3.The Effect of an Oral Contraceptive (Eugynon) Upon Lactation.

There are a number of published reports aimed at clarifying the effect of hormonal contracept -ives upon the quantity and quality of breast milk during postpartum and lactation periods. As to the volume of milk produced by breast feeding mothers, many authors seem to have agreed on a decrease as an established pattern in the periods following regular administration of oral contraceptives containing estrogen in varying dosage. The quality of milk following administration of hormonal contraceptives, however, remsins a controversial issue. Korea's experience in oral contraceptive is rather a brief one, but use of Eugynon has been widespread and frequent since this hormonal contraceptive was introduced into the national program in 1967. The authors have reviewed the potential significance of data concerning regular use of an oral contraceptive as affecting lactation, and have sought to clarify the interrelationship between the administration of Eugynon and quantity and quality of breast milk from mothers contracepting. with Eugynon in different periods following confinement. A total of 85 women, who were at different periods following deliveries, have been divided into, two groups, one comprising 28 women regularly taking oral pills and the other (57 women) not resorting to hormonal contraceptives if they were contracepting at all. 1. Milk Volume. In view of the possible influence of suckling on the amount of milk produced, efforts were made in this study to standardize the technique by extracting breastmilk by applying manual pressures on one side of the maw-nae, While the nipple on the other side was being sucked by the mother's own baby. The effetct of an exogenic ovarian hormone on the quantity as well as quality of breastmilk is generally understood to be inapparent until the drug is administered to women whose milk secreting function has been normalized. ") In the present study, it was observed that the decrease in the ammount of milk obtained from mothers in the periods following the 4th cycle of oral contraceptives or thereafter has turned out to be statistically insignificant. This result conforms i.u the findings by Tubari and others. It is assumed that it takes at lest 2 to 3 cycles of use before mammary glands are functionally adjusted to the use of exogenic hormonal contraceptives. 2. Specific Gravity and Composition of Milk There was no noticeable change in the protein and chloride content following continuous administration of ore.l contraceptives, while meaningful changes were observed in fat (increase) and calcium (decrease up to the 5th cycle use) contents. Also, there was a rather significant decrease in the specific gravity in the period following administration of the first cycle of the oral contraceptive. The findings from the present study partially conforms the results published by Ramadan and others, who reported that little change was noticed in the contents of total solids. ash, chlorides and lactose in the breast milk of women who had taken 4 cycles of ovosiston, although in our study lactose was not measured. Ramaden, however, reported that fat content did increase in the same milk, as in our study. A definitive conclusion, however, could not be made unless measures are taken to rule out the physiological changes of the maternal body affecting the composition of milks.

4.Lateral Lithotomy Position for Simultaneous Retrograde and Antegrade Approach to the Ureter.

PURPOSE: We applied lateral lithotomy position to the severe ureteral stricture cases supposed to fail with only retrograde approach. MATERIAL AND METHODS: From October 1997 to April 1999, 13 patients with severe ureteral stricture (lenghth>2cm or complete obstruction) supposed to fail with only retrograde approach and one patient with study. The causes of ureteral strictures were pelvic malignancy in 5, tuberculosis in 4, trauma in 2 and others in 2. The patient's ipsilateral shoulder was rotated and fixed like lateral position. And ipsilateral pelvis was elevated with sandbag or pad and rotated about 45 degrees, too. The retrograde approach was tried at first, if fail, antegrade approach was combined. RESULTS: We could insert ureteral stent via retrograde approach only in 3 patients and we needed aid of antegrade approach for passage through ureteral stricture in the other 11 patients (79%). Percutaneous antegrade approaches were combined in those 11 patients and we could pass the guide wire and indwell the stent in 10 of 11 patients (91%) using this position. CONCLUSIONS: The lateral lithotomy position was very helpful to the simultaneous retrograde and antegrade approach in severe fibrotic or malignant ureteral strictures.

8.Clinical Study for Retinopathy of Prematurity.

We studied 249 patients with risk factors of retinopathy of prematurity and the results were as follows: 1. Among 249 patients, retinopathy of prematurity(ROP) developed in 36 patients(14.5%). 19 patients(528%) were stage 1, 4(11.1%) patients were stage 2, 11 patients(30.6%) were stage 3, 2 patients(56%) were stage 4 and there was no stage 5 ROP patient. 2. In gestational age, there was not statistically significant difference between normal lnfants and under-prethreshold stage group, but there were statistically significant differences between normal or under-pretheshold stage group and pretheshold or theshold stage group and was not stastically significant difference between prethreshold stage and threshold stage. 3. In birth weight, there was statistically significant difference between normal group and under-prethreshold stage group and also be statistically significant differences between normal or under-prethreshold stage and pretheshold or threshold stage group but there was not statistically significant difference between prethreshold stage and threshold stge. 4. In duration of oxygen therapy, there were no statistically Significant differences among each group.

9.Epidermoid Cyst of Testis.

A 24-year-old man visited our hospital with painless scrotal mass 2 weeks in duration. Physical examination revealed about 3cm in diameter, ovoid, hard, movable and painless mass in the left side of the scrotum. He had no history of trauma, voiding difficulty or infection sign. CBC, blood chemistry and tumor marker were normal. Left orchiectomy was done. Tumor was a well encapsulated white-gray ovoid mass filled with yellowish gray material and histogically diagnosed as epidermoid cyst. Benign tumors of testis are rare, acounting for less than 1% of all testicular tumors. Herein we report a case of epidermoid cyst of testis and review the literatures.

Pulmonary artery aneurysms are extremely rare. Even though their etiology or mechanism remain uncertain, congetial anenrysms follow the congenital heart defects which elevate the pulmonary artery pressure or increase the pulmonary blood flow and acquired aneurysms are developed by syphilis, mycotic change and atherosclerosis. Pulmonary artery aneurysms involve the artery trunk rupture and exanguination if untreated, there has been an emphasis in the need for surgical intervention whenever the diagnosis is made regardless of the presence or absence of symptoms. Here we report a case of patient ductus arterious associated with main pulmonary artery aneurysm in 39 year old female patient with review of literatures.
