3.Study on Korean Daily Walking Activity <1>.

Since there is no report of daily walking activity of Korean, authors studied on normal adult daliy walking activity in korean of various kinds of occupation, personal life and patients. Total 134 cases were studied and in which there were 10 patients, 7 retired elderlies, 4 house wives, 48 employees of compaines, 30 nurses, 4 laboratory technicians, 2 pharmacists, 7 house staff doctors, 12 chief doctors, 4 cases of hicking and 6 cases of 18 holes golf players. The daily walking steps were measured with pedometer in 167 times in total and following results were, obtained. 1. Bed rest patients ; less than 1000 steps/day 2. Retired elderly ; 2,724(2,150-3,500) steps/day 3. House wife ; 5,437(3,030-7,920) steps/day 4. High ranking, old clerk and chief doctor ; 5,000-6,000 steps/day 5. 4.The Expression of Vascular Endothelial Growth Factor, Kinase Domain Region, and Transforming Growth Factor-beta 1 in Cervical Neoplasia.

OBJECTIVE: Angiogenesis is a critical factor in the progression of solid tumors. The mechanisms responsible for angiogenesis in cervical neoplasia, however, are not well defined. Our study was aimed to determine the expression of VEGF(Vascular Endothelial Growth Factor), its receptor(KDR), and TGF-beta1(Transforming Growth Factor-beta1) in cervical neoplasia, to determine the role of these angiogenic factors in preinvasive(dysplastic) process and the progression of cervical cancer and to investigate the progression of angiogenesis in the transition from normal cervix to invasive squamous cell carcinoma of the uterine cervix. METHODS: The cervical lesions of 76 patients were punch biopsied and paraffin embedded. Among these, 5 were normal cervix, 36 were cervical intraepithelial lesion I-III, and the other 35 were invasive squamous cell carcinomas. The tissues were immunostained with antiVEGF, antiKDR, and antiTGF-beta1 polyclonal antibody. RESULTS: The expression of VEGF, KDR, and TGF-beta1 in CIN III was stronger than those of CIN I(p<0.01). Their expression were not significantly different among the each staged cervical cancers(p>0.01). CONCLUSIONS: These observations suggest that VEGF, KDR, and TGF-beta1 are important angiogenic factors in cervical neoplasia, especially in an early event to neoplastic transformation of cervical tissues, but these angiogenic factors are not associated with the progression of cervical cancer.

5.Arthroscopic Treatment of the Localized Pigmented Villonodular Synovitis of the Knee.

Pigmented villonodular synovitis (PVNS) is a relatively rare condition. The disorder almost always involves a single joint and the knee is most commonly affected. We report on three cases of localized PVNS that involved the patella fat pad and synovium. Diagnostic and therapeutic arthroscopies were performed, and typical findings of localized PVNS were found. Complete resection of the lesions were performed arthroscopically. Arthroscopy can be used as an effective diagnostic and therapeutic tool for identification and resection of intraarticular localized PVNS of the knee.

6.Expression of Phospholipase C-gamma1 and gamma2 in Non-Hodgkin's and Hodgkin's Lymphoma.

Phospholipase C (PLC) plays a role in ligand-mediated signal transduction for cellular activity such as proliferation and differentiation. A recent observation that PLC- gamma1 is highly expressed in some kinds of human cancer tissue supports the view that PLC-gamma1 may be involved in proliferation and carcinogenesis. PLC-gamma2 is known to be involved in B cell differentiation and maturation. However, there have been few studies about the expressions of PLC-gamma1 and gamma2 in human lymphoid malignancy. In the present study, we examined the contents of PLC-gamma1 and gamma2 in 10 cases of B cell, 10 cases of T cell non-Hodgkin's lymphoma and 5 cases of Hodgkin's lymphoma to find out whether these enzymes play any role in the carcinogenesis by immunohistochemistry and immunoprecipitation. Immunoprecipitation analysis revealed that in contrast to increased expression of PLC-gamma2 only in B cell lymphoma, a considerably higher level of PLC-gamma1 was detected in both B and T cell lymphoma. Immunohistochemical finding confirmed this observation. PLC-gamma1 and PLC-gamma2 were expressed in the cytoplasm of most tumor cells. PLC-gamma2 was also expressed in mature B cells, while PLC-gamma1 was not expressed in reactive non-tumor cells. These results suggest that PLC-gamma1 mediated signal transduction implicates a significant role in the carcinogenesis of all types of lymphoid tissue, and PLC-gamma2 may play a role in the carcinogenesis of B cell lymphoma as well as B cell differentiation.

7.Diagnostic Efficacy of the Morphological Scoring System of Ultrasound, CA-125, Color Doppler Sonography(CDS), and Magnetic Resonance Imaging(MRI) in Detecting Ovarian Malignancy.

OBJECTIVE: Ovarian cancer is a silent disease with few symptoms. Because early detection of ovarian cancer is difficult, most cases are diagnosed at an advanced stage. To determine whether an ovarian tumor is malignant or not, various diagnostic procedures have been used including physical examination, ultrasound and tumor markers such as CA-125. We recently compared color Doppler sonography(CDS) with the morphological scoring system and CA-125 levels in establishing an accurate preoperative diagnosis of adnexal mass. As a result, we tried to compare the morphological scoring system with CA-125 and CDS in the diagnosis of adnexal mass. METHODS: The medical records of 47 patients referred with preexisting adnexal lesion were reviewed retrospectively by comparing ultrasonic data(morphological scoring system and color Doppler finding) with serum CA-125 and MRI findings. RESULTS: Forty-nine masses were removed surgically and confirmed as 25 malignancies and 22 benign tumors. In cases using the morphological scoring system and CA-125, the accuracy showed 95% sensitivity. In cases using the morphological scoring system and CDS, the sensitivity was 88%. In cases using the morphological scoring system and CDS, the sensitivity was 48%. CONCLUSIONS: The morphological scoring system and CA-125 are the basic methods for predicting ovarian malignancy. MRI and CDS may be also useful diagnosis methods in diagnosis of ovarian malignancy. But combination of these diagnositic modalities may not improve the accuracy of predicting ovarian malignancy, significantly.
