2.Survey on encysted cercaria of trematodes from fresh-water fishes in Tongjin riverside areas in Korea.

In an attempt to clarify the epidemiological feature of distomiasis in Tongjin riverside area, the prevalence of distomiasis in the residents and infection rates of the metacercariae in fresh-water fishes were investigated at the upper, middle and lower reaches of the river from January to April 1984. The results obtained were summarized as follows: Out of a total of 931 fresh-water fishes which composed of 33 different species, 611 fishes (65.6 percent) of 31 species were found positive with digenetic trematode metacercariae of 16 different species, and there were some differences in infection rates of the metacercariae among the fishes in the3 parts of the river; 53.8 percent in upper, 80.7 percent in middle, and 61.0 percent in lower reaches, respectively. Infection rates of the metacercariae of Exorchis oviformis, Metagonimus yokogawai, Echinochasmus japonicus, Metorchis orientalis and Clonorchis sinensis in the fishes were 48 percent, 29 percent, 11 percent, 7.9 percent and 6.3 percent, respectively. The average number of the encysted larvae of Clonorchis found in fish body/gram showed 4.44 in Pseudorasbora parva, Gnathopogon coreanus (1.2), Microphysogobio yaluensis(0.76), Abbottina springeri(0.4), Acanthorhodeus asmussi (0.21) and Cultriculus eigenmanni (0.17), respectively. The average number of the metacercariae of Metagonimus found in fish body/gram disclosed 34.01 in Zacco platypus, Zacco temmincki (16.46), Carassius carassius (5.35), Moroco oxycephalus (1.54), Aphyocypris chinensis (1.5) and etc., respectively. Detection rates of the eggs of Clonorchis and Metagonimus among residents were 1.1 percent and 0.8 percent, respectively, out of a total 923 persons.

3.Extracapsular Cataract Extraction in a Functioning Bleb.

We retrospectively reviewed 11 eyes of 10 patients, with a mean age of 60.2 years, who had a functioning filtering bleb and underwent an extracapsular cataract extraction with implantation of a intraocular lens. Their mean precperative intraocular pressure was 15.8mmHg and six months postoperatively this pressure was 15.5mmHg. There was no change of medicaiton. Six eyes showed a reduction of intraocular pressure with a mean change of 3.3mmHg, 4 eyes showed a mean rise of intraocular pressure of 3.8mmHg and 1 eye remained unchanged. Transient loss of filtering bleb was observed in 3 eyes but within four weeks reformed. We conclude that the results of a filtering precedure remain unchanged after extracapsular cataract extraction with implantation of intraocular lens.

5.Visual Field Changes in Children with Vigabatrin Treatment.

PURPOSE: Recently, several reports have documented persistent visual field changes associated with long-term use of Vigabatrin, an antiepileptic drug. Epilepsy is a common disease in children, therefore we investigated perimetric changes in children who were on Vigabatrin treatment. METHODS: Total 36 eyes of 18 patients who had been treated with Vigabatrin were tested with Humphrey Field Analyzer. The visual fields were assessed using central 30-2 threshold test and peripheral 60-4 threshold test. RESULTS: In central 30-2 threshold tests, 13 eyes(40.6%) showed mild visual field constrictions and 4 eyes(12.5%) showed severe visual field constrictions. In peripheral 60-4 threshold tests, 23 eyes(71.8%) showed visual field changes. CONCLUSIONS: Asymptomatic visual field loss may develop in Vigabatrin-treated children. Regular examinations of the visual field is warranted for Vigabatrin-teated children.

7.Effects of Basic Hospital Infection Control Methods on the Isolation Rate of Methicillin: Resistant Staphylococcus aureus.

BACKGROUND: Methicillin-resistant Staphylococcus aureus (MRSA) is highly prevalent in hospitals in Korea. The overall rate of MRSA in hospitals in Korea was estimated over 60%. Recently hospital infections caused by MRSA are getting increased and problematic in Korea, Our hospital Seoul Paik Hospital, has been trying to solve this problem. We had not applied exact methods for hospital infection control. Therefore, we adapted strict methods under the control of hospital infection control committee (HICC) and assessed the effectiveness of these methods. METHODS: Every month, the number of MRSA isolates was collected and analyzed; the results of the analysis were reported to every ward. All wards were supplied with Microshild(R) (Johnson & Johnson, Australia), Clean N' Fresh(R) (Carroll, USA), and standard paper towels. Health care workers were regularly educated for hospital infection control. The use of antibiotics was controlled by the HICC. We compared MRSA isolation rate between before and after applying the strict infection control methods under the regulation of HICC. Mann-Whitney U test, one-way ANOVA, and Chi-square test were used for statistical analysis of the results. RESULTS: After restricting the uses of antibiotics, glycopeptides and carbapenems were prescribed 15% less (20.3 vs 17.3 DDD /1000 patient-days) and 35% less (9.3 vs 6.0 DDD /1000 patient-days) respectively, compared to before restricting antibiotics. Methicillin resistance rate was decreased from 78% to 69% compared to before the methods(p=0.02). The isolation rate of MRSA was reduced by 31-42% from 2.33 to 1.35-1.60/1000 patient-days compared to before adapting strict infection control methods (p=0.04-0.07). CONCLUSION: This study showed that applying the strict infection control methods in the hospital can be effective to reduce the isolation rate of MRSA.

8.Persistence of Orientia tsutsugamushi in Humans.

We investigated the persistence of viable Orientia tsutsugamushi in patients who had recovered from scrub typhus. Blood specimens were available from six patients with scrub typhus who were at 1 to 18 months after the onset of the illness. The EDTA-treated blood specimens were inoculated into ECV304 cells, and cultures were maintained for 7 months. Sequencing of the 56-kDa type-specific antigen gene of O. tsutsugamushi was performed to ascertain the homology of isolates. O. tsutsugamushi was isolated from all six patients, and nucleotide sequences of isolates serially collected from each patient were identical in all five patients in whom nucleotide sequences were compared. One patient relapsed 2 days after completion of antibiotic therapy; two patients complained of weakness for 1 to 2.5 months after the illness; one patient underwent coronary angioplasty 6 months later; and one patient suffered from a transient ischemic attack 8 months later. This finding suggests that O. tsutsugamushi causes chronic latent infection, which may be associated with certain clinical illnesses, preceded by scrub typhus. Antibiotic therapy abates the symptoms of scrub typhus, but does not eradicate O. tsutsugamushi from the human body.

A gastric intramural hematoma is very rare and commonly associated with trauma, anticoagulation therapy, coagulopathy, pancreatic disease, aneurysm and peptic ulcer disease. This is a case of gastric intramural hematoma which occurred in a patient taking aspirin after hypertonic saline-epinephrine injection for bleeding from a biopsy site. We describe a case of gastric intramural hematoma that was successfully managed with conservative therapy.

10.Analysis of Upper Eyelid Fat Pad Changes with Aging and Body Mass Index in Korean.

PURPOSE: To evaluate differential changes in the central and nasal fat pads of the upper eyelid associated with aging and the effect of body mass index in Koreans. METHODS: Using a standardized scale, the volume grade of the central and nasal fat pads was graded from 0 to 3 (0 for depressed, 1 for flat, 2 for mildly protruded, and 3 for markedly protruded) in normal adults, consisting of 20 males and 20 females from 30 years of age to 80 years of age. The body mass index was calculated for every patient through interviews using their height and weight. Statistical evaluations correlating age and body mass index with central and nasal fat pad changes were performed. Statistical evaluations correlating age with fat pad changes except for the effect of body mass index were performed. RESULTS: Except for the effect of body mass index, the central fat pad increased (r = -0.176, p = 0.007) and nasal fat pad decreased (r = 0.252, p < 0.001) with aging, which was statistically significant. CONCLUSIONS: With aging, the central fat pad increases and nasal fat pad decreases in the upper eyelid. Clinically, this finding has implications in cosmetic and functional upper eyelid blepharoplasty. In addition, the results may be useful to determine whether to conserve or remove the fat as well as the amount of fat removal during upper blepharoplasty.
