2.A Case of Spontaneous Intracranial Hypotension: Detection of Cerebrospinal Fluid Leakage by Early Dynamic Radionuclide Cisternography.

Although cerebrospinal fluid leakage is suggested as one of the causes of spontaneous intracranial hypotension, on]y a few cases with direct evidence of cerebrospinal fluid leakage on radionuclide cisternography have been reported in the literature Indirect evidences of cerebrospinal fluid leakage such as early visualization of the soft tissue and bladder or delayed migration of radiotracer have been observed in most patients with spontaneous intracranial hypotension. We report a case of spontaneous intracranial hypotension in which cerebrospinal fluid leakage was directly demonstrated by early dynamic imaging of spine on radionuclide cisternography. We suggest that early dynamic imaging of spine is an important adjunctive procedure in detecting cerebrospinal fluid leakage in patients with spontaneous intracranial hypotension.

6.Serum Interleukin-6 Concentration in Carcinoma of the Cervix.

Interleukin-6(IL-6) is a multifunctional cytokine, and deregulated IL-6 gene expression has been implicated as being involved in the pathogenesis of a number of diseaaes. So we measured the serum level of IL-6 to find a relationahip between uterine cervical cancer and serum IL-6 level. The serum levels of IL-6 were meesured m 54 patients adrnitted to the Severance hospital, from August, 1992 to February, 1993, of whom 27 patients were diagnosed as invasive cancer of cervix, 17 as CIN, and also utilizing 10 patiente diagnosed as uterine myoma as a control group for matched age and parity. The average serum level of IL-6 in invasive cancer, CIN, and uterine myoma were respectively 30.33pg/ml, 37.27pg/ml, and 12.45pg Jml. In 22% af cervical cancer patients (6 of 27), and 27% of CIN patients (4 of 17) studied blood levels of II-6 were detected whereas only 10% of uterine myoma (1 of 10) shorved the same result. However, the difference does not seem to be statistically significant, There was no statistically significant difference among any of the groups, and age and stage did not show any significant cmrelation, and the results need furt,ber investigations.

7.Efficacy of the Preliminary Ultrasonographic Examination before Suprapubic Cystostomy by Trocar.

Suprapubic cystostomy by trocar is a simple, safe and useful method for urinary diversion in acute urinary retention. But rarely, severe complications occur during or after suprapubic cystostomy such as bowel perforation, through and through perforation of the bladder, rectal injury and severe hematuria. We studied the complications of suprapubic cystostomy retrospectively and compared 19 patients who performed ultrasonographic examination with 30 patients without ultrasonographic examination. The complications in 30 patients without preliminary ultrasonographic examination were initial failure in 4 cases (13.3%), extravesical Foley indwelling in 2 cases (6.7%), puncture site closed to the pubic bone in 2 cases (6.7%), spontaneous bladder rupture in 1 case (3.3%) and bowel injury in 1 case (3.3%). But, the complication in 19 patients with preliminary ultrasonographic examination was only 1 retried case (5.3%) because of extravesical Foley indwelling. As above results, the brief preliminary ultrasonographic examination of the bladder is a very useful method with minimal risk during suprapubic cystostomy by trocar.

9.Flow cytometric DNA ploidy analysis in prostatic adenocarcinoma: a comparison with clinical stage, histopathological grade and prognostic significance.

Recent studies suggest the flow cytometric DNA ploidy analysis may be useful in defining the biologic behavior and prognosis in prostatic adenocarcinoma. Flow cytometric nuclear DNA ploidy analysis was used to study the relationship between DNA ploidy, clinical stage and histopathological grade in thirty two patients with prostatic adenocarcinomas diagnosed from 1987 to 1990. The incidence of aneuploidy in the total population was 18 of 32 (56.3%). The frequency of aneuploidy increased with advancing stage and 63.2% of carcinomas with distant metastases were aneuploidy. Aneuploidy was more frequent in high Gleason sum carcinomas than in low. The incidence of aneuploidy in carcinomas with high Gleason grade (Gleason sum 8 to 10) was 77.8%. comparing to 33.3% in low Gleason grade (Gleason sum 2 to 4). When carcinomas classified according to both DNA ploidy and degree of glandular differentiation, then subgroups with the highest and lowest degree of malignant potential became apparent. None of diploid tumors with low Gleason grade (Gleason sum 2 to 4) formed metastasis, but 71.4% of aneuploidy tumors with high Gleason grade (Gleason sum 8 to 10) formed metastases. The influence of DNA ploidy on survival was examined with Kaplan-Meier method and the generalized Wilcoxon test. Overall, the patients with diploid tumor had a survival advantage over patients with aneuploid tumor (p<0.05). In patients with stage C and D, there was increasing tendency of survival in diploid group. In conclusion flow cytometric determination of DNA ploidy in prostatic adenocarcinoma is correlated strongly with clinical stage and Gleason sum and can be expected to be a valuable adjunct b clinical stage and histopathological grade in the assessment of malignant potential of prostatic adenocarcinoma.

10.Eczema Herpeticum in Darier's Disease.

Eczema herpeticum is a herpes simplex virus infection with disseminated skin involvement superimposed on a pre-existing dermatosis. Dariers disease has been reported to be among the dermatoses susceptible to the sudden onset of a widespread vesicular eruption accompanied by high fever known as Kaposis varicelliform eruption. We report a case of eczema herpeticum in a 46-year-old woman associated with Dariers disease.
