1.Stress Fracture in Lower Extremity

Three cases of fatigue fracture were experienced, whose tibia, fibula and femoral neck had been involved. The mechanism of fracture was explained as the result of repeatation of uncustomed stress mainly in lower extremities. Several authors suggested many types of predisposing factors, such as spastic flat foot, short first metatarsal bone and osteoporosis after steroid administration. Two of the cases were treated conservatively and one case with femoral neck fracture treated by open reduction and Sith-Peterson nailing.

2.Clinical Study and Therapeutic Experience in Tinea Versicolor.

Fifty-six patients with tinea versicolor were studied clinically and therapeutically, from July 1984 to August 1985, at the Department of Dermatology, Hanyang University. L atients were treated with 20g sodium thiosulfate solution, 1g isoconazole nitrate cream, lg bifonazole cream and. oral ketoconazole. The result obtained were as follows: 1. The average age of all patients at visit was RO. 1 years, the oldest patient was 72 year-old and the youngest patient was 13 year-old. The male to female ratio was 3. 7: 1. 2. Distribution of lesions were anterior chest(26.8%), back(23.9%), abdomen (19%), axilla (17%), neck(6. 3%), pubic area(4. 2%), upper extremities(1. 4%), hip(0.7%) and lower extremities(0.7%). The incidence of hyperpigmented lesions was 76. 8%, and that of hypopigmented lesions was 23. 2g. 3. Average of treatment duration in each trial group indicated 3. 71-I--0 83 weeks (M+SD) in 20g sodium thiosulfate regimen group, 3. 07+ 0. 73 weeks in 1g isoconazole nitrate, 3.29+1.14 weeks in bifonazole, R. 00+0. 68 weeks in oral ketoconazole. No treatment results show statistically significant differences among the trial groups.

3.Two cases of Cheilitis Granulomatosa.

We report herein two cases of cheilitis granulcmatosa which were beneficial effects with intralesional triamcinolone acetonide. Casc 1, 27-year-old, showed both lip swelling with moderate scrotal tongue for 7 years and Case 2, 50-year-old, -had lower lip swelling for 1. 4 years. Histopathologic findings show lymphohistiocytic infiltration in the dermis with focal tuberculoid granuloma in case 1 and ill-defined granuloma in case 2.

6.Prevalence of PPNG in Seoul, Korea ( 1983 ~ 1984 ).

Penicillinase producing Neisseria gonorrhoeae(PPNG) have been reported from various cauntries throughout the world. We have been tr ying to assess the prevalence of PPNG in Korea since May, 1980 by means of chromogenic cephalosporin method. The total number of isolated Neisseria gonorrhoeae strains in 1983 was 733, among which 679 were tested for p-lactamase production, among which 178(26. 2%) were PPNGs. In 1984, 791 strains were isolated, among which 214(27.1%) were PPNGs. In all, 392(26.7%) strains were found to be PPNGs arnong 1470 Neisseria gonorrhoeae strains tested between 1983-1984.

7.Microsatellite Instability and hMSH2 Gene Mutations in Sporadic Colorectal Cancers.

Microsatellites are short nucleotide repeat sequences present throughout the human genome. Alterations of microsatellites, comprising extra or missing copies of these se quences, have been termed microsatellite instability(MSI, genetic instability, replication errors, RER(+) phenotype). To date, at least four genes involved in DNA mismatch repair, hMSH2, hMLH1, hPMS1 and hPMS2, are thought to account for the observation of microsatellite instability in tumor from Hereditary nonpolyposis colorectal cancer (HNPCC) patients. The genetic defect responsible for the MIN+ phenotype in sporadic colorectal cancer, however, has yet to be clearly delineated. The purpose of this study was to determine the presence of MSI in sporadic cancer and to correlate its occurrence with clinicopathological parameters, we have studied six microsatellite loci by use of polymerase chain reaction amplification and denaturing polyacrylamide gel electrophoresis. We found that 20%(9 of 46 cases) sporadic colorectal cancers showed RER at two or several loci(RER+). Microsatellite instability was associated with location of the tumor in the proximal colon 66%(6 of 9 cases) and with poorly differentiated tumor phenotype 56%(5 of 9 cases). In order to better understand the role of somatic alterations within hMSH2 in the process of colorectal tumorigenesis, we examined the most conserved regions(codon 598~789) of this gene in nine patients with MIN spotadic colorectal cancer. 6 patient of RER(+) colorectal ca. patients had a polymorphism which was a T to C base change in the intron sequence at -6 position of the splice acceptor site at the 5'end of exon 13. This particular sequence variation is a polymorphism rather than a mutation which increase cancer susceptability. These data suggest that the genetic instability is detect ed in some colorectal cancers and play an important role in the pathogenesis of sporadic colorectal cancer.

8.Treatment of Uncomplicated Male Gonococcal Urethritis with Ceftriaxone.

Ceftriaxone is a new semi-synthetic, broad spectrum third generation cephalosporin. It has a high degree of stability in the presence of beta-lactamase and its half-life averages about 8 hours. A total of 109 patients with uncomplicated gonococcal urethritis seen at the VD Clinic, Choong-Ku Public Health Center, in Seoul in the periods of March April 1986, and April-May 1987, were subjected to this study. The diagnosis of gonococcal infection and the test. of cure were made on the basis of culture and Gram staining of urethral smear. The patients were divided randomly into two groups consisting of 49 and 60, and treated with ceftriaxone, 250mg, IM and 500mg, IM respectively. No failure case was seen in both groups. lt is suggested that ceftriaxone, 250mg, IM regimen may be used as the first line treatment for uncomplicated gonococcal urethritis.

9.Four Cases of Childhood Dermatomyositis.

We describe herein four cases of childhood dermatomysitis of Brunsting type. The ages of these patients were between 7 to 10 years at the time of the disease. All four patients had pathognomOnic cutaneous changes, such as heliotrope erythema and Gottron's papules, which are not seen frequently in adult type dermatomyositis. These patients did not show any marked serologic abnormalities suggestive of having other connective tissue diseases or autoimmune diseases. All patients were initially treated with prednisolone and chloroquine, and have been under control with low dose prednisolone, or chloroquine alone.

10.Postoperative Changes of Herniated Intervertebral Disc: Normal and Discitis MR Findings.

OBJECTIVE: To describe normal postoperative MR findings and MR findings of postoperative discitis in patients who underwent operation due to herniated intervertebral disc. MATERIALS AND METHODS: We retrospectively reviewed normal postoperative MR findings and MR findings of discitis in 30 patients(21-61yrs.) (13 cases dignosed as discitis and 17 cases as normal) who previonsly underwent laminectomy and discectomy, or bony fusion. We analyzed signal intensity of end plate and disc, end plate destruction, and enhancement of end plate and disc on T1- and T2-weighted images(WI) of 1.5T MRI. RESULTS: Among 14 out of 17 patients with no evidence of discitis, 7 patients showed high signal of the posterior portion of disc on T1- and T2-WI and 11 patients revealed enhancement at the same sites. In all 13 patients suspected of having discitis, end plate and disc showed low signal on T1-WI, high signal on T2-WI, heterogeneous enhancement, and irregular destruction of end plate. Meanwhile, 3 cases with no evidence of postoperative discitis clinically who underwent bony fusion showed similiar findings to those of the above 13 patients, except for homogeneous enhancement of end plate and vertebral body. CONCLUSION: The MR findings of postoperative discitis were low signal on T1-WI, high signal on T2-WI, and heterogeneous enhancement of and plate and disc, and destruction of end plate.
