3.Infantile Hemangioendothelioma of the Liver: Brief case report.

Infantile hemangioendothelioma of the liver is a common vascular tumor in infancy. The tumor is usually multinodular or diffuse and classified into two types. We present a case of infantile hemangioendothelioma of the liver, which predominantly consists of type 2. A 4-month-old female was admitted for an evaulation of an abdominal distension. A CT scan of the liver showed a multinodular mass. The right lobectomy was done. Grossly, the mass consisted of round nodules ranging from 2cm to 5cm in diameter. Microscopically, the tumor revealed proliferation of small vascular channels lined by endothelial cells. Bizarre cells and mitotic cells were frequently noted. Vesicular nuclei and multilayering of the endothelial cells were also noted.

4.A Clinical Significance of a Tension-free Hernioplasty with Mesh for Adult Inguinal Hernia Repair.

BACKGROUND: Recently, a tension-free herniorrhaphy method using mesh prosthetics has been per formed increasingly in Korea. However, there are few reports discussing the patch-closure method and providing guidance for its use. Therefore, it is necessary to evaluate the clinical significance of using patch-closure method for adult hernia patients. METHODS: From among 239 adult hernia patients a clinical analysis was done retrospectively on 37 patients who had undergone a tension-free herniorrhaphy. The clinicopathologic features analyzed were age, sex, location, symptom, type of operation, anesthetic method, recurrence rate, use of postoperative analgesics, operating time, complications, and length of hospital stay. The clinical features of the tension- free hernioplasty group and a conventional hernioplasty group were compared. RESULTS: The period of analgesics use was significantly shorter in the tension-free hernioplasty group than in the conventional hernioplasty group (p<0.01, 1.5 days vs 2.8 days, respectively). The operative time was significantly longer in the tension-free hernioplasty group than in the conventional hernioplasty group (p<0.01, 105.8 min vs 71.6 min, respectively). The recurrence rate after a conventional hernio plasty was 2.3% (4/117), but there was no recurrence after a tension-free hernioplasty. CONCLUSION: A tension-free hernioplasty with mesh is a pain-relief operation and has tendency for lower recurrence. It is suggested that a tension-free hernioplasty with mesh should be performed for old-aged people and recurrent cases.

5.Nutritional Consequences and Management After Gastrectomy.

Gastric cancer is the most common cancer and the third most common cause of cancer deaths in Korea. Gastric resection, especially for the early stages of the disease, results in an excellent survival rate, and has been the mainstay of treatment for gastric cancer patients. Due to increasing use of surveillance endoscopy, the diagnosis of gastric cancer at early stages has increased. The 5-year survival rate for early gastric cancer has now improved to better than 90%, and consequently, the population of long-term survivors after gastrectomy has also increased. Therefore, the quality of life including nutritional support has become an important concern for gastrectomized patients during long-term follow-up. Nutritional capacities after gastrectomy should be evaluated by nutritional assessment and absorption tests. Nutritional deficits are more serious after total gastrectomy than after subtotal gastrectomy. Fat malabsorption has been shown to be a significant concern in patients that have undergone total gastrectomy. Other suggested causes of malnutrition include poor oral intake, relative pancreatic insufficiency, bacterial overgrowth, and shortened intestinal transit time. Food residue and bile reflux are frequently observed in the remnant stomach during surveillance endoscopy after a distal subtotal gastrectomy due to gastric cancer. The bile reflux is often associated with remnant gastritis or esophagitis and has an influence on the quality of life following a distal subtotal gastrectomy. Reconstruction methods have not influenced the food retention phenomenon after a distal gastrectomy over long-term periods. In addition, nutritional status after gastrectomy is significantly associated with postoperative complications. In conclusion, the provision of dietary education and nutritional support is highly recommended in postoperative patients for gastric cancer.

6.Increased Serum Level of Inhibin in Oligo-amenorrheic Women with Polycystic Ovaries.

Normal and abnormal follicular growth and steroidogenesis depend on gonadotropins as well as intraovarian peptides, which may mediate or potentiate gonadotropin action. Inhibin also affect follicular development and steroidogenesis and may play a role in dominant follicle selection and follicular atresia. Therefore, we studied the differences of serum inhibin, gonadotropin and androgen levels in the women with only the ultrasound findings and no disorder, and polycystic ovary (PCO) with ovulatory disturbance. We prospectively analysed forty-three women with PCO. The diagnosis of PCO was based on typical appearance of the ovaries on TVS. Twelve women with regular menstrual cycle and normal ovarian morphology were selected as control. Basal levels of inhibin, luteinizing hormone(LH), follicle stimulating hormone(FSH), estradiol(E2), testosterone(T), androstenedione(ADD), dehydroepiandrosterone-sulfate(DS), prolactin and TSH in serum were determined. There were significant differences in basal LH levels and LH/FSH ratio between the control and the women with PCO. The basal levels of inhibin and E2 in the oligo-amenorrheic PCO (N=34) were significantly higher than those in the control. There was higher negative correlation between the inhibin and T levels in the oligo-amenorrheic PCO, but, not in the regular cycling PCO. Also, there was higher positive correlation between the LH and T levels in the oligo-amenorrheic PCO, but not in the regular cycling PCO. These data presume that the initial event of PCO is elevated pituitary LH secretion. Elevated levels of LH may down-regulate LH receptors on granulosa cells and also cause hypertrophy of the thecal layer. High level of androgen secreted by the hypertrophied thermal layer may stimulate inhibin secretion from granulosa cells and can be converted to estrogen by extraovarian tissues and could serve to augment pituitary sensitivity to GnRH with a resultant secretion of more LH follicular development and dominant follicle selection resulted in ovulatory disturbance.

8.Inhibitory Effect of Luteolin Liposome Solution by Animal Model for Atopic Dermatitis in NC/Nga Mice.

Atopic dermatitis is a common chronic inflammatory skin disease, associated with marked inflammatory cells (of mast cells and eosinophils) and severe itching, which leads to clinical problems in the pediatric population. This study was designed to investigate the inhibitory effects of luteolin liposome solution, that is entrapped the hydrophobic luteolin (one of the flavonoids) into ethosome to improve its stability, by using hapten-induced atopic dermatitis animal model (NC/Nga mice).The luteolin liposome treated mice showed anti-inflammatory effect as evidenced by the lowering of erythema and edema in clinical observation, reduction of inflammatory cell infiltration and epidermal thickness in histopathological examination, when compared with TNCB induced controls. Luteolin liposome solution also reduced the serum IgE level which played important roles in the atopic dermatitis model. These results suggest that luteolin liposome solution has some merit in this formulation showing inhibitory effects for the atopic dermatitis.

9.Effect of Adjuvant Immunochemotherapy on Immunologic Function of Gastric Cancer Patients.

BACKGROUND: We have postoperatively performed adjuvant immunochemotherapy (Picibanil plus oral -fluorouracil) for advanced gastric cancer patients. We aimed to study the effect of immunochemotherapy on the immunologic function after gastric resection for advanced gastric cancer patients. METHODS: Twenty gastric cancer patients were randomly selected at the outpatient department from May 1994 to September 1994. All patients had undergone a total gastrectomy with a D2 lymph node dissection (at least) and had also undergone adjuvant immunochemotherapy for two years. No patients had any evidence of a local recurrence or a distant metastasis at the time of the study. The immunologic parameters were the total lymphocyte count and the percentages of T3 cells (CD3), T4 cells (CD4) and T8 cells (CD8), which were determined using monoclonal antibodies and flow cytometry. A delayed skin hypersensitivity reaction was induced with dinitrochlorobenzene (DNCB). These parameters before operation were compared with those after immunochemotherapy. RESULTS: The average total lymphocyte count before the operation showed no significant difference from that after immunochemotherapy (2,138+127.9/ml vs. 2,235.7+140.0/ml, respectively). The average proportions of T3, T4, and T8 cells among the lymphocytes also showed no significant difference (59.0+2.2%, 36.9+1.5%, and 25.8+1.5%, preoperatively, vs. 61.3+3.2%, 39.7+2.7%, and 23.4+1.8%, after immunochemotherapy, respectively). The distribution of the delayed skin hypersensitivity reactions of the patients was not significantly different from that of the normal control group. CONCLUSIONS: The postoperative immunological function was not suppressed by immunochemotherapy. This suggests that a long-term immunotherapy combined with chemotherapy might play a role in supporting immunologic functions after a gastric resection.
