In one case of cancer-associated Lambert-Eaton myasthenic syndrome (LEMS), treatment with cytotoxic chemotherapy and radiotherapy resulted in remission of both cancer and neuromuscular disorder. Serial repetitive nerve stimulation (RNS) findings showed that the typical RNS features of LEMS returned to normal in the orders of dramatic postexercise facilitation, an incremental response in high rate stimulation, and a decremental response in low rate stimulation with clinical improvement.

7.Cutaneous Metastasis on the Face from the Cardiac Angiosarcoma.

Angiosarcoma is a rare, aggressive vascular neoplasm of the endothelial cell origin. It is usually seen in male, elderly patients and generally has a poor prognosis due to its high potential for metastasis. They arise at various sites such as skin, soft tissue, heart and great vessels. However, the tumors have a predilection for skin and superficial soft tissue. Most angiosarcomas of the skin arise in the three clinical variant; primary angiosarcoma of the face and scalp in the elderly patients; following lymphedema, and post radiation or trauma. Although, this tumor tend to recur locally and metastasize to the viscera, especially in the lung, but cutaneous metastasis of angiosarcomas from the internal organ are uncommon. Herein, we report a patient with cutaneous metastasis from primary cardiac angiosarcoma in the right atrium.

8.Characteristics of Scabies Patients in Goyang City and Transmission Route.

BACKGROUND: Scabies is an ectoparasite caused by the mite Sarcoptes scabiei var. hominis, an obligate human parasite. Although its incidence has been decreasing, it is not a rare dermatologic disease and is difficult to diagnosis. OBJECTIVE: To evaluate the clinical characteristics of scabies patients and find out etiologic factors in transmission. METHODS: Two hundred and ninety-five patients who were diagnosed with scabies at the Dermatologic Department, Myongji Hospital, Goyang city, Korea were enrolled. Medical records were evaluated in a retrospective manner. RESULTS: We studied 295 scabies patients who were diagnosed by the mineral oil test or skin biopsy. The mean age of the patients was 58.2 (male: 56.7/female: 61.2), and 74% of patients were over 60 years. There was seasonal variation in occurrence, and scabies was found to be most common in winter. The majority of suspected routes of infection were transmission through convalescent hospitals (38.1%) and caregivers (15.2%). Nosocomial infection and communal living were the major causes. 73.8% of scabies patients had underlying medical disease, such as hypertension (32.2%) and dementia (11.9%). CONCLUSION: Currently, scabies prevalence is still increasing in Goyang city, because of the increased number of convalescent hospitals.

9.Localized amyloidosis of seminal vesicle and vas deferens: Report of Two Cases.

We reported localized amyloidosis involving seminal vesicles and vasa deferentia, which was found in two patients with prostatic adenocarcinoma. A 60-yr-old (Case 1) and a 59-yr-old (Case 2) man came to our hospital with elevation of serum prostate-specific antigen (PSA) and biopsy proven carcinoma, respectively. MRI revealed multiple irregular foci of low signal intensity in the prostates as well as in both seminal vesicles and vasa deferentia on T2-weighted imaging, suggesting prostatic carcinoma with extension to both seminal vesicles and vasa deferentia in both cases. Under the clinical diagnosis of stage III prostatic adenocarcinoma, a radical prostatectomy was performed in both patients. Microscopically, Gleason score 7 adenocarcinoma was observed in both patients. In addition, isolated amyloidosis of both seminal vesicles and vasa deferentia was found without carcinoma involvement. Localized amyloidosis in the seminal vesicles, which is considered as senile process, has been occasionally reported in the autopsy and in the surgical specimens. Amyloid deposition in the vas deferens has also been reported in the literature, however, the deposition mimicking extension of carcinoma has not been reported. In this report, two cases of isolated amyloidosis of the seminal vesicles and vasa deferentia are described with electron microscopic study and literature review.

10.A Case of Basaloid Follicular Hamartoma.

Basaloid follicular hamartoma (BFH), uncommon rare benign neoplasm connected to the adnexal structures, presents with multiple clinical manifestations that can develop into basal cell carcinoma. BFH may be congenital or acquired, and the congenital form can be further divided into the generalized and unilateral type, and the acquired form may present as localized and solitary lesions. Congenital, generalized BFH is associated with systemic diseases such as alopecia, cystic fibrosis, hypohidrosis, and myasthenia gravis. In contrast, sporadic cases are observed as unilateral or localized lesions. BFH shows thick cords and thin strands of anastomosing basaloid proliferations that arise from hair follicles and are enclosed by loose fibrous stroma. Here, we report a 70-year-old man with an acquired, solitary form of BFH.
