1.Comparative study of radiologic-pathologic findings of experimental clonorchiasis in rabbits.

Radiological investigation in patients with clonorchiasis is very important as this is the only method of evaluating the severity of clonorchiasis. In order to correlate the radiologic and pathologic findings of clonorchiasis, fourteen rabbits infested with Clonorchis sinensis and five control rabbits were examined radiologically by ultrasonography, computed tomography and cholangiography and the results were correlated with pathologic findings. Dilatation of the intrahepatic small bile ducts of the liver was due to obstruction by flukes: oval or elliptical small filling defects or irregular margin of the bile ducts on cholangiogram or intraluminal echoes on sonogram represented flukes per se; periductal thickening on sonogram and periductal enhancement of bile ducts on CT were due to inflammatory cell infiltration, adenomatous hyperplasia and periductal fibrosis: band like enhancement at the periphery of the liver on CT represented proliferated bile ducts, destruction of liver cells and resultant fibrosis. The study confirmed the pathological bases for the radiological findings of clonorchiasis in liver and bile ducts and will, perhaps, serve as a basis for the future radiologic-pathological correlation of clonorchiasis and in further clinical and experimental researches in the biliary tract diseases.

2.Ki-I Lymphoma In a Young Adult.

Ki-1 lymphoma is a rare, large cell anaplastic non Hodgkin's lymphoma. It expresses the CD30 antigen and is recognized by the antibodies Ki-1. This Ki-1 positive anaplastic lymphoma was first described in 1985 as a new histological subtype and was added to the updated Kiel classification in 1988. Morphological and immunological features of this lymphoma have been well described, but clinical studies have been limited and follow up has been short. The authors report a case of Ki-1 lymphoma of the skin without systemic involvement in a young adult based on clinical, pathological and immunological features.

3.A Case of Squamous Cell Carcinoma and Bowen's Disease Associated with Superficial Disseminated Porokeratosis.

We describe the clinical and pathologic observation of a 50-year-old man with superficial disseminated porokeratosis who developed a squamous cell carcinoma on the dorsum of his right thumb and Bowens disease on his right upper arm. The tumors were surrounded by lesions of superficial disseminated porokeratosis and were thought to develop from the dysplastic epidermal cells located under the comoid lamellae.

6.Operative Errors in Interlocking Intramedullary Nailing for Tibial Fractures

From retrospective review of 111 tibial fractures treated by interlocking intramedullary nailing, 38 operative errors in 28 cases(25%) were included in this study. The errors were classified into two categories. The first category included eighteen errors that related to fracture reduction; nine angular malalignments, four failures in closed nailing to open the fracture site, two peroneal nerve palsies, two shortenings of tibial length(more than 1cm), and one rotational malalignment. The second category comprised twenty errors that related to the nailing itself; eight protrusions of proximal nail tip, five overdistractions of fracture gap (more than 3mm), four additional fragmentations, two mistakes in interlocking screw insertion, and one ankle joint injury by the distal nail tip. Functional results of those 28 cases at the last follow-up(average, 17.5 months; range, 9 to 29 months) were excellent or good in only 60.8 per cent, while the control group treated by adequate operative technique showed excellent or good results in 92.8 per cent.

8.Survey on the Visual Impairments and Ocular Health Status in Urban School Students.

As a partial effort of the survey in the National Plan for the Eye Health, the analysis of the visual impairment mainly refractive errors among 4,004 School Children in Seoul(1986) with the ancilliary questionnaire on various conditions was attempted. The number of the student with subnormal vision(0.7 or less) was 1,552(38.8 %), and this rate was increased with the higher school grade. Of the 2,551 eyes with subnormal vision(0.7 or less), the confirmed refractive errors consisted of myopia 2,170(78.7%), hyperopia 237(8.6%), and mixed astigmatism 144(5.2%) eyes. The number of glasses wearer among the students of vision 0.5 or below(1,129) was 770(63.2%), of which the under-corrected cases 380(49.3%) weregreater in number with the over corrected cases. The causes of not-wearing glasses in the students who belonged to the vision 0.5 or below indicated in prominence of numbers of 1) no complaints without glasses in 172(42.0%), 2) unwareness of the visual impairment in 77(20.0%), and 3) annoyance to wearing glasses in 51(13.4%), rather than economical reasons in 12(3.1%). The amblyopic components were estimated among 129 cases(3.2%) in combination of refractive errors. Further program to supply the glasses with appropriated prescription and the details on the practical aspects of related elements would be discussed.

9.Diagnostic Significance of ECG Ergonovine Provocation Test in Patients with Vasospastic Angina.

BACKGROUND: The diagnostic significance of ECG ergonovine test and the incidence of vasospastic angina in patients with chest pain are still uncertain. METHODS: From May 1998 to June 1999, 133 consecutive patients with chest pain were admitted for diagnostic coronary angiography. ECG ergonovine provocation tests were performed in 64 patients who have a clinical history suggesting vasospatic angina, including 27 of 28 patients with vasospastic angina documented electrocardiographically, or who's coronary angiographic findings could not be explained by their clinical history. Ergonovine was administered intravenously in incremental dose of 0.05, 0.1, 0.2, 0.3, 0.4 mg up to total cumulative dosage of 1.0mg during coronary angiography(41 cases), in the exercise laboratory(21 cases) or both(2 cases). RESULTS: Of 133 patients with chest pain, vasospastic angina was documented in 32(24%), unstable angina in 52(34%), stable angina in 10(8%), and others in 39(29%). Angiography demonstrated coronary spasm in 4 additional patients as well as 22 patients with vasospastic angina documented electrocardiographically, but ergonovine tests in the exercise laboratory showed positive response in 5 of 6 patients with vasospastic angina documented. Among the 25 patients with coronary spasm proved angiographically during ergonovine test, ECG findings at the time of coronary spasm were ST segment elevation in 21(84%), depression in 1(4%) and no change in 3(12%) patients, who had branch vessel spasm, 3 vessel spasm or incomplete spasm on coronary angiogram. Of the 31 patients with vasospastic angina who underwent ergonovine tests, positive response occurred in 24(77%) after a cumulative dose of < or = 0.4mg and in additional 3(10%) after the higher dose of >0.4mg. However the other 4(13%) had no ECG changes even after the higher dose(>0.4mg) of ergonovine. CONCLUSION: This results suggest that vasospastic angina appear to be a common cause of chest pain, and ECG ergonovine test with high dose can improve diagnostic sensitivity but cannot detect some patients with vasospastic angina.

10.Patterns of Iron Utilization According to the Growth of Staphylococcus aureus.

To elucidate iron utilization patterns of Staphylococcus aureus according to the growth, we checked the residual iron concentration and the production of siderophores at the indicated times while culturing S. aureus ATCC 6538 and 25923 strains in brain heart infusion broth. By using streptonigrin susceptibility test and investigating growth curves in three culture media of which iron concentration is 0.2, 20, 45 uM, respectively, we found out that iron metabolism of 6538 strain was more active than that of 25923 strain. In point of tendency of iron consumption, 6538 strain steeply consumed iron just before the onset of stationary phase, but 25923 strain did gradually iron throughout the growth phase. Nevertheless, total amount of iron consumed by each strain during the growth was almost no difference between the strains. CAS diffusion assay in detecting siderophores showed that siderophore production followed iron consumption. These results suggest that the siderophores play significant role in iron utilization in vitro.
