6.Effects of Trans Pars Plana Vitrectomy for Fibrous Membrance in Chronic Uveitis.

The complications of peripheral or posterioruveitis, such as vitreous opacity, epiretinal membrance, and tractional retinal detachment may need vitreous surgery. This study was undertaken to evaluate the efficacy of transpars plana vitrectomy in the case of epiretinal membrance or tractional retinal detachment secondary to chronic uveitis.We included 23 eyes of 21 patients, and among them there was 16 eyes of epiretinal membrance and 7 eyes of tractional retinal detachment respectively. Improvement of vision, 2 lines or more in Anellen Chart, was noted in 10 eyes(63%) for epiretinal membrance and 2 eyes(29%) for tractional retinal detachment within post-operative 1 month. Final visual acuity decrease, compared with preoperative vision, was noted in 4 eyes(25%) for epiretinal membrance and 6 eyes(86%) for tractional retinal detachment with a mean follow-up of 34 months. The most common cause of visual acuity decrease was recurrent or persistent inflammation and resultant fibrous tissue proliferation, which became obvious within post-operative 3 months.Therefore, we`re able to conclude that the main cause of postoperative visual acuity decrease is persistent intraocular inflammation and fibrous change. So it is necessary to remove posterior hyaloid membrance actively and combine with medical treatment like corticosteroid therapy in order to control the inflammation even after vitreous surgery.

7.Clinical Results of Modified Ab Externo and One-Knot Technique.

PURPOSE: To introduce a more efficient and time-saving scleral fixation technique for a posterior chamber foldable intraocular lens and to report the clinical results. METHODS: A foldable acrylic 3-Piece IOL was sutured to the sclera via a small corneal incision. The guiding hollow needle was not used, which differs from other ab externo techniques. Instead, the curved long needle was directly pulled out through the cornea. The scleral flap was not used to bury the scleral suture knot; Instead, the scleral suture knot was translocated at the temporal area. RESULTS: A total of 30 cases were studied retrospectively. The mean follow-up period was 2 years. The IOL was correctly positioned in all cases. There were no major complications, such as endophthalmitis or suture erosion through the conjunctiva. CONCLUSIONS: Based on the authors' experience, the modified ab externo and one-knot technique is a faster and easier technique than the conventional ab externo technique.

9.Percutaneous Transluminal Angioplasty of a Stenosis of an Internal Mammary Artery Graft.

Successful percutaneous transluminal angioplasty (PTA) of an internal mammary artery graft was performed in a 40 year old male patient who had recurrent angina soon after bypass surgery. Coronary angiography showed total occlusion of proximal portion of the left anterior descending artery and normal right coronary artery. Angiography of the left internal mammary artery graft revealed a tight stenosis (90% diameter narrowing) in the mammary artery at its insection into the left anterior descending artery.Angiography after the angioplasty demonstrated a widely patent graft (residual stenosis 10%) and translesional pressure gradient was 10 mmHg.
