement. Retrocaval ureter is a rare cause of obstructive uropathy or medical deviation of ureter.We analysed 16 cases of postrenal segment anomalies daignosed by CT, vena cavography, retrograde pyelography andultrasonography including six rare positional anomalies at hepatic segment of inferior vena cava diagnosed bycardiac angiography. The results were as follows. 1. Postrenal segment anomalies were 6 cases of bilateral IVC, 8cases of left-sided IVC and 2 cases of retrocaval ureters. On CT scan, 3 cases of bilateral IVC and 4 cases ofleft-sided IVC were accompained by malignant tumors, but caval veins could be discriminated from enlarged nodesbecause of continuous tubular nature of vein on consecutive sections with homogeneous strong enhancement. Twocases of retrocaval ureters showed hydroureteronephrosis due to ureteral compression by IVC. 2. Hepatic segmentanomalies were 6 cases. Five cases of IVC on left side of vertebral crossed midline at live to enter right-sidedright atrium and one case of IVC on right side crossed midline to enter left-sided right atrium. Four cases ofcomplex cardiac anomalies, 4 cases of asplenia and 2 cases of situs ambigus were associates with these anomalies.

6.Magnetic Resonence Imaging of Anomalous Pulmonary Venous Connections.

PURPOSE: We evaluated the capability of MR in the diagnosis of anomalous pulmonary venous connection (APVC). MATERIALS AND METHODS: The patient group consisted of 11 total APVC and 8 partial APVC diagnosed with MR. Echocardiography was performed in all cases, cardiac angiography in 12 cases and operation in 12 cases. We compared MR findings with those of operation, echocard iography and cardiac angiography. RESULTS: In surgically proven 12 cases, diagnostic accuracy of preoperative MR, echocardiography and cardiac angiography was 100%, 67% and 63%, respectively. In the remaining cases, MR findings well correlated with those of echocardiography or cardiac angiography. Stenosis of common pulmonary vein or superior vena cava was identified in 4 cases. In one patient, MR depicted associated cor triatriatum clearly. CONCLUSION: MR is an effective modallty in depicting anomalous pulmonary venous con nections.

7.Infrahepatic interruption of inferior vena cava

Congenital anomaly of IVC is rare, but understanding of this anomaly is important in radiological diagnosis,angiographic procedures and major retroperitoneal and thoracic surgery. We analysed 23 cases of IVC interruptiondiagnosed by cardiac angiography at Seoul National University Hospital. The results were as follows: 1. Theincidence of infrahepatic interruption of IVC was 0.45% of the patients having cardiac angiography and mostpatients showed cyanosis(91%). 2. The most common associated cardiac anomaly was right ventricular outflow tractobstruction (60%). Other associated cardiac defects were VSD, ASD, valvular anomaliesin 9 cases(39%) respectively;double outlet right bentricle, bilateral superior vena cava, single ventricle in 6 cases(26%) respectively; singleatrim, PDA in 5 cases(22%) respectively. 6 cases of situs inversus, 3 cases of situs ambiguus,2 cases of visceralheterotaxia and one case of asplenia were observed also. 7 cases of left-sided IVC were associated with IVCinterruption in normal situs.

8.Clear Cell Sarcoma of the Kidney: Report of two cases.

Clear cell sarcoma is a rare malignant rumor of the kidney which occurs in children and is differentiated from Wilms' tumor by its different clinicopathologic features and natural history. Previous studies indicate that this tumor may be of mesenchymal cell origin; however, this has not been proven conclusively. Further accumulation and study need to be conducted in order to clarify the histogenesis of this tumor. We report two cases of clear cell sarcoma of the kidney which occurred in a 2 and a half-year old and a 2-year old boy. This report places special emphasis on the clinicopathologic characteristics of these two cases including electron microscopic and immunohistochemical findings. Attempts were also made to differentiate the clinicopathologic aspects of clear cell sarcoma from Wilms' tumor and speculate on the histogenesis of this rumor.

9.CT Findings of Ureteral Metastases.

PURPOSE: To evaluate CT features of metastatic ureteral tumors. MATERIALS AND METHODS: CT findings in 16 patients with ureteral metastases were evaluated retrospectively ;there were eight cases of bilateral ureteral involvement. Primary tumors metastatic to the ureter were advanced gastric cancer (n=13), breast cancer (n=l), colon cancer (n=l), or adenocarcinoma of unknown primary (n=l). We analysed CT findings with regard to the site of ureteral obstruction, configuration of obstructed sites of ureter, presence or absence of periureteral soft tissue density, and status in other organs. RESULTS: Among 24 ureters involved, ureteral wail thickening was noted in 15, periureteral soft tissue density in 13. Small nodular enhancing lesions less than lcm, within the retroperitoneal space around the involved ureters were noted in seven patients, and four of them were multiple lesions. CONCLUSION: Among various primary tumors, gastric cancer was the most common cause of ureteral metastasis. The common CT findings of ureteral metastases were thickening of ureteral wall, periureteral soft tissue density, and small periureteral enhancing nodular lesions. The constellation of these CT findings may be helpful in making the diagnosis of ureteral metastases.

10.Necrotizing Pneumonia: CT Findings & its Clinical Significance.

PURPOSE: To analyze CT and follow-up chest radiographic findings in patients with necrotizing pneumonia and to evaluate clinical significance of the extent of necrosis. MATERIALS AND METHODS: We reviewed medical records and retrospectively analysed CT scans and follow-up chest radiographs of 22 patients with necrotizing pneumonia, confirmed by biopsy(n=7) and culture (n=15). Inclusion criteria for necrotizing pneumonia was necrotic low attenuation, with or without cavitation on postcontrast enhanced CT scan. The study group included 15 men and seven women, aged 11-66 years (average: 47years). RESULTS: The pathogens of necrotizing pneumonia were Klebsiella spp(n=7), Enterobacter spp(n=5), Actinomyces spp(n=4), Pseudomonas spp(n=4), Nocardia spp(n=4), and others(n=5). Average duration of pneumonia was 4.1 months. On CT scan, pneumonic consolidations were well-marginated in 14 patients and there were cavities on initial CT scan in 16 cases. Margins of the necrotic portion on CT scan were well-demarcated in majority of the patients(16/22). Low attenuation areas on initial CT scan resulted in cavitation, fibrosis and volume loss as shown on follow-up chest radiographs. The larger the necrotic areas on CT, the more the volume loss was. CONCLUSION: CT findings of necrotizing pneumonia were well-marginated air-space consolidation with low attenuation area, with or without cavity. The extent of necrotic area was closely related with the degree of fibrotic change later on. CT is important tool for diagnosis and prediction of parenchymal damage in necrotizing pneumonia.
