5.The Clinical Results of Tension Band Wiring

We have experienced 52 intra-articular fractures(24 malleolar, 17 patellar, 11 olecranon) treated with tension band wiring from 1972 to 1981 and the results are as followings; 1. The operative technique is relatively simple and easy. 2. In avulsion fractures without communition, anatomical reduction and rigid internal fixation can be achieved. 3. In comminuted fractures, the fragments can be securely fixed with a few additional K-wires including tension band wiring. 4. In avulsion fractures, where the fragment is either too small for screw fixation or too osteoporotic a tension band wiring can be applied with no difficulty. 5. Early joint motion reduces the functional disability and promote the fracture healing.

7.Examination of Endoscopic Mucosal Resection of Early Gastric Cancer.

BACKGROUND/AIMS: The endoscopic mucosal resection has now been adopted for a useful modality in both curative therapy and accurate diagnosis of early gastric cancer. A retrospective study was done to evaluate the indication and the limitation of endoscopic mucosal resection of early gastric cancer. METHODS: We studied 20 cases of early gastric cancer treated by endoscopic mucosal resection in Chungnam National University Hospital from November, 1995 to July, 1997. RESULTS: 1) The size affected the curability: 83. 3% of lesions less than 2 cm and 50. 0% of those larger than 2 cm were resected completely. 2) The depth of cancer invasion affected the curability: 87. 5% of lesion confined to mucosa were resected completely, while all of submucosal cancers were resected incompletely. 3) Among fourteen cases resected completely, three cases of lesions larger than 2 cm were residual cancer and two of those less than 2 cm were recurred locally. CONCLUSIONS: To be a curative therapy by endoscopic mucosal resection of early gastric cancer, we think that careful selection of the lesion, that is lesion less than 2cm and confined to mucosa, is necessary

8.Unusual Cutaneous Neutrophilic Infiltration in Myelodysplasia Syndrome : A Nuclear Segmentation Anomaly.

We report a case of myelodysplastic syndrome (MDS) with unique histopathological findings in which numerous infiltrated neutrophils exhibited nuclear segmentation anomalies. In comparison with well-described neutrophilic dermatoses of myelodysplastic syndrome such as Sweets syndrome and pyoderma gangrenosum, this case took a rapid and aggressive clinical course. We suggest that a nuclear segmentation anomaly, pseudo Pelger-Hiiet anomaly, is likely to reflect acute transformation of MDS and can be a poor prognostic marker.

9.A Case of Similar Pattern of Hepatotoxicity after Propylthiouracil and Methimazole.

A 46-year-old male patient developed jaundice after 23 days' treatment of propylthiouracil (PTU) for Graves' disease. Serum alkaline phosphatase was elevated markedly with moderate increase in serum aminotransferase, and the peak level of total bilirubin was 7.3 mg/dL. After withdrawal of PTU, serum aminotransferase and bilirubin began to decrease simultaneously, and completely normalized. One week after the discharge from hospital, he received radioiodine treatment for Graves' disease, but he showed aggravation of hyperthyroidism and ophthalmopathy. So we prescribed methimazole inevitably 16 days' after the radiodiodine treatment. He developed jaundice again after 69 days' treatment of methimazole, but the pattern of hepatotoxicity was slightly different from that of the previous PTU-nduced hepatotoxicity. Serum aminotransferase increased slightly and quickly normalized after discontinuance of methimazole, but serum total bilirubin increased continuously and reached to 24.6 mg/dL. Eosinophilia was prominent and the result of liver biopsy was compatible with cholestatic jaundice. The patient was treated with prednisolone and recovered from jaundice rapidly.

10.Effects of Androgen on Expression of Androgen Receptor mRNA and Penile Erection in the Rat.

PURPOSE: Although it is well known that androgens have beneficial effects on erectile dysfunction, their precise roles have been contentious. Recently it has been shown in the rat that castration induces loss of penile reflexes and considerable reduction in the erectile response to electrical stimulation(ES) of the cavernosal nerve. Both of these effects can be reversed by testosterone replacement. In this study, we have investigated the effects of androgen on the expressions of androgen receptor(AR), penile reflex(glans engorgement, cup, flip), erectile response to electrical stimulation, and penile NOS activity. MATERIALS AND METHODS: Male Sprague-Dawly rats(300-35Og) were castrated and implanted with silastic brand silicone tube containing dihydrotestosterone (DHT) or testosterone with or without daily injections of the 5a-reductase inhibitor(MK434). Animals of control groups were either sham operated or castrated with no androgen supplementation. All animals were maintained for 7 days under normal animal house conditions. Penile tissues were removed and processed either for Northern hybridization using [a-32P]UTP-labelled cKNAs and for measurement of NOS activities. In penile reflex, number of glans engorgement. flip and cup were measured in each group and compared with control. And, ES-induced intracarvenosal pressure(ICP)/systolic blood pressure(SBP) ratio and penile NOS activities of each groups were compared with control. RESULTS: Penile AR mRNA expressions were down-regulated by androgen Penile reflex erection was significantly reduced in castrated group and restored to the control level by androgen supplementation. However, the reflex erection was decreased in MK434-treated group. Erectile response to ES and penile NOS activity of each group showed similar patterns. CONCLUSIONS: These results showed that androgen might play a important role in regulation of penile erection and its actions were mediated by AR. Androgens, especially DHT, were essential modulators of the normal penile function including erectile relaxation of the cavernous smooth muscle, their effects being mediated, at least partially, by changes in nitric oxide synthase levels.
