2.Anterior impingement of the Ankle Treated by Arthroscopic Removal of bony Spur

From September 1992 to April 1994, we reviewed 7 patients after arthroscopic surgery for anterior impingement in the ankle, who complained ankle pain and limitation of motion. All patients were mem and main symptoms were severe pain at full dorsiflexion of ankle and limitation of motion. Bony spur was located on the anterior aspect of tibia, and it was possible to resect the tibial bony spur arthroscopically without distraction devices. Mean operation time was 55 minutes. Pain, swelling and limitation of motion were much improved. There was no postoperative complication. Arthroscopic resection of the anterior tibial bony spur is an effective treatment for anterior impingement in the ankle.

3.Subacute Hematogenous Ostemyelitis in Children

Subacute hematogenous ostemyelitis is mostly likely due to an altered relationship between host and pathogen, associated with increase host resistance, decreased virulence of the causative organism and/or antibiotics modification. In a retrospective study of the hematogenous ostemyelitis in 156 cases, 19 cases of them were the subacute form of the ostemyelitis. Pain is the most consistent symptom and most of the constitutional symptoms were usually mild. However, the ESR was often elevated. From our data, the following results were obtained. 1. The most common skeletal involvement was the distal part of the tibia(8 out of 19 patients). 2. The causative organism was staphylococcus aureus by the bacterial culture of the specimens. 3. In the literature, acute hematogenous ostemyelitis rarely cross the physis in patients older than 18 months of its age. However, subacute ostemyelitis frequently does cross the physis(5 out of 19 patients). 4. Cross physeal lesions were healed without any permanant damage to the growth plate in our cases(5 out of 19 patients). 5. All patients except for one case were treated by curettage and antibiotics. 6. The mush higher incidence of the cross physeal lesions may be expected by MRI in identifying the lesion. 7. Seven out of 19 patients had radiographic findings similar to eosinophilic granjuloma, Ewing's sarcoma or osteosarcoma.

4.A Study on the Practical Behavior of Health Care Managers at the Worksite.

This study was conducted to find out the characteristics, type of the duty and job satisfaction of health care managers at the worksite. The subjects of this study were 306 health care managers serving at worksite situated in Kyungin presently in 1994, concerning their job. Major depedent variables were health management, environmental management, and health planning related with health care managers' job; these variables were measured by nominal scale consisted of 9 items respectively. The collected data were analysed and some major results are as follows; 1. It appeared that according to the qualification of health care managers, nurses showed high performing rate in duty of health managerment and health planning, Industrial hygienist showed high performing rate relating to working environmemt managemant, and regarding their holding an additional post, the number of environmental managers held more an additionnal post than other qualified managers, thereby showing significant relatively, and however, no relation with qualification was there in decision making. 2. The performing rate relatiag to health management was significantly correlate with female, youth, unmarried, higher education, general affairs in post of duty, general employee in position, only duty of health, low monthly income and nurse. But, the performing rate relating to environmental management was significantly correlate with manufacturing industries, industrial health and safety committee, male, old age, married, long work duration, additional post, high monthly income, and hygienist and environmental managers. The performing rate relating to health planning was significantly correlate with the performing rate relating to health and environmental management, but the performing rate relating to health management was not significantly correlate with the performing rate relating to environmental management. 3. In multiple regression analysis, common major determinants of the performing duty relating to health management, environmental management and health planning were industrial health and safety committee, type of industry, education, and qualification of industry's health care managers. Additionally, age was selected as highly correlated with the performing duty relating to environmental management, and work duration and post of duty of health care managers in industry were selected as highly correlated with the performing duty relating to health planning, Based on the results indicated above, it could be understood that characteristics of the worksite-industrial health and safety committee, scale and types of industry, and characteristics of health care managers-age, education, post of duty, work duration and qualification had relations with their type of the duty. Therefore, it is required that the reference to the above-showing results such as guiding for them to precisely grasp their performing duty according to their qualification, heightening their participation in decision-making, extending the industrial health service, etc., operation and management of health care managers at the worksite should be made into being more efficient.

5.Buschke-Ollendorff Syndrome: A Case Report

The osteopoikilosis is commonly known as harmatoma in which metaphyseal and epiphyseal area of long bones and the bone of pelvis, hands, feet and et al, contain islands of dense cortical bone with normal Harversian system without any symptoms. The radiologic findings of the osteopoikilosis is multitude of oval or well-circumscribed areas of increased density, 2 to 10 mm in size, in symmetrical distribution and normal uptake in bone scan. Buschke-Ollendorff syndrome, which is transmitted by autosomal dominant, is characterized by the association of osteopoikilosis and connective tissue nevi which are popular and symmetrically distributed lesions on chest, back, buttock, thigh or arm. Authors report three Buschke-Ollendorff syndrome, studied by CT scan, MRI, bone scan and bone and skin biopsy, among five patients associated with osteopoikilosis found by simple radiologic study from ten members in one family with their pedigree.

6.Factors Influencing Final Visual Acuiry After Penetrating Ocular Infuries.

We analyzed the records of 124 eyes of 124 perforating ocular injury patients retrospectively from June 1 1994 to examine the relationship between final visual acuity and preoperative factors such as age, sex, initial visual acuity, location of injury, length of injury, mechanism of injury, intraocular foreign body, traumatic catarcact, secondary operation, hyphema, vitreous hemorrhage. The preoperative factors that influence final visual were age, initial visual acuity, length of infury, vitreous hemorrahage, secondary operation. However, other factors such as gender, mechanism, location of injury, intraocular foreign body, traubatic cataract, and hyphema did not influence final visual acuity. Final visual acuity was better in the case of young patients, good initial visual acuity were age, initial visual acuity, length of injury, vitreous hemorrhage, secondary operation, However, other factors such as gender, mechanism, location of injury, intraocular foreign body, traumatic cataract, and hyphema did not influence final visual acuity. Final visual acuity was better in the case of young patients, good initial visual acuity, short wound length, absence of vitreous hemorrhage and secondary operation.

7.The effects of age adjustment on the diagnosis of noise induced hearing loss.

In Korea there is no specific method of age adjustment in noise induced hearing loss(NIHL). we attempted the age adjustment to understand the effects of age on the diagnosis of NIHL. We used the International Standard Organization 1999 as an age adjustment method. We used the 1,617 otologically normal person's hearing data from a health examination center, and 206 workers diagnosed as NIHL. we concluded as follows; 1. The ISO 7029 function used for age specific hearing loss is not suited to Korean people. 2. The mean of age specific hearing loss is 11.0 6.2dB, and the older of age, the more decrease on hearing loss, especially in 4000Hz. 3. The difference of NIHL between before age adjustment and after age adjustment in the 3rd decade is 5.4dB, in the 4th decade is 6.7dB, in the 5th decade is 8.5dB, in the 6th decade is 10.4dB, and in the 7th decade is 12.9dB. The older, the more is the difference. 4. After age adjustment, the number of workers diagnosed as NIHL decreases to 60% of unadjusted.

8.Expression of Vascular Endothelial Growth Factor in Human Ovary.

OBJECTIVE: The ovarian cycle is characterized by repeating patterns of cellular proliferation and differentiation that accompany follicular development and the formation and regression of the corpus luteum (CL). That angiogenesis may play an important role in this process. Angiogenesis is supposed to be regulated by vascular endothelial growth factor (VEGF). The goal of the present investigation, therefore, was to determine whether the expression of VEGF was changed in the normally cycling human ovary. We also investigated VEGF expression in the regressed CL (ie, nonfunctiong CL) of normal term pregnancy to define the association with steroidogenic activity. To our knowledge there is no report available on VEGF expression in the CL of term pregnancy. METHODS: We assessed VEGF expression in ovaries obtained from, 26-42 yr of age, and from patients undergoing hysterectomy and salpingo-oophorectomy for nonendocrinological or nonovarian disorders. Tissue samples from premenopausal women included specimens from follicular (n=4) and luteal (n 4) phases. In addition, we studied ovarian specimens from pregnant women (n=3). Immunohistochemical analysis for VEGF was performed using a rabbit polyclonal antibody directed against human VEGF. RESULTS: These data demonstrate a development-related VEGF expression in the follicle and indirectly show that VEGF expression may be up to the existence of LH-receptor. And also, VEGF was overexpressed in the regressed CL of pregnant women compared with the functioning CL of nonpregnant cycles CONCLUSION: This study suggests that the intensity of VEGF expression is not correlated with steroidogenic activity, although both of them are stimulated by LH.

9.Quantification of Retinal Nerve Fiber Layer Thickness in the Normal Subjects Using Optical Coherence Tomography.

Optical coherence tomography is a new imaging technique analogous to ultrasound B scan that can provide cross sectional images of retina with approximately 10micrometer resolution. This study was designed to evaluate the quantification of nerve fiber layer [NFL] thickness in normal eyes using OCT. We studied 66 eyes of 66 subjects by conventional ophthalmoscopic physical examination, Humphrey central 30-2 visual fields, stereoscopic optic nerve head photography, retinal nerve fiber layer photography, HRT and OCT. Imaging with OCT was performed circumferentially around the optic nerve head with a circle diameter of 2.3mm & 3.4mm using an internal fixation technique. At a scan diameter of 2.3mm and 3.4mm, average, superior, inferior, nasal and temporal NFL thicknesses were 125.45 +/-19.05 micrometer, 150.33 +/-25.53micrometer, 154.55 +/-25.72 micrometer, 102.89 +/-29.13micrometer, and 92.65 +/-27.04micrometer and 109.02 +/-15.68 micrometer,135.53 +/-22.65 micrometer, 132.98 +/-20.19micrometer, 73.11 +/-26.85 micrometer and 89.70 +/-14.65 micrometer each. The NFL was thickest in the superior and inferior quadrants,thinnest in the nasal quadrant at a scan diameter of 2.3mm, and thinnest in the temporal quadrant at a scan diameter of 3.4 mm. In general the thickness for the 2.3 mm scans was greater than that for the 3.4mm scans.NFL thickness decreased significantly with age in the temporal quadrants [P<0.05]. At a scan diameter of 3.4 mm, NRA decreased significantly with decreased average NFL thickness and average NFL thickness decreased significantly with decreased disc area [P<0.05]. By direct investigation of the NFL with OCT, earlier diagnosis of glaucoma and earlier detection of glaucomatous progression can be made, before conventional signs such as visual field loss, cupping of the optic nerve head, and NFL defects are evident, thus allowing earlier treatment and reducing the damage.

10.Re-evaluation of Association between Thrombotic Diasthesis and Legg-Calve-Perthes Disease.

Legg-Calve-Perthes disease (LCP) has been reported to be associated with hypofibrinolysis and a deficiency in coagulation. The goal of this prospective study was to confirm or refute these findings and to establish a guide for a screening test. The coagulation systems of twenty-three patients with Legg-Calve-Perthes disease were evaluated by means of the tests which included prothrombin time, activated partial thromboplastin time, antithrombin-III (AT-III), protein C, protein S, lipoprotein(a), and anticardiolipin antibody (ACA). Study subjects were 21 males and 2 females. Mean age was 8.7 years. No abnormal results were found for Antithrombin-III and Protein S, but 2 patients had low Protein C level. Two of 23 patients had Lipoprotein(a) values greater than 30 mg/dl. Another two patients had positive finding for Anticardiolipin antibody. There were no statistical differences in all parameters between Catterall group stage in the patients with Legg-Calve-Perthes disease. We were unable to establish an association between thrombotic tendency and Legg-Calve-Perthes disease based on this prospective study. So, routine screening of patients with Legg-Calve-Perthes disease for abnormalities of antithrombotic factors would be not warranted.
