1.Clinical application of absolute ethanol as an embolizing material

Transcatheter infusion of absolute ethanol was applied clinically in 3 cases of artificial embolization; 25yrsold female with bilateral renal angiomyolipoma, 19yrs old female with right paralumbar liposarcoma and 25 yrs oldmale with hypernephroma of right kidney. Selective or subselective manual infusion was made and 9 to 22cc of pureethanol was delivered in a speed of 1 to 2cc per second. The sclerosing effect of absolute ethanol was potent.However, all 3 cases revealed mild post-emboliaztion syndrome.

2.Hazards Exposed to Firefighters in Fire: Physical, Chemical, and Biologic factors.

According to National Emergency Management Agency in Korea, numbers of death and injured in firefighters were 34 and 1,555 persons, respectively, between 2003 and 2007. Firefighting is a very dangerous and difficult work because of physical, chemical, and biological hazards. Firstly, physical hazards include burn induced by smoke and other combustion products, heat, noise, and ergonomic factors. Secondly, chemical hazards include chemical asphyxiants such as carbon monoxide, hydrogen cyanide, and hydrogen sulfide, simple asphyxiants such as carbon dioxide, chemical irritants including as hydrogen chloride, and acrolein, and so on, and other carcinogens. Finally, biological hazards include infectious agents such as hepatitis, tuberculosis, and so forth. We expect this study to help management andpromotion of firefighters' health.

4.Clinical Study on Tuberculous Meningitis (Correlation with brain CT findings).

We tried to correlate brain CT findings with clinical state at admission and outcome at discharge in 42 selected cases from 94 adult patients under the diagnosis of tuberculous meningitis at Seoul National University Hospital during last four years from 1981. Their clinical state at admission and outcome at discharge were classified into three groups by severity of symptoms, respectively. The final outcome of them were well correlated with their clinical states at admission. Observed abnormal brain CT findings in this series were hydrocephalus (74%), dirthy cisternal enhancement (52%), infraction (38%), periventricular low density (36%), and tuberculoma (19%). The poorer the clinical state at admission and outcome at discharge, the more frequent the abnormal brain CT findings, especially of periventricular low density and infraction. But periventricular low density without infarction seemed to affect more deleterious effect on clinical state at admission than on final outcome.

9.Thyroid Acropachy.

Thyroid acropachy is a rare complication of Graves' disease, manifested by clubbing of the terminal phalanges, periosteal new bone formation and overlying soft tissue swelling, It may occur when the patient is hypothyroid, euthyroid or hyperthyroid. In most cases, it is a part of the syndrome, including exophthalmos and/or pretibial myxedema. The authors have experienced one case of thyroid acropachy and report with a review of the literature review. The patient a 56-year-old female with a characteristic feathery new bone formation on the medial side of the shaft of the left first metatarsal bone and overlying soft tissue swelling. However, there was no pretibial myxedema and clubbing of fingers. She was hypothyroid and treated with systemic corticosteroid for mild pain and persistent swelling. The treatment had temporarily improved the patient's condition.

10.An experimental study on renal infarction with ethanol

Renal infarction with ethanol was induced exprimentally in rabbits and selective renal angiography wasperformed to evaluate the effectiveness of ethanol as embolic material. The results were as follows; 1. Completeobstruction of renal artery was produced in all cases within 1 week after injection of absolute ethanol(0.5ml/kg). 2. Incomplete obstruction of renal artery was produced in majority after injection of absolute ethanol(0.2ml/kg) and changed to complete obstruction above half cases with time. 3. Incomplete obstruction of renalartery was produced in minority after injection of 60% ethanol (0.2ml/kg) and complete obstruction of renal arterywas not produced. It was considered that ethanol is an effective agent for complete renal infarction and 0.2 to0,5ml/kg of absolute ethanol is effective dose for complete renal infarction.
