1.A Case of Cavernous Type of Angioleimyoma Occurring on the Buttock.

Angioleiomyoma(ALM) of the cavernous type is a rare subtype of leiomyomas arising from the smooth muscle of veins. ALM of this type invariably shows clinically and histopathologically distinctive features, compared with the classical solid or venous type. However, no case of ALM of this type has been yet reported in Korea although there have been several reported cases of other types. We herein present the case of a 39-year-old man with a 7-year history of a painless ALM of the cavernous type on his buttock with the histological features of markedly ectatic vascular channels and thin intervascular bundles of smooth muscles.

2.A Case of Pellagra that Initially Mimics Vibrio Sepsis.

Pellagra is a disease caused by a deficiency of niacin. It is usually found among chronic alcoholics in Korea. Dermatitis, dementia and diarrhea are characteristic symptoms of pellagra. Vibrio sepsis is due to a virulent, gram-negative rod infection caused by Vibrio vulnificus. It is generally acquired in coastal areas near warm water. Underlying disease and initial clinical presentations of pellagra are very similar to that of vibrio sepsis. Herein, we report a case of pellagra that initially mimicked vibrio sepsis.

4.Clinical Evaluation on Treatment of Calcaneal Fractures

Calcaneus is more frequently fractured than any other tarsal bones. Because of complex contour of calcaneus, it is difficult to evaluate the type of fracture exactyly. Futhermore fractures involving subtalar joint give rise to chronic pain and functional incapacity. Many different methods have been tried for treatment, but there was no aggrement of its methods and results. From January 1981 to December 1986, calcaneal fractures of 62 cases in 52 patients were analyzed and the results obtained from this study were as follows :l. Of 52 patients, 41 patients were male and 11 female. 2. The cause of fractures was a falling down in 38 patients (76%), traffic accident in 8 patients (15%) and direct blow in 6 patients (12%) 3. According to Rowe;s classfication, Type I fracture was in 5 cases, Type II in 6 cases, Type lll in 5 cases, Type IV in 12 cases and Type V in 34 cases (55%). Of these Type V fracture, tongue type was in 21 cases and joint depression type in 13 cases. 4. The treatment was as follows : cast immobilization in 33 cases, closed reduction and pin fixation in 24 cases and open reduction in 5 cases. 5. The functional result was satisfactory in 44 cases (71%). The result was better in fractures not involving subtalar joint and among type V fractures, the result was better in fractures of tongue type treated with open reduction. 6. So, we recommended compressive bandage and early mobilization for fractures without displacement, axial fixation for tongue type fractures and open reduction for joint depression type fracture.

5.The Multidimensional Evaluation of Dementia Patients and the Correlation among Measures.

OBJECTIVES: Behavioral problem and functional impairment in addition to cognitive impairment are induced, and the medical comorbidity is common in dementia patients. Thus the evaluation of these domains is needed for the management of dementia patients. This study presents a method practically and easily assessing above all dimensions, and analyzes the results. METHODS: The multidimensional evaluating method consists of one general staging measure (GDS) and four measures(MMSE, B-ADL, BEHAVE-AD, GMHR) evaluating four domains;cognition, activities of daily living, behavior, and medical comorbidity. These measures are administered to 29 dementia patients(Alzheimer's dementia 21, mixed or vascular dementia 9) admitted in a long-term care residence of dementia patients. The correlation of measures and the difference of the type of dementia, sex, and age are analysed by Pearson correlation coefficient and independent t-test respectively. RESULTS & CONCLUSIONS: In the correlation of scales, the score of MMSE was significantly correlated to the score of B-ADL in Alzheimer's dementia patient group(p<0.05). The score of B-ADL was significantly higher(p<0.05) and the score of GMHR was statistically insignificantly higher in Alzheimer's dementia patient group than mixed or vascular dementia patient group. The sex and age was unrelated to cognitive or functional ability. This multidimensional evaluating method, including the assessment of the medical comorbidity, is thought to be convenient to apply in the long-term care residence of dementia patients, and be of practical help to the management of dementia patients.

6.Toxic Epidermal Necrolysis and Stevens-Johnson Syndrome Caused by Topical Ophthalmic Use of Dorzolamide.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are the potentially life-threatening, acute hypersensitivity reaction to inciting drugs. These diseases have been often associated with systemic carbonic anhydrase inhibitor such as acetazolamide or methazolamide in Korean and Japanese patients. Dorzolamide is a recently developed topical carbonic anhydrase inhibitor with few significant systemic adverse effects. To the best of our knowledge, there have been only a few reports of SJS or TEN caused by topical dorzolamide in the literature. We herein present two cases of TEN and one case of SJS related with topical use of dorzolamide. It should be emphasized that although rarely, topical dorzolamide may cause serious sulfonamide hypersensitivity such as SJS or TEN in the susceptible patient through the systemic absorption.

7.A Case of Non-nasal T/NK-cell Lymphoma Occurring in A Patient with Autiphospholipid Syndrome.

Non-nasal T/NK-cell lymphoma(TNKCL) without nasal cavity invasion is a subtype of angiocentric T-cell lymphomas showing the biphenotype of the T-cell and NK-cell. This tumor is characterized by the absence of clonal rearrangement of the T-cell receptor gene, frequent association with Epstein- Barr virus(EBV), poor clinical course, and common extranodal invasion. Antiphospholipid syndrome (APS) is characterized by repetitive spontaneaus abortion, thrombocytopenia, thrombotic symptoms and the positivity to the antiphospholipid antibody. Our patient was a 35-year-old woman with skin lesion on her face. Previously, she had been diagnosed of APS. Histopathological, immunohistochemical and serologic studies could disclose EBV-associated non-nasal TNKCL.

8.The Expression of CLA(Cutaneous Lymphocyte-Associated Antigen) in the Cutaneous Lymphoma.

BACKGROUND: A pathogenesis of skin-homing lymphoid cells in primary cutaneous T-eell lymphoma(CTCL) has not been elucidated, CLA(cutaneous lymphocyte-associated antigen) defined by HECA-452 monoclonal anfibody has been proposed as the novel skin-homing receptors of infiltrative lymphocytes in atopic dermatitis and graft-versus-host disease. Accordingly, CLA may be a determinant explaining about the skin-homing properties of lymphoid tumor cells in CTCL. OBJECTIVE: This study was conducted to investigate the immunohistochemical expression of CLA in the lesional tissue specimens of cutaneous lymphomas. METHODS: Immunohistochemicslly, we examined the expression of CLA, E-selectin, ICAM-1 and LFA-1 antigens in the tissue specimens taken from the skin lesions and lymph nodes of the 22 patients with cutaneous lymphomas and the 20 control subjects with non-cutaneous lymphomas. Results : 1. The expression rate of CLA was 40.9% in the skin specimens of cutaneous lymphomas. We could not fad any expression of CLA in non-cutaneous lymphomas. 2. CTCL showed a more frequent expression of CLA(80%) than cutaneous B-cell lymphomas (CBCL)(8.3%). We found more frequently CLA+ high endothelial venules in non-cutaneous lymphomas(40%) than in cutaneous lymphomas(9.1%). 3. CLA and E-selectin were more frequently expressed in primary CTCL(100%, 83.3%) than in secondary CTCL(50%, 0%). 4. The positivity to ICAM-1 and LFA-1 antigens was higher in primary cutaneous lymphomas (72.7%, 81.8%) than in secondary forms(9.1%, 9.1%). Conclusion : CLA and E-selectin may represent the pivotal skin-homing receptor of infiltrative tumor cells and vascular counter-receptor in primary CTCL, respectively. Also, ICAM-1 and LFA-1 may have a role in the primary cutaneous infiltration of lymphoma cells as the additional cofactors.

10.Staphylococcal Scalded Skin Syndrome in an Immunocompetent Adult.

Staphylococcal scalded skin syndrome (SSSS) is an exfoliative infectious disorder caused by the exfoliative toxins of Staphylococcus aureus. The condition usually affects infants and children younger than 5 years of age. However, SSSS is rarely observed in adults. Adult SSSS is usually associated with renal failure, immunosuppression, malignancy, alcohol abuse, or HIV infection. In contrast to infant cases, adult SSSS is often accompanied with a high mortality rate, sepsis and poor prognosis. We report a rare case of SSSS in an immunocompetent 34-year-old man without renal failure.
