2.Endoperoxides and Thromboxane A2 in Porcine Coronary Arteries with Regenerated Endothelium.

BACKGROUND: Endothelin-1 is a potent vasoconstrictor substance that causes slow and longlasting contractions of isolated blood vessels. In the aorta of the spontaneously hypertensive rat, endothelin-1 stimulates the release of a cyclooxygenase-dependent, endothelium-derived contracting factor(EDCF), presumably thromboxane A2. There have not been any studies about the response of porcine coronary arteries with regenerated endothelium to endothelin-1. METHODS: The present study was designed to determine the role of EDCF in the response to endothelin-1 in arteries with regenerated endothelium. Rings of porcine coronary arteries, with and without endothelium of previously deendothelialized left anterior descending coronary artery and native left circumflex coronary arteries, were suspended in conventional organ chambers for the measurement of isometric force. RESULTS: In quiescent rings of the previously deendothelialized left anterior descending coronary artery treatd with the nitric oxide synthase inhibitor nitro-Larginine, endothelin-1 caused contractions which were larger in the rings with edothelium than in those without endothelium. Under the same experimental conditions, in the left circumflex coronary artery, the contractions to edothelin-1 were significantly greater by the removal of the endothelium. In rings with endothelium of the previously deendothelialized left anterior descending coronary artery, indomethactin(inhibitor of cyclooxygenase) and ridogrel(thromboxane A2 receptor antagonist, and inhibitor of thromboxane synthase) shifted the concentration-response curve to endothelin-1 to the right and to a comparable extent. Dazoxiben(inhibitor of thromboxane syndthase) and BQ-123(selective antagonist of endothelin-A receptor subtype) inhibited, to the same extent as indomethacin and ridogrel, the response to higher concentrations of endothelin-1. The endothelium-dependent component of the response to lower concentrations of endothelin-1 was inhibited by indomethacin and ridogrel, but not by dazoxiben and BQ-123. In rings without endothelium of both previously deendothelialized left anterior descending and native left circumflex coronary arteries, indomethacin and ridogrel did not affect the contractions to endothelin-1. CONCLUSION: These findings suggest that in regenerated endothelium, high concentrations of endothelin-1 stimulate the release of thromboxane A2 through activation of edothelin-A receptors. Endoperoxides generated by activation of endothelial cyclooxygenase may be the endothelium-derived contracting factor(s) released in regenerated endothelium by lower concentrations of the peptide.

5.Subcotneal Pustular Dermatosis: Report of a Case.

A case of subcorneal pustular dermatosis is reported. This 39 year old woman repeatedly developed the erythematous vesicopustular eruptions on the trunk and the extremeties with scvere tiching sensation for about 7 months. Histological examinstion revealed the subcorneal pustules and the mild inflammatory cell infiltration, so tetracycline 1500mg. And chloramphenicol 1000mg. Were adminiistered without benefit. The second biopsy was obtained and it revealed as the subcorneal pustular dermatosis. Therapy with DDS was instituted and was followed by marked clearing of the lesions.

6.Angiographic Evaluation of Occlusive Coronary Arterial Disease

Angiographic findings were analysed in 22 cases of coronary occlusive disease which were selected from 50 cases of coronary arteriographies done in Department of Radiology, Seoul National Un iversity Hospital from September, 1981 to August, 1984. There were 16 cases of single vessel disease, in which 12 were LAD involvement and 4 were RCA involvement, 2 cases of two vessel and 4 cases of three vessel disease. Site, degree and extent of stenotic involvement in those occlusive coronary diseases were anal ysed with review of literature for selective coronaryarteriography. Since coronary arteriography is the final and accurate examination for the evaluation of occlusive coronary arterial disease, it is expected to be widely used for the diagnosis of occlusive coronary arterial disease which is rapidly increasing recently in this country.

7.The Changes in Polysomnographic Sleep Variables by Periodic Limb Movements During Sleep.

OBJECTIVES: Periodic limb movement disorder (PLMD) has been debated with regard to its clinical significance and diagnostic criteria. The current diagnostic criterion for PLMD in adults has been changed from periodic limb movement index (PLMI) > 5/ hour to PLMI > 15/hour by the International Classification of Sleep Disorders (ICSD). In this study, we aimed to investigate the changes in polysomnographic sleep variables according to PLMI and to determine the relevance of the diagnostic criterion for PLMD. METHODS: Out of 4195 subjects who underwent standard polysomnography, we selected 666 subjects (370 males and 296 females, aged 47.1 ± 14.8) who were older than 17 years and were not diagnosed with primary insomnia, sleep apnea, narcolepsy, or REM sleep behavior disorder. Subjects were divided into three groups according to PLMI severity: group 1 (PLMI ≤ 5), group 2 (5 < PLMI ≤ 15), and group 3 (PLMI > 15). Demographic and polysomnographic sleep variables and Epworth sleepiness scale (ESS) were compared among the three groups. RESULTS: There were significant differences among the three groups in age and gender. Sleep efficiency (SE) and stage 3 sleep percentage in group 1 were significantly higher than those in groups 2 and 3. The wake after sleep onset (WASO) score in group 1 was significantly lower than those in groups 2 and 3. However, there were no significant differences in SE, stage 3 sleep percentage, or WASO between groups 2 and 3. Sleep latency (SL) in group 1 was significantly lower than that in group 3, but there was no difference in SL between group 2 and group 3. ESS score in group 1 was significantly higher than that in group 3, but there was no difference between group 2 and group 3. Partial correlation analysis adjusted by age showed that PLMI was significantly related to SE and WASO. CONCLUSION: This study suggests that PLMI influences polysomnographic sleep variables. In addition, we found the individuals who did not have PLMD but had PLMI > 5 were not different in polysomnographic sleep variables from the individuals who had PLMD according to the current criterion. These results raise questions about the relevance of the current diagnostic criterion of PLMD.

9.Clonorchiasis and its complications: cholangiogram revisited.

Clonorchiasis is known to be closely related with the development of recurrent pyogenic cholangitis and carcinoma of the bile ducts. In order to ascertain the cholangiographic signs for recurrent pyogenic cholangitis or carcinoma of the bile ducts arising in patients with clonorchiasis. we reviewed cholangiograms in 42 patients with proven clonorchiasis. The population consisted of 29 patients with clonorchiasis alone, six patients with clonorchiasis and recurrent pyogenic cholangitis, and seven patients with clonorchiasis and carcinoma of the bile ducts. Cholangiographic abnormalities in 29 patients with clonorchiasis alone, six patients with clonorchiasis and recurrent pyogenic cholangitis, and seven patients with clonorchiasis and carcinoma of the bile ducts. Cholangiographic abnormalities in 29 patients with clonorchiasis alone were intrahepatic multiple, oval, or elliptic filling defects measuring 2-10 mm in size, representing adult flukes (n=24). The peripheral bile duct were obstructed (n=18), and the margins were ragged (n=20) and hazy (n=12) the intrahepatic bile ducts were dilated diffusely (n=27), and the dilated peripheral small tributaries gave the impression of "too many ducts appearance" (n=7) and dilatation was mid (n=17) In six patients with clonorchiasis and recurrent pyogenic cholangitis, there were filling defects of stones, and the extrahepatic ducts and larger intrahepatic ducts were predominantly dilated. In seven patients with clonorchiasis and cholangiocarcinoma all the biliary tree proximal to the tumor was markedly and diffusely dilated In the latter two groups, filling defects of flukes and associated findings were less prominent, but there was disproportionately severe dilatation of too many intrahepatic ducts. In patients with recurrent pyogenic cholangitis or cholangiocarcinoma, clonorchiasis should be considered as a underlying cause when cholangiogram shows "disproportionately" severe dilatation of too many intrahepatic ducts. intrahepatic ducts.
