1.A Case of Epithelial Downgrowth as a Complication of Penetrating Keratoplasty.

Authors have experienced a case (aged 24, female) of epithelial downgrowth following partial penetrating keratoplasty. Clinically, this patient was received 7mm penetrating keratoplasty due to herpetic corneal Scar of right eye. After keratoplasty, iris prolapse through the wound dehiscence was occurred and then it was closed immediately with resection of prolapsed iris tissue. But, thereafter, the graft showed haziness which is suspected as a immune reaction and eventually the graft became irreversible opaque. A cosmetic epikeratoprosthesis waS attached on the opaque eye. But intractable secondary glaucoma was not controlled by medications. Finally the illed eye was enucleated and was confirmed histopathologically the main cause of graft failure which was presumed due to epithelial downgrowth into the anterior chamber.

4.Endoscopic retrograde cholangiographic findings in choledocholithiasis

Although ultrasonography replaced many invasive studies in biliary tract diseases, direct cholangiography does still play an important role in the diagnosis and management of choledocholithiasis. Endoscopoic retrograde cholangiography (ERC) is regarded as the best method in evaluation of exact extent of the disease and its frequent complication, cholangitis. Authors analysed 56 cases of choledocolithiasis diagnosed by ERC and compared these with ERC in 18 cases of normal, 22 cases of cholecystitis, 15 cases of clonorchiasis and 9 cases of parenchymal diseases of liver. The results are as follows; 1. ERC findings of choledocholithiasis are filling defects by stoneor stones, dilation of common hepatic as well as common bile ducts and findings of cholangitis. 2. ERC findings of cholangitis are dilatation of larger intrahepatic biliary radicles and acute peripheral tapering, decrease of arborization, increased or right angle branching pattern, straightening and rigidity as well as irregular narrowing of intrahepatic biliary trees. This findings are observed in majority of choledocholithiasis. 3. Over9mm in diameter at intraprancreatic portion of common bile duct was regarded as abnormal, with 95% sensitivity,85% specificity and 91% diagnostic accuracy by decision matrix analysis. 4. In the presence of dilatation of CBD and findings of cholangitis in ERC, one should consider choledocholithiasis in spite of absence of stone defect.

5.Evaluation of VITROS(R) 5,1 FS Automated Chemistry Analyzer.

BACKGROUND: VITROS(R) 5,1 FS (Ortho-Clinical Diagnostics, NY, USA) is a newly developed, fully automated analyzer that uses the dry slide type technology to process routine chemistry items, electrolytes, therapeutic drug monitoring and immunity items. We evaluated the usefulness of the VITROS(R) 5,1 FS in the aspect of precision, linearity, recovery rates and comparison. METHODS: We evaluated the analytical performance of the VITROS(R) 5,1 FS for sodium, potassium, chloride, total CO2, BUN, creatinine, glucose, amylase, total protein, albumin, ALT, AST, ALP, GGT, total bilirubin, unconjugated bilirubin, calcium and phosphorus. Commercialized quality control material, Performance Verifiers (Ortho-Clinical Diagnostics, NY, USA), chemTRAK(R).H (Medical Analysis Systems, California, USA) and patient' sera for evaluation of precision, linearity, recovery rates and comparison were used. MODULAR Systems (Roche Diagnostics, Switzerland) and CX3 (Beckman Coulter, California, USA) were used as a comparative analyzer to evaluate comparison study. RESULTS: Within-run and within-day coefficients of variation (CVs) of all items were below 5%. Between-day and total CVs were less than 5% in almost all items except of total CO2, amylase, ALT, AST, total bilirubin, unconjugated bilirubin and phosphorus. The linearities were statistically acceptable (R2>0.99) for all items (P<0.001). The recovery rates for all items were in the range of 96.20 to 111.45%. The comparison study indicated good correlation between VITROS(R) 5,1 FS and MODULAR Systems or CX3, and correlation coefficients of all items were above 0.975 except sodium (0.971) (P<0.01). CONCLUSIONS: VITROS(R) 5,1 FS showed satisfactory precision, linearity, recovery rates and comparison. Because it has features of one system with many capabilities, we recommend that VITROS(R) 5,1 FS would be used for the emergency laboratories in general hospitals with many examinations or in medium-sized laboratories with relatively small numbers of cases.

6.The relationship between spontaneous diuresis and changes of pulmonary function in infants with respiratory distress syndrome.

To evaluate the relationship between spontancous diuresis and improvement of pulmonary function in respiratory distress syndrome, 15 premature neonates requiring mechanical ventilation for RDS, who admitted in NICU of Wonkwang University Hospital from January 1990 to June 1992, were studied. The results were as follows (results are mean+/-SEM) 1) Mean birth weight of study infants was 1.82+/-0.08Kg, and mean gestational age was 32.80+/-0.54 weeks. 2) The onset of diuresis occured at a mean age of 29.1+/-2.48 hours with maximum diuresis at a mean age of 41.9+/-1.8 hours 3) Mean urine output increased from 2.25+/-0.43ml/Kg/hr at the stage of pre 12 hours-onset of diuresis (p<0.001), then decreased 3.5+/-0.2ml/Kg/hr at the stage of after maximum diuresis (p<0.05). 4) After maximum diuresis, inspired oxygen gradient, peak inflating pressure, peak end expiratory pressure, mean airway pressure, and respiratory rate were decreased significantly (p<0.05), and alveolar-arterial oxygen gradient was decreased more significantly (p<0,01). So, We conclude that the pulmonary function was rapidly improving after maximum diuresis and these changes reflect the general principle used in ventilatory management of infants with RDS.

8.A Model of Education Program for Childhood Asthma.

PURPOSE: The size of the burden placed on all concerned, the chronicity of the disorder, and the importance of patient compliance in optimal management make asthma an excellent target for the development of patient education programs. Despite the usefulness and the need of the asthma education programs, there is no asthma education programs developed in Korea. In order to develop the education programs for childhood asthma, we offer a model for educating parents, as the first step. METHODS: Most1y, adult education programs consist of 3 components-planning, implication, evaluation. At first, we are going to p1an the asthma education programs for parents of the patients. 2O Volunteers in mothers will be gathered in one c1ass. The curiculum is made of four 1-hour sessions. During the each session, mothers wi1l be taught about the asthma, discuss the shared experiences and practice the skins. RESULTS: This model consists of 4 sessions such as the definition of asthma, the treatment-medicines and correct use of inhalation devices, prevention and the management of acute episodes. CONCLUSIONS: Use of this education model wi1l achieve better control of childhood asthma by leading the parents and patients to participate actively in their asthma treatment. The effects of this program will be assessed by a formal objective evaluation using a pretest, posttest experimental research design.
