1.Result of a Minimum Five-Year Follow-Up of Hip Arthroplasty Using the Bencox(R) Hip Stem.

PURPOSE: The purpose of this study is to report the results of artificial hip arthroplasty with minimum five-year follow-up using the Bencox(R) (Corentec) hip stem, the first total hip prosthesis developed in Korea. MATERIALS AND METHODS: We evaluated 27 hips in patients with femoral neck or intertrochanteric fracture (fracture group) and 58 hips in patients with arthritis or osteonecrosis of the femoral head (arthritis group) who underwent hip arthroplasty using a Bencox(R) hip stem in combination with Bencox(R) bipolar cup and Bencox(R) acetabular cup between September 2006 and February 2008. Patients in the fracture group underwent bipolar hip arthroplasty, and those in the arthritis group underwent total hip arthroplasty. RESULTS: During the follow-up period, there were no cases of revision of the femoral stem. Mean Harris hip score was 94 at the latest follow-up in (femoral neck or intertrochanteric) the fracture group, and improved from 57 preoperatively to 98 at the latest follow-up in the arthritis (or avascular necrosis) group. Radiographically, endosteal bone ongrowth was found in 23 of 27 cases in the fracture group (85.2%) and 56 of 58 cases in the arthritis group (96.6%). Stem loosening, infection, dislocation, and ceramic breakage were not noted. CONCLUSION: Clinical and radiographic evaluations of hip arthroplasty using the Bencox(R) hip stem showed excellent outcomes with a minimum of five-year follow-up.

2.A Case of Melanoacanthoma.

Melanoacanthoma is an extremely rare pigmented benign mixed epithelial tumor of both melanocytes and keratinocytes. A 52-year-old female presented an asymptomatic, slightly elevated, 1. 2 cm x 0. 8 cm, dark black-colored plaque on her right buttock of 2 years' duration. Histopathologic finding of the plaque shows hyperkeratosis, focal parakeratosis, follicular plugging, acanthosih, papillomatosis and pseudo-horn cyst in the epidermis. The tumor is composed of basaloid and squarnous cells with many foci of clusters and whorls of melanin pigment-laden cells. Electron microscopic finding reveals many scattered melanocytes in the entire epidermis. Melanin granules are found mostly in the melanocytes but can also be found in a few keratinocytes.

3.A Case of Mucopolysaccharidosis Type I with Spinal Cord Compression.

A 21-year-old woman with gargoyloid face and short trunkal stature showed progressive quadriparesis. Cervical spine MRI showed circumferential compression of cervical spinal cord by thickened dura mater. Elevated urinary dermatan sulfate and decreased enzyme activity of -iduronidase revealed that she has mild form of mucopolysaccharidosis type I. Her weakness was improved with cervical laminectomy and duraplasty. In this case, progressing quadriparesis resulted from cervical spinal cord compression by thickened dura mater. Earlier surgical decompression could have been better for the patient.

7.Gastrointestinal Fellowship Education in Korea.

Since the residency training program in internal medicine changed from a four-year to a three-year program in 2017 in Korea, issues of an optimal training program for the gastrointestinal (GI) fellowship has arisen. Currently, the evaluation criteria for a GI fellowship in Korea include the following; 1) the total number of assigned patients during the training period, 2) the number of GI endoscopy procedures, 3) attendance at academic conferences, and 4) research presentations. However, competency-based training should be introduced in the GI fellowship training program. The current issues of GI fellowship training in Korea include the following; 1) reorganization of the GI fellowship education system and consideration of an optimal training period following the introduction of the three-year internal medicine residency training program, 2) development of a standardized, competency-based GI fellowship training program, 3) provision of a support program for instructors in GI fellowship education, 4) introduction of a mentor-mentee system, 5) introduction of an accreditation system for GI fellowship, 6) supplementation of a GI sub-specialty qualification system, and 7) provision of benefits to GI sub-specialists.

10.Rahnella Aquatilis Bacteremia in a Hemodialysis Patient.

Rahnella aquatilis, an infrequently isolated gram-negative rod, is the only species of the genus Rahnella within the Enterobacteriaceae family. The organism's natural habitat is water, from which most isolates have been recoverd. Infecions in humans have only occasionally been reported, especially in immunocompromised patients. We reort a case of bacteremia caused by this organism in a hemodialysis patient.
