1.A Case of Primary Intracranial Malignant Melanoma.

The primary malignant melanoma arising in the leptomeninges of the central nervous system is extremely rare. We report a case of malignant melanoma, apparently thought to be originated from leptomeninges, with diffuse neoplastic process widely spreaded over the leptomeninges of 42 years old man, who had suffered severe headache, nausea and left hemiparesis. This patient is free of the symptoms for six months after operation. The literature on this subject is briefly reviewed.

3.A Case of Retinal Periphlebitis Associated With Barre Syndrome.

Authors' presented an interesting case, 19 years old boy, who was been suffered from retinal periphlebitis of both eyes which thought to be induced by Barre Syndrome. This patient also has pulmonary tuberculosis and visual symptoms of Barre Syndrome in this case were ocular pain and visual disturbance that appeared and disappeared suddenly due probably to irritation of the ventral nerve roots (C5 to T1) with also irritation of sympathetic fibers contained in them.

5.Arthroscopic Synovectomy in the Rheumatoid Arthritis of the Knee Joint.

The short term beneficial effect of arthroscopic excision of synovial tissue in knees with rheumatoid arthritis have been well documented. The purpose of this study to report the results of synovectomy in patients with rheumatoid arthritis who have been observed at least 3 years. We analysed the clinical result with modified Laurin criteria, radiologic change and patients own satisfaction degree with survey. The results were as follows; 1. The preoperative symptoms were pain (100%), swelling (100%), decreased range of motion (83%) and joint tenderness (74%), and those were improved postoperatively to 43%, 60%, 11%, 43% each other. 2. Among the laboratory parameters, C-reactive protein (CRP) was improved most significantly (pvalue =0.026). 3. On simple supine A-P X-rays, the width of medial joint space and lateral joint space were compared to preoperatively and postoperatively. The width of medial joint space was 4.33mm preoperatively, and it was changed to 3.22mm at last follow up (p=0.032). Those of lateral joint space was changed from 4.24mm preoperatively to 3.27mm at last tollow up (p=0.106). 4. The grade of articular cartilage damage was related with severity of preoperative symptoms and the patients with low grade of articular cartilage damage showed more postoperative improvement. The result of operation was related to symptom duration, degree of X-ray change and degree of cartilage damage. 5. 58% ot patients were satisfied to results of synovectomy, 28% were answered neither satisfied nor ansatisfied and 14% were unsatisf'ied to operation. We concluded the arthroscopic synovectomy of the knee joints was effective procedure to improve the clinical symptoms, range of knee motion and to alleviate the inflammatory reaction at mid-term period al'ter the operation. So we consider this procedure is a palliative treatment that modified and alleviate the progression of rheumatoid arthritis of knee joints.

Twin reversed arterial perfusion (TRAP) sequence is a rare specific anomaly of twin gestation with fused placenta and umbilical anastomosis. This syndrome occurs once in about 34,600 births and reported first by Beneditti in 1533. We report on prenatal diagnosis of a case of TRAP sequence with color Doppler sonography, this case is presented with a brief review of the literature.

7.Malignant Granular Cell Tumor of the Shoulder: A case report.

A malignant granular cell tumor (MGCT) occurred in the left shoulder of a 62-year-old man. The patient underwent wide marginal excision followed by chemotherapy and radiotherapy. A metastatic tumor was identified in the axillary lymph node 22 months after the excision of the shoulder mass. The primary tumor was a poorly circumscribed mass measuring 5 5 4 cm. On cut section, it was a solid mass with yellowish tan color. Histologically, both primary and metastatic tumor consisted of polygonal cells with abundant granular cytoplasm and a vesicular nucleus with a prominent nucleolus. Two to three mitotic figures per ten high power fields at 200 were counted. Tumor cells were weakly stained with periodic acid-Schiff (PAS) preparation both before and after diastase digestion, and were positive for S-100 protein, neuron-specific enolase (NSE), and vimentin. By electron microscopy, the cytoplasm was filled with numerous autophagolysosomes containing myelin figures, mitochondria, and fragmented rough endoplasmic reticula. Basal laminae and angulated bodies were also noted. These findings suggest schwannian differentiation of this tumor.

STATEMENT OF PROBLEM: The researches on the influence of design variables on the stress distribution in cortical and trabecular bones and on optimal design for implant system were limited. PURPOSE: The purpose of this study is to identify the sensitivities of design parameters and to suggest the optimal parameters for designing the onebody type implant system. MATERIAL AND METHODS: Stresses arising in the implant system were obtained by finite element analysis using a three dimensional model. An onebody type implant system [Oneplant (Warrantec. Co. Ltd., Korea)]was considered in this study. Vertical load(150 N) was applied on the top of the abutment along the axial direction. The initial design variables set for sensitivity analysis were radius of fixture, numbers of micro thread, numbers of power thread, height of micro thread, fixture length, tapered angle of fixture, inclined angle of thread, width of micro thread and width of power thread. The statistical technique of Design of Experiments(DOE) was applied to the simulation model to deduce effective design parameters on stress distributions in bones. The deduced design parameters were incorporated into a fully automated design tool which is coupled with the finite element analysis and numerical optimization to determine the optimal design parameters. RESULTS: 1. The result of sensitivity analysis showed six design variables - radius of fixture, tapered angle of fixture, inclined angle of thread, numbers of power thread, numbers of micro thread and height of micro thread - were more influential than the others. 2. The optimal values of design variables can be deduced by coupling finite element analysis (FEA) and design optimization tool(DOT).
