3.Usefulness of IV-DSA in Peripheral Arterial Obstructive Disease.

PURPOSE: This study was designed to evaluate usefulness of intravenous digital subtraction angiography (IV-DSA) in detection of peripheral arterial obstructive disease(PAOD) and in follow-up of percutaneous transluminal angioplasty(PTA). MATERIALS AND METHODS: 35 Patients who had clinical symptoms and signs of compromised lower extremity perfusion, was screened with IV-DSA and then performed confirmative conventional angiography. We obtained sensitivity, specificity and accuracy of IV-DSA by comparing the findings of IV-DSA with those of conventional angiography. 21 patients who had been performed successful PTA, were foliowed-up with IV-DSA in 3, 6, 12months. We studied patency rate and factors that influenced restenosis. RESULTS: The sensitivity, specificity and accuracy of IV-DSA were 100%, 97%, 97% in lilac artery, 92%, 96%, 93% in femoropopliteal artery, 85%, 75%, 70% in proximal tibioperoneal artery retrospectively. IV-DSA follow up after PTA showed patency rate of 67% in first 3month, 67% in 2nd 3month, 60% in next 6month. Longer length and more irregular surface of the stenotic site, and higher incidence of run-off of vessel and of residual stenosis in the patients with restenosis were noted. CONCLUSION: High sensitivity, specificity and accuracy of IV-DSA in evaluating PAOD suggest that IV-DSA is useful in screening, planning therapy and following up after PTA.

6.Subpulmonic Ventricular Septal Defect with Aortic Insufficiency.

From January to December 1984, 191 patients with associated ventricular septal defect(VSD) were studied in the cardiac cathetherization laboratory. Among theses 191 cases, 58 cases(30.4%) of subpulmonic VSD were identified by angiocardiography. Among the 59 cases of subpulmonic VSD, 12 cases(20.6%) were complicated with aortic infficiency(AI). The relative frequency of subpulmonic VSD increased with the increase of age and the frequency of complication of AI with the subpulmonic VSD also increased with the increase of age. The amount of left to right shunt in the patients with subpulmonic VSD and AI was small and Qp/Qs ratio was less than 2.0 in all of the 12 cases. Among the 12 patients 4 cases had a pressure gradient greater than 20 mmHg across the infundibular region of the right ventricle.

7.Effects of Dietary Protein on the Progression of Early Chronic Renal Failure in Subtotally Nephrectomizid Rets.

Endodermal sinus tumor of the mediastinum is a very rare malignant tumor, usually affecting young adolescents, and its histologic findings are characteristic as that of gonadal germ cell origin. We describe the cytologic finding of fine needle aspiration of mediastinal endodermal sinus tumor in a 19-year-old male patient, comparing with tissue section. The tumor cells in smears were usually in tight clusters with large overlapping cells, which were arranged in a papillary or microacinar pattern, suggesting carcinoma. The tumor cells were large, round or oval with a small amount of cytoplasm which was occasionally vacuolated and had indistinct border. The nuclei were large, pleomorphic, and vesicular with large promi- nent nucleoli. The tissue sections showed typical findings of endodermal sinus tumor. Careful correlation of cytological findings and the serum alpha-fetoprotein level would be helpful to confirm the diagnosis.

9.Fulminant Hepatitis after Operation - Two cases report.

Although halothane is one of the most widely used inhalational anesthetics, it may cause postanesthetic complications such as halothane hepatitis. Halothane hepatitis has been reported lintermittently since 1958 with variable incidence. Changes in hepatic functions by halothane hepatitis are usually mild and reversible but they may be fatal it the patient had already shock, sepsis, massive blood transfusion, previous hepatic disease, hypoxia or induction of microsomal enzyme system. Hepatitis due to blood transfusion has high incidence especially in Korea where 10~20% of all population has vital hepatitis. Most of transfusion hepatitis is also mild but it may progress to chronic condition and eventually death. We experienced 2 cases of fulminant hepatitis after operation. Several factors such as massive whole blood transfusion, upper abdominal trauma, inhalational anesthetic agent, induction of microsomal enzyme system and shock might be attributed to development of the fulminant hepatitis.

10.Masseter Spasm following Intravenous Suxamethonium - A case report.

Isolated masseter spasm after succinylcholine administration is a rare abnormal response, but has been reported by a number of authors and is considered and early manifestation of malignant hyperpyrexia. We experienced a isolated masseter spasm after a small dose of succinylcholine administration in a facial palsy patient of central type due to brain tumor. It was likely that the intensity of this spasm was dose dependent to the amount of succinylcholine administration, and the spasm was prevented by pretreatment of nondepolarizing muscle relaxant. Anesthesia was performed safely for about 5 hours with nitrous oxide and halothane. We considered that this spasm was due to supersensitivity appearing at the denervated muscle.
