There was decreasment of the domestic animal rabies, but increasment of the sylvatic animal rabies now a days in the world. We could see stable statistics of the ministry of health and social affairs annuala report, however, only rabies we could occationally find out suggestive human rabies by domestic animal for the slight more incidence in Korea. Now, we report on the three cases of sugg. Rabies in our children and review of literature. We think the improvement should be needed for postmortem examination that isn't taking by the Korean custom.

3.A Case of Renovascular Hypertension Cured by Percutaneous Transluminal Angioplasty.

Pertutaueous transluminal angioplasty is the treatment of choice for renovascular hypertension in cases due to the renal vascular occlusion which is not severe enough to pass dilatation catheter. The advantages of percutaneous transluminal angioplasty are the use of local anesthesia, its noninvasiveness without surgical risk, feasibility of repetition and shorter hospital days compared with surgical methods. We are presenting a case of renovascular hypertension due to atherosclerosis cured by percutaneous transluminal angioplasty with the review of literatures.

4.The Effect of Hypothermia on the Duration of Vecuronium and Atracurium during Cardiac Surgery using Cardiopulmonary Bypass.

The purpose of this study was to investigate the influence of hypothermia on the recovery time after either vecuronium or atracurium given by repeated bolus injection, and to determine the adequate time of administration of neuromuscular blockers during cardiopulmonary bypass. Two groups of 10 patients each were studied before(Tl), during(T2) and after(T3) hypothermia. Following an initial intravenous bolus dose of 2X ED95 of either vecuronium(Group I) or atracurium(Group II), repeated bolus dose of ED95 was injected whenever twitch tension had returned to 20% block of train-of-four response before bypass of control As a result, the action duration of vecuronium(from injection of loading dose to twitch tension=20%) was 29.5+/-5.5 mins, 103.1+/-12.0 mins and 70.8+/-9.9 mins before, during and after hypothermia, respectively. The eorresponding values for atracurium were 26.3+/-5.1 mins, 85.7+/-11.4 mins and 42.5+/-5.1 mins. We conclude that hypothermia increases significantly the duration of action of both muscle relaxants(P<0.001) and vecuronium is influenced by hypothermia more than atracurium. The rate of recovery of twitch height after hypothermia is more rapid in atracurium than vecuronium(P<0.001). Single bolus injection of ED95 of vecuronium administered just before cardiopulmonary bypass is suitable for maintaining the neuromuscular block throughout cardiopulmonary bypass.

5.A Case of Cavernous Type of Angioleimyoma Occurring on the Buttock.

Angioleiomyoma(ALM) of the cavernous type is a rare subtype of leiomyomas arising from the smooth muscle of veins. ALM of this type invariably shows clinically and histopathologically distinctive features, compared with the classical solid or venous type. However, no case of ALM of this type has been yet reported in Korea although there have been several reported cases of other types. We herein present the case of a 39-year-old man with a 7-year history of a painless ALM of the cavernous type on his buttock with the histological features of markedly ectatic vascular channels and thin intervascular bundles of smooth muscles.

6.Local Control of Local Excision for T1/T2 Rectal Cancer .

PURPOSE: The aim of this study was to review the outcome of local control after the local excision for T1/T2 rectal cancers and, thus, to assess its effectiveness as an alternative to a more radical resection. METHODS: This retrospective study analyzed 23 patients with T1/T2 rectal cancer treated by local excision (LE), and their results were compared with the results for 22 patients with rectal cancer of the same stage treated by a radical resection (RR). All patients with pT2 lesions in the LE group received postoperative adjuvant chemoradiation. The outcomes were defined as 5-year local-recurrence-free survival (LRFS). The median follow-up was 72 (range, 40~92) months. RESULTS: Recurrence occurred in 4 patients (pT1, 1; pT2, 3) in the LE group and in 3 patients (all pT2) in the the RR group. One patient with vascular invasion (T2N1M0) in the RR group showed multiple liver metastases at 23 months postoperatively. The difference in 5-year LRFS was not statistically significant between the two groups. In the LE group, the 5-year LRFS for pT2 lesions was significantly less favorable than that for pT1 lesions (40% vs. 94%; P= 0.005). The 5-year LRFS for pT2 in the RR group was more favorable than that in the LE group, although the difference was not statistically significant (76.9% vs. 40%, P=0.138). CONSLUSIONS: Local excision provides a favorable local control for pT1 rectal cancers. A more radical resection, however, remains an effective surgical option for pT2 lesions because local excision, even combined with adjuvant chemoradiation, showed substantial local recurrences.

8.A Study on Serum Lipid in Hypertension and Ischemic Heart Diseases.

The present study has been undertaken to investigate the values of serum lipids with hypertension and ischemic heart diseases. Serum lipids and lipoproteins were measured from 231 cases of hypertension, 23 cases of angina pectoris, 18 cases of acute myocardial infarction and 67 healthy adults together with normal value as controls. The results obtained were as follows: 1. The mean values of serum lipids in healthy adults were 111.0+/-41.3mg% for triglyceride, 171.2+/-33.5mg% for cholesterol, 175.1+/-36.3mg% for phospholipid, 479.9+/-82.6mg% for total lipid. The values of serum lipoproteins were 219.8+/-46.8mg% for beta-lipoprotein, 93.4+/-53.8mg% for pre beta-lipoprotein and 164.1+/-40.8mg% for alpha-lipoprotein. The values of cholesterol, triglyceride and beta-lipoprotein were gradually increased with aging. 2. The mean values of serum lipids in patients with hypertension were 176.3+/-94.5mg% for triglyceride, 199.7+/-36.9mg% for cholesterol, 207.8+/-38.0mg% for phospholipid, 601.9+/-139.9mg% for total lipid. The values of serum lipoproteins were 266.0+/-73.5mg% for beta-lipoprotein, 147.7+/-89.5mg% for pre-beta-lipoprotein and 187.7+/-56.7mg% for alpha-lipoprotein. The serum lipid values in patients with hypertension were significantly higher than those in healthy controls. 3. The mean values of serum lipids and lipoproteins in patients with angina pectoris were significantly higher than in healthy controls. 4. The serum lipid values in general were higher rather in patients with acute myocardial infarction than healthy controls, but the values of serum phospholipid, pre-beta-lipoprotein and alpha-lipoprotein in them were not significantly higher than those in healthy controls. 5. The values of serum HDL-cholesterol in patients with hypertension, anginal pectoris and acute myocardial infarction were lower than those in healthy controls. The values of serum HDL-cholesterol in all cases were in general higher rather in femal than male.

9.Changes on the urinary Prostaglandin E2 Values in the Upper Urinary Calculi.

Although some recent studies showed that increased synthesis of vasodilatory prostaglandin E2 were possible factor in the maintenance of renal blood flow during the states of impaired renal perfusion. However this fact is not studied in cases of upper urinary tract calculi yet. This study is to estimate the value of urinary prostaglandin E2 in cases of upper urinary calculi. The amount of prostaglandin E2 in urinary excretion was measured by radioimmunoassay in ten cases of complete ureteral obstruction compared with normal side, in eleven cases of partial ureteral obstruction compare with normal side and in three cases of bilateral renal calculi. The results obtained were summerized as follows: 1. The values of urinary prostaglandin E2 were highly elevated in cases of bilateral renal calculi. 2. In ten cases of complete ureteral obstruction, the values of urinary prostaglandin E2 decreased in comparison with those of the normal side. 3. In eleven cases of partial obstructions of the ureter, urinary prostaglandin 2, values were elevated than those of the normal side.

10.A study of screening for anemia in 9 month old infants in well baby clinic.

Iron deficiency remains the most common cause of anemia in infants and chidren despite increasing availability of iron-fortified foods. We screened out anemia in 9-month old infants in well baby clinic to know the prevalence of anemia and the weaning status. The results were as follows: 1) Among 345 infants screened, 24 infants (7%) were found to have anemia. 2) The kind of anemia was microcytosis and 7 infants (2.2%) revealed hypochromia. 3) Among the remaining 311 non-anemic infants, 6 infants (1.9%) revealed microcytosis and 7 infants (2.2%) revealed hypochromia. 4) In anemic group the mean hemoglobin concentration was 8.75+/-1.03gm/dl, hematocrit was 27. 73+/-3.11%, MCV was 65.84+/-8.21 fL, MCHwas 20.95+/-2.93 pg and MCHC was 31.34+/-1.89 gm/dl. 5) In 9 anemic infants, weaning was delayed until 9 months of age.
