1.A Case of Infantile Polycystic Kidney.

The infantile polycystic kidney disease is rare fetal urinary tract anomaly. It is inherited with an autosomal recessive pattern and recurrence rate is 25%. The gene locus is on chromosome 6p. The pathogenesis of infantile polycystic kidney is the primary defect of the collecting ducts. The ultrasonographic finding of infantile polycystic kidney is oligohydramnios, bilaterally symmetrical enlarged kidneys with maintenance of their reinform shape. The differential diagnosis with adult polycystic kidney disease is important. The massive enlargement of the kidneys is rarely seen in adult polycystic kidney disease and the examination of the parents and other members of the family is helpful to confirm the adult polycystic kidney disease. If there is severe renal involvements, stillbirth or neonatal death secondary to pulmonary hypoplasia would be developed. If it were diagnosed before viability, termination of pregnancy is recommended. In a fetus at risk, diagnosed after viability, pregnancy termination is also recommended since this condition is uniformly fatal. We present a case of infantile polycystic kidney.

2.A Case of Similar Pattern of Hepatotoxicity after Propylthiouracil and Methimazole.

A 46-year-old male patient developed jaundice after 23 days' treatment of propylthiouracil (PTU) for Graves' disease. Serum alkaline phosphatase was elevated markedly with moderate increase in serum aminotransferase, and the peak level of total bilirubin was 7.3 mg/dL. After withdrawal of PTU, serum aminotransferase and bilirubin began to decrease simultaneously, and completely normalized. One week after the discharge from hospital, he received radioiodine treatment for Graves' disease, but he showed aggravation of hyperthyroidism and ophthalmopathy. So we prescribed methimazole inevitably 16 days' after the radiodiodine treatment. He developed jaundice again after 69 days' treatment of methimazole, but the pattern of hepatotoxicity was slightly different from that of the previous PTU-nduced hepatotoxicity. Serum aminotransferase increased slightly and quickly normalized after discontinuance of methimazole, but serum total bilirubin increased continuously and reached to 24.6 mg/dL. Eosinophilia was prominent and the result of liver biopsy was compatible with cholestatic jaundice. The patient was treated with prednisolone and recovered from jaundice rapidly.

3.Clinical Significance of Double Contrast Arthrography

Double contrast arthrography has been widely accepted as a method to diagnose the internal derangement of the knee. It is a safe and technically simple procedure permitting accurate diagnosis of lesions of the menisci. In 20 cases operated upon, with the diagnosis of internal derangement of the knee, we performed double contrast arthrography before surgery. We analysed the cases concerning the diagnostic accuracy of arthrography, its clinical significance and discrepancies between arthrographic findings and postoperative lesions. The results were as follows: 1. The cases consisted of 16 cases of meniscal lesion, 4 cases of ligamentous lesion and 3 cases of loose body. 2. In the meniscal lesions, 14 of the 16 cases (87.5%) were diagnosed accurately by arthrography, Therefore arthrography was more useful in diagnosing meniscal lesions as compared with clincal symptoms (69%). 3. In cases of ligamentous lesions arthrography was not significant-ly helfpful, in contrast clinical signs afforded important clues. 4. We encountered 1 case of false positive and 1 case of false negative.

4.Tripe synchronous primary lung cancer: one case report.

Multiple primary lung cancer is not common and classified as a synchronous primary lung cancer and a metachronous primary lung cancer. We experienced one case of the triple synchronous primary lung cancer of different cell types. We conducted right pneumonectomy for preoperative diagnosed neuronendocrine tumor of the RUL and adenocarcinoma of the RLL. Pathologic examination revealed the carcinoid tumor of RUL bronchus, the squamous carcinoma of the RML and the adenocarcinoma of the RLL.

5.The Effectiveness of Free/Total Prostate Specific Antigen (PSA) Ratio for Increasing the Detection Rate of Prostate Cancer in Patients with Serum PSA Level 4-10ng/ml.

PURPOSE: We studied how to increase the detection rate of prostatic cancer in men with normal DRE and the diagnostic gray zone of PSA in whom transrectal prostate biopsies were done. MATERIALS AND METHODS: We analyzed 132 patients with normal DRE and serum PSA greater than 4ng/ml, in whom transrectal prostate biopsies were done between January 1, 1996 and July 31, 2000. These patients were divided into two groups, one group having normal DRE and serum PSA level between 4 and 10ng/ml, in whom transrectal prostate biopsies was done and another group having normal DRE and serum PSA level between upper limit of age-specific PSA reference ranges (APSA-R) and 10ng/ml, in whom transrectal prostate biopsies was done. We applied PSAD and F/T PSA ratio to each group. RESULTS: Eighty-one patients with serum PSA level between 4 and 10ng/ml and 67 patients with serum PSA level between upper limit of APSA-R and 10ng/ml, underwent prostate sextant biopsies. In each group, prostate cancer detection rates were 7.4% and 7.5%. When we applied F/T PSA ratio cut off of 0.15 in each group, prostate cancer detection rates were 19.4% and 22.7%, individually (p<0.05). CONCLUSIONS: Application of F/T PSA ratio can be used significantly to elevate the detection rate of prostate cancer and reduce safely the number of biopsies performed in patients with gray zone of serum PSA levels.

Sex cord-stromal tumors of the ovary are the third most common types of neoplasms that develop in the ovary and account for about 5-8% of all ovarian malignancies. This group of ovarian neoplasms is derived from the sex cords and the ovarian stroma or mesenchyme. Granulosa-stromal cell tumors include granulosa cell tumor, thecoma and fibroma. 8.The Association of Ovarian Preservation during Hysterectomy with Obesity in Premenopausal Women.

OBJECTIVES: To determine if hysterectomy with or without ovarian preservation is asso-ciated with obesity in premenopausal women. METHODS: 581 women for routine check up from Jan. 1985 to Dec. 1995 in the depart-m ent of Gynecology at Korea Veterans Hospital were included. The obesity was evaluated by BMI(body mass index) scores. 80% of hysterectomy were confirmed from the hospital records. The age, weight, height, blood pressure, menstr- ual history, history of DM and hypertension, social history of alcohol and smoking and other cardiovascular risk factors such as cholesterol, HDL-cholesterol, triglycerides were checked. The 248 women with natural postmenopausal status before hysterectomy, taking hormonal replacement therapy after hysterectomy and having performed hysterectomy due to malign- ancy were excluded. RESULTS: All women performed hysterectomy were in premenopausal status in this study. Therefore, the final numbers of subjects included in the analysis was 333. Hysterectomy wit- hout ovarian preservation were performed in 58 cases and hysterectomy with ovarian pres- ervation were in 57 cases. The 218 premenopausal cases which did not performed hystere- ctomy were considered control group. BMI scores were higher in cases without ovarian pres- ervation than with ovarian preservation. And BMI score was associated with the postoper- ative duration in hysterectomy with ovarian preservation. CONCLUSIONS: We suggest that hysterectomy without ovarian preservation in premeno- paussal women were associated with increased obesity, especially BMI scores.

9.Autogenous Bone Graft with Bone peg for the Medial Tibial Bone Defect in Total Knee Arthroplasty.

PURPOSE: The aim of this study is to report the usability of autogenous bone graft with bone peg in total knee arthroplasty in the patients with tibial medial bone defect from osteoarthritis due to severe varus deformity. MATERIALS AND METHODS: From July 1998 to December 2000, fifteen total knee arthroplasties with autogenous bone graft were performed for the medial tibial bone defects. The proximal tibia was resected and then bone pegs which were prepared from resected portion of distal femur and proximal tibia were inserted into the defect site with the shape of peg in the anterior posterior view using press fitting method. Instead of applying screw or K-wire, we performed press-fitting method for early stability. For attaining the stability, we preserved the sclerotic rim and designed precisely the bony defect site. We reviewed the result using the HSS Knee Rating Scale and observed the radiologic change. RESULTS: In HSS Knee Rating Scale review, 52.4 of preoperative score was improved to 84.7 at postoperative 1 year. Tibio-femoral angle , the average 17.8 degrees of varus was improved to the average 5.8 degrees of valgus at 1 year postoperatively. The average union period was 5 months, and there was no evidence of the displacement of grafted bone and loosening of the inserted implants. CONCLUSION: The method of autogenous bone graft applied with bone peg could be supposed as the stable fixation method without using screws or K-wires.
