1.Effect of Radioactive Isotope 131I on Epiphyseal Plate of Tibia in Mice

The purpose of this investigation was to study the changes in the epiphyseal plate of growing mice by histological, histochemical and electron microscopic observation through the administration of radioactive isotope I which induced hypothyroidism. To confirm hypothyroidism induced, radioimmunoassay for triiodothyronine was carried out and the morphologic changes of thyroid gland were observed by a light microscope. Of the total of eighty ICR strain of mice, fifty received single intraperitoneal injection of 20ue/gm of radioactive isotope I which were dissolved in 0.2ml of saline. Thirty received single intraperitoneal injection of only 0.2ml of saline and these mice were used as control groups. At the end of first, second, third, fourth, sixth and eighth week after injection, both saline injected and I treated mice were sacrifised. The thyroid gland and epiphyseal plate being removed were stained with hematoxylin and eosin, alcian blue and with uranyl acetate lead citrate in each for histological, histochemical and electron microscopic studies, The results obtained are as follows: 1. The thickness of epiphyseal plate was narrowed in the I treated animals. In each zone of epiphyseal plate the cell size was reduced although the cell numbers of each cell column were not changed. This means that the reduction of thickness in the epiphyseal plate was due mainly to decreased cell size and scanty territorial matrix not to decreased cell number. 2. Alcianophility was markedly decreased ln proliferative zone, indicating that acid mucopolysaccharide diminished markedly or was absent in the proliferative zone. 3. In electron microscopic examination, the number of rough surfaced endoplasmic reticulum was decreased and their cisternae were narrowed. Decreasing tendency of dilatation and sacculation of cisternae, .reduced number of attached ribosomes on the endoplasmic reticulum, and atrophy of the Golgi apparatus with diminished number of its vesicles were noted, However glycogen particlea were accumulated. 4. The number and length of each trabeculae in the metaphysis just adjacent to the epiphyseal cartilage plate were reduced markedly, and occasionally chondrocytes which were not consumed by endochondral ossification were observed in the cartilage lattice.

2.Macrophage Activation after In vitro Stimulation with the TSP Antigen of Mycobacterium tuberculosis H37Rv.

Present study aimed to investigate the immunological activities of cell wall associated protein antigen solubilized with Triton X-100 (TSP) from Mycobacterium tuberculosis H37Rv and conducted on 43 patients with pulmonary tuberculosis (newly diagnosed, medicated within 12 months and chronic refractory patients) and 17 normal healthy controls. These immunological responses were compared with those induced by the PPD or 30 kDa antigen from M, tuberculosis H37Rv culture filtrates, identified as biologically important secreted proteins. Proliferative responses to mycobacterial antigens were compared in peripheral blood mononuclear cells (PBMC) of healthy subjects and pulmonary tuberculosis patients. Signiticant blastogenic responses to the TSP were observed in healthy tuberculin reactors, newly diagnosed and some of antituberculosis drug-medicated patients by H-thymidine incorporation assay. IL-12 p40 and IFN-r mRNA expressions to the TSP were markedly increased, whereas IL-10 and TNF-a mRNA expressions were decreased at a 5 day-stimulation by PBMC in healthy tuberculin reactors, newly diagnosed and medicated patients. However, patients with chronic refractory tuberculosis exhibited more depressed IL-12 p40 and IFN-r mRNA expressions to all of the antigens than another groups. Interestingly, very low IL-10 and TNF-a mRNA expressions cultured with the TSP were also shown. These data suggest that the TSP may be involved in the macrophage activation by induction of Th1 stimulatory signals, such as IL-12, and suppression of Th1 inhibitory cytokine, IL-10.

3.Blood Concentration of Lidocaine after Subarachnoid Administration.

Ten healthy adult female patients scheduled for elective abdominal hysterectomy were evaluated for their rates and degrees of absorption following subarachnoid administration of 150 mg of 5% lidocaine in 5% dextrose solution. The mean blood concentrations of lidocaine measured at 5, 15, 30, 60 and 90 minutes after subarachnoid administration were 0.34+/-0.23, 0.78+/-0.27, 1.10+/-0.26, 1.24+/-0.24 and 1.07+/-0.19 mcg/ ml, respectively. The mean values of Cmax and Tmax obtained from these measurements were 1.30+/- 0.21 mcg/ml and 57+/-17 minutes, respectively. These results revealed that in comparison with other regional administration, absorption of lidocaine follwing subarachnoid injection is slower.

4.Hemodynamic Responses and Oxygen Availability in Unanesthetized Dogs during Apnea.

BACKGROUND: This study examined hemodynamic variables, oxygen delivery, extraction, and consumption in response to acute progressive hypoxia and hypercarbia in the setting of apnea. METHODS: Apnea was induced in 9 healthy mongrel dogs by disconnecting animals from mechanical ventilation of 30 minutes with pure oxygen. Hemodynamic variables, oxygen transport, extraction, and consumption were rapidly and repeatedly measured using pulmonary arterial and arterial catheters until cardiac output was undetectable. RESULTS: The baseline PaO2, PaCO2, pH, base excess were 318 +/- 137 mm Hg, 36 +/- 3.5 mm Hg, 7.30 +/- 0.06, 6.81 +/- 2.65 mmol/l respectively. Hypercarbia and hypoxemia (76 +/- 33 mm Hg) was first noted at 1 and 4 minute respectively. Base excess was not changed. Indices of preload (PCWP and CVP) were increased early in the time course (p<0.05). In contrast, indices of afterload (SVR) increased later, just before cardiac decompensation began (p<0.05). No significant reduction of cardiac output, oxygen delivery, extraction, and consumption was detected just until abrupt cardiac decompensation started, 5 minute. CONCLUSIONS: These data suggest that the early increase in preload was primarily due to hypercarbia whereas the late increase in afterload was due to hypoxemia, but the main cause of acute cardiac decompensation was a critical decrease in arterial oxygen tension with some contribution of increased preload and afterload.

5.Clinical Results of Radiofrequency Dorsal Root Entry Zone Coagulation for Paraplegic Pain.

OBJECTIVE: Dorsal root entry zone(DREZ) operation has been the most common surgical treatment for paraplegic pain in the past, but the results differ according to the patients. In this study, we attempt to analyze the results from the patients by the different patterns of the pain before the surgery. METHODS: A total of twelve paraplegic patients have undergone a total of thirteen radiofrequency DREZ coagulation procedures between April 1994, and March 2003. The patients were divided depending on patients' subjective description on the character, frequency, and range of pain. RESULTS: The patients were divided into the two groups, postoperatively: improvement of more than 75% of pain was defined as treatment success, and any level less than 75% of improvement was defined as treatment failure. Eight of the ten mechanical pain group cases were regarded as treatment success, and remaining two cases with thermal type of pain as treatment failure. Also, one case with combined pain failed to obtain any favorable outcome. Intermittent pain group (6 of 7 cases), continuous pain group (2 of 6 cases), and localized pain group (7 of 11 cases) showed treatment success, respectively. However, diffuse pain group (1 of 2 cases) resulted in poor outcome. CONCLUSION: Radiofrequency DREZ coagulation is more effective in managing intermittent and/or mechanical pain than continuous and/or thermal pain in the paraplegic patients following spinal cord injury.

6.Study on Korean Daily Walking Activity <1>.

Since there is no report of daily walking activity of Korean, authors studied on normal adult daliy walking activity in korean of various kinds of occupation, personal life and patients. Total 134 cases were studied and in which there were 10 patients, 7 retired elderlies, 4 house wives, 48 employees of compaines, 30 nurses, 4 laboratory technicians, 2 pharmacists, 7 house staff doctors, 12 chief doctors, 4 cases of hicking and 6 cases of 18 holes golf players. The daily walking steps were measured with pedometer in 167 times in total and following results were, obtained. 1. Bed rest patients ; less than 1000 steps/day 2. 7.Infantile Hypertrophic Pyloric Stenosis Treated with Intravenous Atropine Sulfate.

PURPOSE: The pharmacologic effect of atropine on HPS can be considered to control pyloric muscle spasm. Therefore, we studied the effects of intravenous atropine sulfate on the clinical course of HPS, and periodically observed the ultrasonographic appearance of the pyloric muscles after atropine treatment. METHODS:From April 1998 to May 1999, 14 infants who were diagnosed with HPS were treated with intravenous atropine sulfate. Intravenous atropine sulfate was administered at an initial dose of 0.04mg/kg/day, which was divided into 8 equal doses. The daily dose was increased by 0.01 mg/kg/day until vomiting was controlled for an entire day while infants received unrestricted oral feeding. Ultrasonographic examinations were performed during hospitalization and repeated at least every 2 months until normalization of pyloric muscles was confirmed. RESULTS: Intravenous atropine was effective in 12 of 14 infants with HPS and the conditions of 9 of them improved. Two infants who were not free from vomiting despite a week of intravenous atropine sulfate treatment underwent pyloromyotomy. A series of ultrasonographic examinations were done after vomiting had improved with intravenous atropine sulfate. The ultrasonographic findings showed good passage of gastric contents through pyloric canals despite thickening of the pyloric muscles. CONCLUSION: Intravenous administration of atropine sulfate is an effective therapy for HPS and can be an alternative to pyloromyotomy. (J Korean Pediatr Soc 2000;43:763-768)

9.Changes in Psedocholinesterase Activity Following IV Bolus Administration of Succinylcholine .

Plasma cholinesterase was assayed during the period immediately following IV bolus injection of succinylcholine 1mg/kg to test the effect of succinylcholine on pseudocholinesterase activity. Twenty healthy adult patients scheduled for elective surgery were studied. The resutls were as follows: The mean value of pre-injection pseudocholinesterase activity was 1124.15 IU/L, and the activity following succinylcholin injection was 1159.55IU/L during fasciculation, 982.70 at 1 min, 936.60 at 3 min, 891.25 at 5 min, 926.80 at 7 min, 1015.45 at 10 min, and 1007.70 at 15 min. It was concluded that the tendency to increase pseuducholinesterase activity during fasciculation seems to be due to choline, the metabolite of succinylcholine, however the cause of the significant decrease in pseudocholinesterase activity after fasciculation is uncertain. The only suggested mechanism is due to the inhibition of pseudocholinesterase by succinylcholine and its metabolites.

10.A Negative Pressure Method Using a CVP Manometer for the Ascertainment of the Epidural Space .

To identify the epidural space various methods have been recommended and the methods are divided into two major categories, either loss of resistance or negative pressure technics. A s#yringe technic to feel loss-of-resistanse is now widely used clinically due to its reliability and simplicity. However, in some instances, it is very difficult to recognize the epidural space despite using the above methods so that the need for a more safe and easier one is required. After the patient is placed in a sitting flexed position, an 18 gauge Tuohy needles is inserted epidurally at the lumbar area with a saline filled CVP manometer connected via a 3-way stopcock, then a sudden drop of pressure is usually observed and a fluctuation of the pressure can be observed in the water column of the manometer according to the changes in positions, respiration and heart beat. Although this method can be criticized because the technique is somewhat cumbersome and is difficult for retrial when the dura has been punctured, the authors appreciate the experimental values of the technic and suggest its aduantanges as follows: 1) it is a visual thchnic. 2) the measurement of epidural pressure is possible. 3) it prevents back-drip of local anesthetic solution with a 3-way stopcock. 4) it is useful as an index for determination of local anesthetic volume to be injected.
