2.Metastasis of Carcinoma to Carcinoma.

Two unusual cases of metastasis of carcinoma to carcinoma are reported. The initial malignancies, an adenocarcinoma of the rectum and a duct carcinoma of the breast, were found to have metastasis to a clear cell carcinoma of the kidney. The various theories on the peculiar tendency for the renal cell carcinoma to be the most common host tumor for other primary carcinoma are commented upon briefly.

4.Clinical Significanc of Septal Malalignment in Ventricular Septal Defect.

Objective: Depending on the direction and degree of the septal deviation, the patient with malalignment-type VSD present with a variety of clinical syndrome. It is the purpose of this study to examine clinical characteristics of malalignment-type VSD, and to emphasize the clinical significance of septal malalignment in VSD and thus to increase the awareness for it. Methods: We analyzed echocardiographic or cardic catheterization and angiographic findings of 85 patients who were diagnosed as malalignment-type VSD at the department of pediatrics, Chonnam University Hospital between July 1990 and September 1992 Results: 1) Among 85 patients with malalignment-type VSD, there were 52 Cases (61. 2%) of Fallot-type VSD. 14 cases (16.5%) of Eisenmenger-type VSD, 19 cases (22.3%) of coarctation-type VSD. 2) All the 85 cases with malalignment-type VSD had the infundibular defects, and perimembranous infundibular VSD was most frequently associated with septal malalignment. 3) There was no prolapsing valve in all the cases with malalignment-type VSD. 4) Pulmonary hypertension was observed in all the cases with Eisenmenger-type and coarctation-type VSD. 5) Among the 19 cases with coarctation-type VSD, 8 cses (42.1%) were associated with coarctation of the aorth, and 3 cases (15.8%) with interrupted aortic arch. Conclusion: We conclude that septal malalignment in VSD has considerable clinical significance. Therefore, the authors believe that we should always consider the presence or absence of septal malalignment, in addition to the size and location of defect at the time of diagnostic evaluation of VSD, especially in the Koreans showing high incidence of infundibular VSD.

6.Changes on Index of Nausea, Vomiting, and Retching in Hospitalized Cancer Patients Undergoing Chemotherapy.

PURPOSE: The purpose of this study was to determine the changes on Index of Nausea, Vomiting, & Retching (INVR) during a cycle of chemotherapy. METHODS: Forty-three patients hospitalized for chemotherapy at C University Hospital during a period of 5 days from March to May, 2003 were examined. Scores of INVR were measured once a day. Anxiety, anorexia, fatigue, and sleep satisfaction were measured before chemotherapy. Data was analyzed by repeated measures of ANOVA. RESULTS: The score of INVR increased over time during the days of hospitalization and showed a peak on the third day. The score was significantly higher on the third and consecutive cycles than on the first and second cycle. The score was significantly higher in patients in their forties and fifties rather than in their sixties. The score was higher in women than in men, and also increased as the sleep satisfaction decreased. CONCLUSION: These results suggested that specific interventions for relief of nausea & vomiting were needed in middle age, women, the third chemotherapy cycle, and the third day after chemotherapy.

7.A Case of Retinal Periphlebitis Associated With Barre Syndrome.

Authors' presented an interesting case, 19 years old boy, who was been suffered from retinal periphlebitis of both eyes which thought to be induced by Barre Syndrome. This patient also has pulmonary tuberculosis and visual symptoms of Barre Syndrome in this case were ocular pain and visual disturbance that appeared and disappeared suddenly due probably to irritation of the ventral nerve roots (C5 to T1) with also irritation of sympathetic fibers contained in them.

8.The Differences of Biochemical Status and Dietary Habits according to the Obesity Degree among Obese Elementary School Students in the Gyungbuk Area .

The prevalence of obesity in children has steadily risen during recent years in developed countries. Child obesity has become a major concern to health providers since it has grown to epidemic proportions over the past few decades. This study was conducted to investigate the biochemical status, dietary habits and life styles according to the obesity degree among obese children residing in the Gyungbuk area. The subjects were 148 elementary school students (boy 103, girl 45) and classified as mildly obese (n = 56), moderately obese (n = 61) and severely obese (n = 31) by the obesity index. The average body weight and height increased significantly according to the obesity degree (P <0.05, P <0.001). Serum cholesterol concentration came under the normal level in all groups. Serum AST activity increased according to the obesity degree but it was not significant. Serum ALT activity increased according to the obesity degree (P <0.05). Dietary habit score was lower significantly according to the obesity degree (P <0.01). Life style of the subjects was significantly different for the level of 'healthy state (P <0.05)', 'body perception (P <0.01)', 'watching TV and video (P <0.05)', 'normal sleeping hour (P <0.05)' and 'degree of stress (P <0.05)' according to the obesity degree. A positive relationship between obesity index and blood pressure, ALT has been shown in the subjects. Obesity index and dietary habit scores were negatively associated. Therefore, these results suggest that a practical nutrition-exercise education program for the prevention of child obesity should be provided to elementary school children.

9.Skin Characteristics of Normal Korean Subjects According to Sex and Site using Non-Invasive Bioengineering Methods.

BACKGROUND: During the last few years, the in vivo study of the physiological parameters of the skin by non-invasive methods has been considerably developed. So far, there have been some reports on the skin characteristics only in parts, but there has not been any criteria to classify those of normal subjects. OBJECTIVE: The aim of the present study was to investigate the skin characteristics of healthy Korean subjects according to sex and sites using non-invasive methods. METHODS: To determine normal levels of sebum, skin hydration, transepidermal water loss (TEWL), skin elasticity and skin color according to sex, 163 subjects (male; 124, female; 39) were used to investigate 5 different anatomical sites. 6 different instruments were used: The Sebummeter SM 410, Corneometer CM 820, Evaporimeter EP1, Cutometer SEM 474, Chromameter CR-121, and Mexameter MX 16, for evaluating sebum excretion rate, capacitance, TEWL, mechanical property and skin color respectively. RESULTS: Differences were noticed depending on the anatomical sites and sex. Most of the measuring parameters were significantly different according to sites and sex. The values of sebum levels, capacitance and TEWL were higher in the males on the cheek, forehead and crows foot, whereas in the females, higher values were observed on the dorsum of the hand. The skin elasticity varied considerably among the nine-parameters but, for the elastic ratio (R2, R5), the females showed significantly higher values than the males in all sites except the forehead. Skin lightness (L* value) was higher in the females, whereas the males showed lugher values in the category of redness (a* value) and yellowness (b* value). The values of the eqrthema index (EI) and melanin index (MI) were also higher in the males on all sites. Correlations between the skin parameters mentioned above were calculated. A negative, correlation between capacitance and TEWL was observed only on the cheek (male/female, r =-0.2/ r =-0.4, p<0.05). The L* value correlated negatively with MI. Mioreover the values between a* and El also showed sipificant correlations in the male (cheek and dorsum of hand, y =0.2, forehead and crows foot, r =0.3, p<0.05). There were considerably significant correlations between the visual pigmentation score and instrumental skin parameters in the males (visual pigmentation score vs. L* value measured by Chromameter ; cheek/crows foot, r = -0.3/y =-0.4, visual pigmentation score vs. MI by Mexameter ; cheek/crows foot, r =0.2/ r =0.4, viisual winkle score vs. sebum excretion rate measured by Sebumeter ; cheek, r=0.2, visual winkle score vs. elasticity parameters measured by Cutometer ; cheek, R2/R5/R7, r =-0.3/ r =-0.2/ r =-0.3, p<0.05). CONCLUSION: Skin physiological parameters can be evaluated by non-invasive skin bioengineering methods which show quantitative modifications in physiological conditions in relation to sites and sex.
