1.Acute Rotator Cuff Tears due to Low Voltage Electrical Injury: A Case Report

Since shoulder have a higher proportion of muscle which would have low electrical resistance, there could be more electrical damage to the rotator cuff muscles. We present a patient with acute rotator cuff tear by sudden uncontrolled jerking contractions caused by an electrical shock. A case of 42-year-old man with acute rotator cuff tear due to electrical injury to the shoulder was presented. Magnetic resonance imaging showed a full thickness tear and an undulating appearance of the peripheral end of the torn supraspinatus and infraspinatus muscle, suggesting an acute complete rupture. By arthroscopic surgery, the torn rotator cuff tendons were repaired with a suture bridge technique. At the final follow-up, the patient had a full, pain-free range of motion and had fully recovered shoulder muscle power.

2.A Case of Portal and Splenic Vein Thrombosis Developed by Complication of Histoacryl Injection Therapy in Gastric Varix.

Liquid tissue adheisve, Histoacryl (n-butyl-2-cyanoacrylate) has been used for the treatment of gastric variceal bleeding. The techniques are as follows: 0.5cc Histoacryl mixed with Lipiodol per each injection are used. 3 to 4 injections are usually required for large variceal convolutes under the fluoroscopic visualization after the injection of Histoacryl. Complications of sclerotherapy with Histoacryl are bleeding, perforation, stenosis and embolism depending on the concentration and amount used, as well as the intensity of the treatment. Here we describe a case report developing portal and splenic vein thrombosis as a side effect after Histoacryl injection therapy for the treatment of gastric varix bleeding, A 59-year-old male patient with liver cirrhosis was admitted due to acute gastric varix bleeding. The control of gastrie variceal bleeding was achieved by several injections of 0.7c mixture of 0.5cc histoacryl and 0,8cc Lipiodol. However, simple X-ray and ultrasonography revealed the elements of Histoacryl-lipiodol mixture in the portal and splenic vein.

3.Primary Transitional Cell Carcinoma of the Ovary: A case report.

Primary ovarian transitional cell carcinoma(TCC) is a recently described, distinct subtype of ovarian carcinoma resembling TCC of the urinary bladder. TCC differs from malignant Brenner tumor(MBT) by absence of benign or proliferative Brenner component and prominent stromal calcification. TCC also represents a high-stage tumor with more aggressive biologic behavior than MBT, but it has a relatively favorable response to chemotherapy. TCC may arise from the pluripotential surface epithelium of the ovary or from the cells with urothelial differentiation, rather than from a benign or proliferative Brenner tumor precursors. We report a case of pure form of primary TCC presenting as a left ovarian mass in 45-year-old woman.

5.Carcinosarcoma (Malignant M llerian Mixed Tumor) of the Female Genital Tract: A clinical and pathologic study of ten carcinosarcomas.

Carcinosarcomas of the female genital tract have generally been regarded as a type of sarcoma. Recent studies, however, suggest the tumor may be more closely related to carcinoma and may represent metaplastic carcinoma in histogenesis. We analyzed clinicopathologic and immunohistochemical features of 10 carcinosarcomas to evaluate the relative importance of the carcinomatous and sarcomatous components in metastasis and recurrence. The primary tumor originated in the uterine body in seven cases, the uterine cervix in two and the ovary in one. Patient,s ages ranged from 54 to 71 years (mean, 64). The most common symptom of the uterine mass was vaginal bleeding. The median survival time was 21 months following diagnosis in five cases. Surgico-pathologic FIGO stages of five patients who received an operation were stage III and IV, but clinical FIGO stage of three patients (60%) among them were I. Lymphovascular invasions were identified in seven areas; five vascular invasion lesions showed the carcinomatous component alone, one the sarcomatous component alone, and remained one admixture of both components. Metastatic and recurrent lesions to the paraaortic lymph node, ovary, pelvic wall, or vaginal vault showed characteristically carcinomatous component only. Immunohistochemically, positive reactions for cytokeratin and epithelial membrane antigen were noted in the sarcomatous component of five cases. Vimentin positivity was detected in carcinomatous component of three cases. We conclude that the dominant element in carcinosarcomas of the female genital tract is the carcinomatous component. The survival rate of carcinosarcoma is extremely poor. The surgico-pathologic stage is better indicator of survival than the clinical stage. Immunohistochemical findings suggest that carcinosarcoma may represent a metaplastic carcinoma in histogenesis.

7.Possible Association between Differences in Nasalance Scores and Early Spread of COVID-19 Based on Linguistic Analysis

Objective:
The World Health Organization (WHO) declared a pandemic on March 11, 2020 after more than 118,000 cases of coronavirus disease 2019 (COVID-19) had been reported in 114 countries. Our study analyzed the cumulative incidence rate based on WHO data starting with the first confirmed patient until the peak of transmission. In addition, the numerical values of nasometry from normal subjects were quantified to analyze the linguistic features.
Methods:
This study consisted of two main methodologies including a meta-analysis based on nasometry data involving normal adults and cumulative incidence rate based on WHO data. In addition, the numerical values of nasometry from normal subjects were quantified to analyze the linguistic features.
Results:
The pooled overall mean differences (MDs) for oral text nasalance among linguistic families were 14.655 (95% confidence interval [CI], 7.986–21.324) in Arabic, 24.441 (95% CI, 17.920–30.962) in Chinese, 14.964 (95% CI, 13.677–16.251) in European, and 11.437 (95% CI, 9.880–12.994) in Ural-Altaic. The pooled overall MDs for cumulative incidence rate of COVID-19 were 190.3 (95% CI, 56.10–324.60) in Arabic, 283.20 (95% CI, 1.80–564.60) in European, and 5.70 (95% CI, 4.90–6.60) in Ural-Altaic. Correlation between oral nasalance score and cumulative incidence was significant (P=0.0004).
Conclusion
Our study showed the possible association between language characteristics and early spread of COVID-19. Further studies are needed to validate our outcomes based on various epidemiologic and behavioral factors including mask wearing.
8.Clinical Study for Open Fracture of Tibia

118 Cases of open tibia fractures treated in Chosun university Hospital were survayed and evaluated. Results: 1. The cases are predominantly male(80%) and most often in their 4th decade. 2. They were very often comminuted(44%) 3. Treatments consists of debridement with or without initial internal(plate and screws) or external (Hoffmann's device) fixations. The end results seems to be better in cases with initial fixations of bone(internal or external)than those treated with initial debridement and plaster cast immobilization only and osteosynthesis later.

9.Surgical Treatment of Tarsometatarsal Joint Fracture: Dislocation

Injuries to the tarsometatsrsal joint are not common, and the results or treatment are often unsatisfactory. Whatever the severity of the initisl injury, prognosis depends on accurate reduction and its maintenance. 5 cases of fracture and dislocation of the tarsometatarsal joint were treated at Department of Orthopaedic Surgery, Capital Armed Forces General Hospital. The length of follow-up period ranged from 12 months to 18 months, with a mean of 15 months.

10.Phagocytosis of Drug-Resistant Mycobacterium Tuberculosis by Peripheral Blood Monocytes.

BACKGROUND: Phagocytosis is probably the first step for mycobacteria to be virulent in host because virulent strains are more readily phagocytosed by macrophage than attenuated strains. According 13 the traditional concept, multi-drug resistant strains have been regarded as less virulent. However, this concept has been challenged, since recent studies(reported) showed that the degree of virulence and drug-resistance is not related. The purpose of this study is to evaluate whether the phagocytic activity of M. tuberculosis by peripheral blood mononuclear cells(PBMC) is different according to drug-resistance or host factor. To evaluate this, we estimated the difference of phagocytic activity of drug-resistant and drug-sensitive M. tuberculosis and also estimated the phagocytic activity of PBMC from intractable tuberculosis patients and healthy controls. METHODS: PBMC from ten intractable tuberculosis patients and twelve healthy control and three different strains of heat-killed M. tuberculosis, ie, ADS(all drug sensitive), MDR(multi-drug resistant), and ADR(all drug resistant) were used. After incubation of various strains of M. tuberculosis with PBMC, the phagocytic activity was evaluated by estimating proportion of PBMC which have phagocytosed M. tuberculosis. RESULTS: Drug-resistant strains of M. tuberculosis were phagocylosed easily than drug sensitive strains(Percentage of PBMC phagocytosed M. tuberculosis in healthy control : ADS : 32.3α2.9%, ADR : 49.6α3.4%, p=0.0022, Percentage of PBMC phagocytosed M. tuberculosis in intractable tuberculosis patients : ADS : 34.9α3.6%, ADR : 50.7α4.5%), p=0.0069). However, there was no difference in phagocytic activity of PBMC from healthy control and intractable tuberculosis patients. CONCLUSION: Drug-resistant strains of M. tuberculosis were phagocytosed easily than drug sensitive strains and host factors does not seems to influence the phagocytosis of M. tuberculosis.
