4.A Case of Sarcoidosis Showing Various Cutaneous Manifestations.

Sarcoidosis is a chronic idiopathic disorder characterized by the accumulation of noncaseating granulomas. We report a case of sarcoidosis with various cutaneous manifestations in a 42-year old female. On cutaneous examination, there were multiple papules on the posterior neck and anterior chest, subcutaneous nodules along an old scar of the right scapular area, violaceous nodule with satellite lesions and subcutaneous nodule on the right knee. Histopathology revealed noncaseating granulomas in all 5 specimens. She was treated with oral prednisolone leading to an excellent clinical response within 3 months of treatment, and remains well without recurrence for follow-up period of 20 months. .

5.A Case of Spindle Cell Hemangioma.

Spindle cell hemangioma is an uncommon variant of vascular tumor, and it was first described as a unique vascular lesion that's characterized by cavernous blood vessels separated by spindle cells. It is recently thought to be a benign hemangioma with a reactive process. It mostly affects the distal extremities and it present as single or multiple lesions that are often painful. Histologically, thin-walled cavernous vessels are seen to be intermixed with varying proportions of bland spindle cells with vesicular nuclei. We report here on an interesting case of spindle cell hemangioma in a 39-year-old woman.

6.Two Cases of Warty Dyskeratoma.

We report two cases of warty dyskeratoma which is uncommon skin lesion. A 50-year-old male presented with an asymptomatic dermal nodule on periauricular area. Histopathologic findings showed the dermal cystic structures lined by epithelial wall of papillomatosis and suprabasal cleft formation with numerous villi and acantholytic cells. The second case was a 48-year-old male with an asymptomatic papule on cheek. Histopathologic finding showed the cup-shaped keratin-filled invagination lined by papillomatotic wall and filled with numerous acantholytic, dyskeratotic cells. Both findings were the characteristic features of warty dyskeratoma.

7.Analysis of Dermatologic Consultations in Neuropsychiatric Inpatients.

BACKGROUND: Many patients seeking treatment for skin disorders have an underlying psychiatric problem that either causes or exacerbates a skin condition; thus, the demand for dermatologists' expert opinions on such cases is increasing. OBJECTIVE: The purpose of this study was to analyze and quantify the pattern of dermatologic consultations referred by the department of neuropsychiatry in a tertiary hospital. METHODS: We retrospectively studied data from 337 inpatients referred by the department of neuropsychiatry for dermatologic consultation during an 8.5-year period in a tertiary hospital. RESULTS: The percentage of department of neuropsychiatry patients who were referred to the department of dermatology for consultation was 15.3%. The most prevalent age group for referrals was 30~39 years (26.4%), and the male-to-female ratio was 1:1.6. Bipolar disorder was the most common underlying neuropsychiatric problem (31.1%), and eczema (36.5%) was the most common dermatoses, followed by infectious disease (27.6%). The main reasons for dermatologic consultations were dermatologic disease (71.5%) and treatment-related dermatoses (10.1%). Thirty-five cases were discordant as to the reason for consultation between psychiatrists and dermatologists. Neuropsychiatric medication-related adverse reactions were reported in 9.8% of referred patients. CONCLUSION: This study revealed the distribution of skin disorders in patients referred by department of neuropsychiatry for dermatologic consultation. We believe its findings could be helpful as educational material to encourage cooperation between the specialties of dermatology and neuropsychiatry.

8.A Case of Impetigo Herpetiformis Occurring In Puerperium.

Impetigo herpetiformis is a rare, severe dermatologic disorder occurring predominantly in the third trimester of pregnancy. Rarely, it may present during the puerperium, and this suggests that impetigo herpetiformis should be included in the differential diagnosis of puerperal fever, particularly in those cases associated with dermatoses. We report a 32-year-old woman who developed impetigo hermpetiformis during puerperium. She was a primigravida and did not have a previous history of psoriasis. Three days after parturition she developed a severely pruritic pustular rash composed of erythematous patches first presented in both axilla. The patient showed rapid improvement with oral prednisolone 40 mg/day for 5 days, and the lesions healed in 10 days. Follow-up for 12 months revealed no recurrence.

9.A Case of Tuberculosis Verrucosa Cutis.

We report a case of tuberculosis verrucosa cutis in a 39-year-old male patient, who presented verrucous plaque on the dorsum of right third finger. Seven months ago, he had a cut with paper and skin lesion developed. Histopathologic features revealed parakeratosis, irregular acanthosis in the epidermis and inflammatory cellular infiltrates and tuberculoid granulomas in the dermis. AFB- positive bacilli were demonstrated. He has been treated with isoniazid, rifampicin, ethambutol, and pyrazinamide for 4 months till now and the skin lesion has been markedly improved.

10.Repair of Skin Defects after Excision of Plantar Tumors.

BACKGROUND: The plantar aspect of the foot is a difficult area for wound coverage and offers restricted/limited possibility for surgical repair. Due to the shearing and ground-reactive forces that the sole of the foot encounters and its unique organizational makeup of plantar skin and fat pad, reconstructive surgical techniques used on other parts of the body cannot be readily adapted to the plantar foot. OBJECTIVE: The authors present an overview of wound repair after excision of benign or malignant tumor on plantar area. METHODS: Retrospective review was done on 27 patients whose benign or malignant tumors on the plantar area were excised. From January 2007 to December 2012, we repaired defects on the plantar foot after simple excision of benign cutaneous tumors or wide excision of malignant melanomas. Patients were reviewed for results and complications. RESULTS: Simple elliptical closure was used for the defects of which the short diameter was below 1.5 cm in 13 patients. Transposition flap was used in 2 patients. The local flap was used when the length to width ratio was smaller than 3 to 1, and the laxity of the skin adjacent to the defect was available. Larger defects of other 12 cases needed to be reconstructed with full thickness skin graft (FTSG) or by secondary intention healing (SIH). The re-epithelialization was shown to be faster in the FTSG group, but the functional and cosmetic results were better in the SIH group. CONCLUSION: Many defects on the sole were difficult to repair because of the distinct anatomy. In order to treat such large wounds in weight bearing area, both FTSG and SIH are relatively good options.
