2.Endoscopic Retrograde Cholangiopancreatography Using Barium Sulfate As a Contrast Material.

An occasional patient has both a pressing need for visualization of the biliary system in spite of a past history of reaction to iodinated contrast media. We report a patient, who had reacted adversely to contrast media, underwent ERCP using barium sulfate in order to opacify the biliary and pancreatic duct without side effect successfully. In conclusion, ERCP using barium sulfate as a contrast material is of value in patients who are intolerant of the iodinated contrast media.

3.A Case of Multiple Intraosseous Lipomas

Fatty tumours are,the most common soft tissue tumours whereas they are among the rarest tumours in bone. Dahlin quoted an incidence of 1 in 1, 000 of all bone tumours, and fatty tumours in bone are almost single intraosseous lipoma and of four types; I. Soft tissue lipomata or liposarcomata, secondarily affecting bone. 2. Parosteal lipomata. 3. Intraosseous lipomata. 4. Liposarcoma of bone. Here we report a case of multiple intraosseous lipomas in a 23 year old woman.

4.Clinical Study of the Fracture of the Carpal Scaphoid

Ten cases of fracture of the carpal scaphoid analysed clinically and treated by admission at Department of Orthopedic Surgery, National Police Hospital during the period of 1975 to 1980. The results were summerized as follows: 1. The mode of the injury were consist of 3 in football game, 2 in judo, 2 in army training and 1 in fallen from height, 1 in slipping down, 1 in direct blow and almost falling down by outstretched hand. 2. Of the 10 cases, there were 9 male and 1 female patients. This result showed marked predominence of male over female patients and most commonly occured between 20 and 29 of age. 3. In 8 cases, the fracture was found on the waist of the carpal scaphoid. 4. Nonunion of the carpal scaphoid were noticed in 8 cases. 5. 2 cases of the recent fracture, there were appeared by rechecking X.-ray after 2 weeks. 6. The good results were obtained by bone graft according to method of Russe.

5.Experience with the Ipsilateral Thigh Flap for Closure of Heel Defects in Children

Soft tissue coverage of heel defects has long been a difficult problem. In 1982, Iron reported eight cases of heel defects in children using an ipsilateral posterior thigh flap with use of the Hoff-man's skeletal fixation apparatus for immobilization. We report three cases of heel defects treatment in children using ipsilateral posterior thigh flap and detach flap in an average 13 days with aid of single photon emission computed tomography(SPECT) for evaluation of circulation status of flap.

7.Cellular Schwannoma Arising in a Facial Nerve.

Cellular schwaninoma is a variant of schwannoma, which is characterized by predominance of cellular Antoni A area, presence of mitotic activity, nuclear hyperchromasia, pleomorphism, and absence of Verocay body. These pathologic features often prompted a misdiagnosis of malignancy. However, the clinical outcome has indicated the benignity of the tumor. We have experienced a case of cellular schwannoma arising from right facial nerve with right hemifacial weakness and erosion of mastoid process. Grossly, it was a 3.5 x 3 cm sized and relatively well encapsulated mass with yellowish, friable cut surface. Microscopically, cellular growth with moderate cellular pleomorphism and some mitotic activity (5/40 HPFS, up to 2/HPF) were noted. Immunostaining for S-100 protein showed diffuse strong positive reaction.

9.A Case of Generalized Cranuloma Annulare Treated with Dapsone.

Generalized granuloma annulare is manifested by numerous, symrnetrical, shiny, normal-skin colored, discrete or confluent, papular eruption on the sunexposed areas, mostly in females of past middle age. Up to date, a variety of therapeutic approaches have been tried with inconsistent results. Recently several agents have been reported to be inconsistent results. Recently several agents have reported to be effective in the treatment of genealizecl granuloma annulare. Of these, dapsone has been used with favorable respones. The fact dapsone can inhibit the lysosomal enzyme release and the mononuclear cell-rnediated rnyeloperoxidase cytotaxicity would account for its effect in granuloma annulare even in part. We observed a case of generalized granulare in a 49-year old man, who had had numerous normal-skin colored or faintly violaceous, flattopped papular eruption on the sun-exposed areas for 3 years. He was treated successfully with dapsone 100mg a day for about 22 months.

10.A Case of Transepidermal Elimination in Gouty Tophus.

We herein report a case of gout, who was a 45-year-old man and had had multiple various-sized papules or tophi on the helices of ears, hands, elbows, knees, ankles and feet for 10 years. Histopathologic findings of the large tophus of right knee revealed the typical pattern of gout with needle-shaped urate crystal. The phenomenon of iransepidermal elimination was observed in serial sections of histologic speeirnerls, The nsrrow transepidermal channel, lined by pseudoepitheliomstous lips bilaterally, was Eilled with the cellular debris and urate crystal fragments and cornmunicated direotly with the upper dermis. Presumably, transepidermal elirnination in gouty tophus may be the phenomenon whieh could be found rnore commonly than the scarcity of literature.
