1.Positivity of Hepatitis B Virus(HBV) Markers and Changes of HBV Markers After Vaccination Among Healthy Works.

Hepatitis B Virus(t-IBV) infection is one of the major health problems in Korea and HBsAg positive rate was known to be as 6-10% in general population. This study was carried out to determine HBV infection rate of workers who worked in one company and to assess the conversion rate of HBV markers(HBsAg: by RPHA method, Anti-HBs: PHA method) from 1990 to 1992. A total number of study subject was 1,972 male workers who were interviewed during health check up at 1992. Healthy workers were defined those who have none of the past histories of transfusion, hepatitis, abnormal liver function. The results were as follows: 1. HBsAg positive rate among healthy workers in 1990 was 2.7% and it was highest in age group thirties as 3.0% with no statistically significance by age group. 2. HBsAg positive conversion rate was 3.0% and negative conversion rate was 7.7% from 1990 to 1992. 3. The rate of Anti-HBs formation was 32.5% after 3 doeses hepatitis B vaccination.

6.The Influence on Neural Conduction after Alcohol Injection into Penile Dorsal Nerve of Rats.

PURPOSE: Premature ejaculation is the most common male sexual disorder, affecting perhaps as many as 75% of men, but its cause has not been well established. The objective of this study is to evaluate whether alcohol injection on the dorsal aspect of the penis can induce a desensitization of penile sensory nerve fiber in rats. MATERIALS AND METHODS: Forty-four male Sprague-Dawley rats were divided into 4 groups. Group I of 4 rats, the control group, did not undergo any procedure. Group II of 8 rats, received only an alcohol injection without electrical stimulation. In group III of 4 rats, only electrical stimulation was performed without alcohol injection. In group IV of 28 rats, all rats received an alcohol injection on the dorsal aspect of the penis, and groups of four rats were sacrificed after electrical stimulation on the glans penis at 1, 2, 4, 7, 10, 14, and 21 days after alcohol injection. Spinal cords were sectioned and processed for immunohistochemical staining for c-Fos protein. RESULTS: No c-Fos protein was detected in the normal control group, and either none or few c-Fos protein positive neurons were seen in the alcohol injection only group. The number of c-Fos protein positive neurons in the electrical stimulation only group was 30.25+/-3.53 cells/section. Four days after alcohol injection, the level of c-Fos protein positive neurons was reduced significantly compared with the control group (p<0.05), and was very close to the control group at 10 days after alcohol injection. CONCLUSIONS: Our study demonstrates that alcohol injection in the dorsal aspect of the penis reduces c-Fos protein positive neurons in the spinal cord segment through desensitization of penile sensory nerve fibers.

Total hip arthroplasty (THA) can be a good treatment option for avascular necrosis (AVN) of the femoral head. However, because dislocation can frequently occur after surgery, THA is a concern in patients with a neuromuscular disease, such as cerebral palsy or poliomyelitis. In patients with poliomyelitis, only one case of AVN of the femoral head in the affected limb has been reported in the English literature. Here, the authors report on a case of AVN of the femoral head in a patient with poliomyelitis, who was treated with a large diameter femoral head metal-on-metal THA using a modified minimally invasive-2-incision technique.

8.The Effects of Handgrip and Cold Pressor Test on Mitral Flow in Patients with Left Ventricular Hypertrophy.

BACKGROUND: Handgrip and cold pressor test may increase the afterload of the heart. And in left ventricular hypertrophy, it is known that mitral flow pattern is affected by decreased left ventricular compliance. We investigated the effects of handgrip and cold pressor test on mitral flow pattern in patients with left ventricular hypertrophy. METHODS: Handgrip and cold pressor test were performed in 12 subjects with left ventricular hypertrophy and in 14 healthy normal subjects. In supine position, blood pressure, heart rate and Doppler echocardiographic parameters(early peak flow velocity : E, atrial peak flow velocity : A) were obtained at rest, 1 and 3 minutes after the onset of tests respectively. RESULTS: In both groups, handgrip and cold pressor test increased blood pressure slightly without a statistical significance. In left ventricular hypertrophy group, there were significant increments in heart rates at 1 minute of handgrip (78+/-12min-1, p<0.001) and cold pressor test(77+/-7min-1, p<0.05) as compared to that at rest (73+/-12min-1). Mitral flow velocities did not show significant change after the tests in control group. A waves after 1 minute of handgrip(85.2+/-18.4cm/sec, p<0.05) and cold pressor test (87.3+/-17.8cm/sec, p<0.001) showed significant increases as compared to that at rest (79.1+/-14.9cm/sec) in left ventricular hypertrophy group whereas E waves did not. CONCLUSION: Although handgrip and cold pressor tests did not affect the mitral flow significantly in control group, each test raised A waves in left ventricular hypertrophy group. These results suggest that increased A waves may be due to a rise in afterload and decreased left ventricular compliance caused by handgrip and cold pressor test in left ventricular hypertrophy group.

9.An experimental study on the morphologic changes of rat mandible following occlusal interference.

The occlusal interference during adolescent period makes some effects on growth and development and morphological changes. And so, if we could predict the timing and results of orthodontic treatment who have occlusal interference during adolescent period, it may be helpful for diagnosis and treatment planning of orthodontic treatment. For about those, the purpose of this study was to evaluate the effects of the posterolateral displacement by the metal casting crown with inclined pathway on the mandibular condyle and morphologic changes of mandible in the rat. The experimental animals were thirty six Sprague-Dawley male rats of 8 weeks old. Eight of them was used as control group, and experimental group I (continuous appliance wearing group) was composed of sixteen and experimental group 2 (appliance removal group after worn the appliances during 3 months ) was composed of remaining twelve. The animals of experimental group 1 were sacrificed after 1, 2, 3, 6 months from beginning of the experiment and experimental group 2 were sacrificed 1, 2, 3 months after removal of the appliance from worn the appliance during 3 months. Both of mandible and temporomandibular joint were observed histologically and radiologically. The results were as follows: 1. In experimental group 1, the mandibular length and lower posterior height were decreased with experimental period, while the lower anterior height was increased, and the curvature of lower incisors and lingual inclination of anterior alveolar bone were profound as compared with control group. 2. In experimental group 1, both of the thickness of the condylar cartilage were thinned in the posterosuperior region, and this phenomenon was more prominent on right than left in 3-Mo experimental period and both sides were marked thinned in 6-Mo experimental period. 3. In experimental group 2, the lower anterior height was low and lower posterior height was high as compared with experimental group 1, and the curvature of lower incisors and lingual inclination of anterior alveolar bone were recovered to control group. 4. In experimental group 2, both of the thickness of the condylar cartilage were thickened in the posterosuperior region, and this phenomenon was more prominent with experimental period. 5. In experimental group 2, the mandibular length was short, lower anterior height was hight, the curvature of lower incisors were profound, and in histologically, both of the thickness of the condylar cartilage were thickened in the posterosuperior region as compared with control group. As shown above, the occlusal interfernce affected the condylar cartilage, curvature of lower incisor, inclination of anterior alveolar bone, mandibular length, and anterior and posterior height. When the interference was removed, significant recover was found in condylar cartilage, mandibular length, and posterior height. Although no significance was found, other items of measurement showed trends for recovery.

10.Exploration of Insomnia With Excessive Daytime Sleepiness From Clinical and Polysomnographic Perspectives

Objectives:
Insomnia is a disorder that involves poor sleep quality at nighttime and various symptoms in the daytime. Although it is known to increase cardiovascular risk as well as psychiatric comorbidities such as depression and anxiety, less attention has been paid to daytime sleepiness in patients with insomnia. This study aimed to investigate clinical and polysomnographic characteristics and identify factors influencing excessive daytime sleepiness in patients with insomnia.
Methods:
Between 2014 and 2022, 1,241 patients with insomnia as the primary symptom who underwent polysomnography were included in the study. Patients with an Epworth sleepiness scale ≥10 were classified as having insomnia accompanied by daytime sleepiness, and sleep-related questionnaire responses and polysomnography findings were utilized for analysis.
Results:
A total of 327 patients (26.3%) had daytime sleepiness, and a higher prevalence was observed in younger and male individuals. Poor sleep quality, severe depression, and insomnia were identified through the questionnaire. Polysomnography showed longer total sleep times, a higher proportion of stage N3 sleep, and higher sleep efficiency. The variables related to the Epworth sleepiness scale identified in multiple regression analysis were age, the Korean Beck Depression Inventory-II, sleep latency, and the apnea-hypopnea index on polysomnography.
Conclusions
Insomnia with excessive daytime sleepiness was more affected by depression than indicated by polysomnography findings. The pathophysiology is presumed to be related to sleep misperception, and further studies are needed to elucidate the discrepancy between excessive daytime sleepiness and polysomnography findings.
