1.Inductoin of Radioresistance by Overexpression of Glutathione S-Transferase K1 (hGSTK1) in MCF-7 Cells.

PURPOSE: This study was conducted to assess the effects of x-irradiation on the expression of the novel glutathione S-transferase K1 gene. MATERIALS AND METHODS: Human glutathione S-transferase K1 (hGSTK1) DNA was purified and ligated to a pcDNA3.1/Myc-His(+) vector for the overexpression of hGSTK1 gene. MCF-7 cells were transfected with or without the recombinant hGSTK1 gene, and irradiated with 6 MV x-ray. After incubation of 14 days, cell survival was measured and compared. The expression of hGSTK1 and the effect of x- irradiation on hGSTK1 expression were also estimated in MCF-7 cells transfected with or without the hGSTK1 gene by RT-PCR. RESULTS: Following 2 to 12 Gy of x-irradiation, the cell survivals were higher in the MCF-7 cells transfected with the hGSTK1 gene than in those without transfection. Despite the higher cell survival in the hGSTK1-transfected cells, RT-PCR for hGSTK1 mRNA revealed no significant differences according to radiation dose, fractionation, and time after irradiation. CONCLUSION: The MCF-7 cells transfected with the hGSTK1 gene showed higher cell survival than those without transfection of the gene. The hGSTK1 gene might be associated with the radiosensitivity of MCF-7 cell line and further analysis should be needed.

2.Positivity of Hepatitis B Virus(HBV) Markers and Changes of HBV Markers After Vaccination Among Healthy Works.

Hepatitis B Virus(t-IBV) infection is one of the major health problems in Korea and HBsAg positive rate was known to be as 6-10% in general population. This study was carried out to determine HBV infection rate of workers who worked in one company and to assess the conversion rate of HBV markers(HBsAg: by RPHA method, Anti-HBs: PHA method) from 1990 to 1992. A total number of study subject was 1,972 male workers who were interviewed during health check up at 1992. Healthy workers were defined those who have none of the past histories of transfusion, hepatitis, abnormal liver function. The results were as follows: 1. HBsAg positive rate among healthy workers in 1990 was 2.7% and it was highest in age group thirties as 3.0% with no statistically significance by age group. 2. HBsAg positive conversion rate was 3.0% and negative conversion rate was 7.7% from 1990 to 1992. 3. The rate of Anti-HBs formation was 32.5% after 3 doeses hepatitis B vaccination.

3.Pulmonary coccidioidomycosis Diagnosed in an Immigrant.

Coccidioidomycosis is a common infectious disease in southwestern North America, which is caused by the soil fungus, Coccidioides immitis. Due to the mobility of the modern population, increasing numbers of cases are being diagnosed outside the endemic areas. Moreover, diagnosis in non-endenmic areas may be delayed or confused due to its rarity. Here we report a case of pulmonary coccidioidomycisis in an immigrant. The patient presented with unexplained pulmonary symptoms. A history of recent travel to or immigration from an endemic area may suggest coccidioidomycosis. In addition an early tissue biopsy is helpful in establishing this diagnosis.

4.Evaluation of Nutritional Status of Inpatients with Medical Health Problems.

PURPOSE: To investigate nutritional status of inpatients by using subjective. and objective evaluation methods and to find the relationship between them. METHOD: The subjects were 101 inpatients with medical health problems at a university hospital. Nutritional status was evaluated by the Subjective Global Assessment (SGA) and physical assessment including percentage of weight loss, serum albumin, hemogloin, and hematocrit. Data were analyzed using frequency, percentage, mean, Kendall's tau. RESULTS: Grouping by the SGA, 61.4% were classified as severe malnutrition group. When applying the objective methods{physical assessment). 1.9~42.6% were diagnosed as malnutrition each item. The percentage of weight loss during previous 1-6month(r=.43~.54. P=.0001), serum albumin(r=-.26, P=.0003), hemoglobin of male(r=-.38, P=.0001), and hematocrit of male(r=-.34, P=.0001) were significantly correlated with SGA score. The coincidence rate of nutrition evaluation between the objective methods and SGA were 27.7 35.6%, 20.8%, 47.5%, 58.4% in percentage of weight loss, albumin, hemoglobin, and hematocrit. CONCLUSION: These findings showed a majority of inpatients were exposed to the risk of malnutrition. We recommend to evaluate inpatients' nutritional status periodically and to develop nursing intervention to solve their nutritional problems.

5.Relationship between the Discomfort of Tinnitus and the Quality of Life in Tinnitus Patients.

PURPOSE: To investigate audiometery and characteristics related to tinnitus in tinnitus patients. and to understand the relationship between the discomfort of tinnitus and the quality of life. Methods: The subjects were 110 patients with tinnitus. The data were collected using tinnitus handicap questionnaire. quality of life, and pure tone audiometry, which measured hearing level in dB and frequency in Hz matching of tinnitus. RESULTS: Majority of tinnitus patients (75.5%~81.8%) had normal auditory sensation. but 82.7% of patients indicated that they have heard tinnitus frequently. or always, and the major sounds of tinnitus were wing, wung, wang, The matching sounds of tinnitus had various range in hearing level(dB) and frequency(Hz). Tinnitus patients reported moderate levels of the discomfort of tinnitus and the quality of life, and there were negative correlation between the discomfort of tinnitus and the quality of life(r=-.314, p .00l). CONCLUSION: These findings suggest that special attention is needed for tinnitus patients because the physical. psychological. and social impacts of tinnitus could be decrease their quality of life, Specialized nursing intervention considering specific conditions related to tinnitus is required to solve various health problems of tinnitus patients.

6.Clinical Study of Calcaneous Fracture

Eighty four patients with 104 calcaneal fractures which were treated at the dept. of orthopaedic surgery, Inchon Christian Hospital from October, 1979 to September, 1983 were clinically analysed and the results were as follows; 1. 72 patients were male and 12 patients were female. The male were more injuried in the ratio of 6:l. 2. Their ages were ranged from 6 yrs. to 66 yrs. (average; 35.4 yrs.) and the half of the patients were occured at the 3rd and 4th decade. They admitted for average 7.6/7 weeks. 3. The main cause was a fall from a height in 62 patients (73.8%) and its average height was 4.5m. 4. The open fractures were occured in 13 feet (12.5%) and the spine fracture was associated in 16 patient (19%). 5. The 104 cases were classified according to the Rowe and Essex-Lopresti classification. Fractures involving subtalar joint were 74 feet (71.2%) and the Rowes type V were 58 feet(55.8%). 6. The 22 cases were treated by elosed reduction and axial pin fixation, 10 cases were treated by open reduction and the subtalar fusion was done at 2 cases. Others were treated by cast immobilization with or without closed reduction. The satisfactory result was obtained in 64 cases(61.5%). 7. The most common problem after any treatment was persistent foot pain, especially beneath the lateral malleolus.

7.Clinical Observation of Corneal Graft: An Interim Report of 25 Keratoplasties.

Authors performed 25 corneal grafts in 24 patients since the inauguration of The Central Eye Bank, attached to the Department of Ophthalmology, St. Mary's Hospital, Catholic Medical College, in April 19, 1967. Method and results of these cases are as follows; METHOD: In technique of surgery, the grafts in most of the cases were 7 mm in diameter(Table 3), in two types of penetrating and lamellar corneal grafts(Table 4). The donor material came from patient age group of 51-60 years old in most and was used within 24 hours after death (Table 5). In most cases we placed 12 interrupted sutures except of two cases for continuous suture by 8-0 virgin silks. Preoperative and postoperative cares were routine with systemic dexamethasone, 7.0mg a day was given routinely on the 5 th postoperative day for 30-50 days because of prevention of graft rejection. RESULTS AND SUMMARY: Table 7 summbrizes the results of our observation. Nineteen of the 25 grafts remained clear. And there was improved vision above 20/200 in ten eyes out of nineteen clear grafts. Table 10 lists the reasons why nine clear grafts did not improve vision. Postoperative complications occurred in fourteen grafts (Table 11). Penetrating corneal grafts for adherent leucoma courneae, staphyloma corneae and corneal scars from alkaline burn were failed to maintaining the clear graft. The donor material, the recipient cornea, graft rejection as a complication and its prevantion were discussed. 8.Mutagenicity of Human Urine Excreted after Ingestion of Roast Beef.

This study was undertaken to observe the mutagenic occurrence in urine excreted after the ingestion of roast beef. Two healthy nonsmoker persons of both sex were selected for this test, employing two strains (TA98, TA100) of Salmonella typhimurium according to Ames' method. The mutagenic activity began to appear in urine of both sex three hours after ingestion of 300 g of roast beef, gradually increasing until 6 hours and declining thereafter.

9.Outcome of Aflibercept-Bevacizumab Alternate Dosing in Neovascular Age-related Macular Degeneration with Limited Response to Aflibercept

Purpose:
To investigate the efficacy of aflibercept-bevacizumab alternate dosing (AD) in neovascular age-related macular degeneration (AMD) with a limited response to bimonthly aflibercept injections.
Methods:
This retrospective study included patients given aflibercept-bevacizumab AD to treat neovascular AMD with bevacizumab given every alternate month between bimonthly aflibercept injections when they had a limited response to bimonthly aflibercept alone. The best-corrected visual acuity (BCVA) and central retinal thickness (CRT) were compared before AD and after the last AD. The incidence of subretinal fluid (SRF) and intraretinal fluid (IRF) before and after AD was also examined.
Results:
The study included 18 patients. The mean period between the diagnosis and AD was 37.9 ± 18.8 months and a mean of 3.1 ± 2.2 ADs were performed. The BCVA improved significantly from 0.45 ± 0.28 before AD to 0.36 ± 0.23 after AD (p = 0.036). The CRT decreased significantly from 440.3 ± 122.9 μm before AD to 317.7 ± 103.5 after AD (p = 0.001). The SRF was present in 94.4% and IRF in 55.6% before AD and in 22.2% and 11.1%, respectively, after AD.
Conclusions
Aflibercept–bevacizumab AD was effective at resolving SRF/IRF in neovascular AMD with a limited response to bimonthlyaflibercept injections and should be a useful treatment option.
