6.A Case of Remitted Lambert-Eaton Myasthenic Syndrome with Small Cell Lung Carcinoma Following Chemotherapy and Radiotherapy.

In one case of cancer-associated Lambert-Eaton myasthenic syndrome (LEMS), treatment with cytotoxic chemotherapy and radiotherapy resulted in remission of both cancer and neuromuscular disorder. Serial repetitive nerve stimulation (RNS) findings showed that the typical RNS features of LEMS returned to normal in the orders of dramatic postexercise facilitation, an incremental response in high rate stimulation, and a decremental response in low rate stimulation with clinical improvement.

7.A Statistical Study of Dermatoses in Goyang City (2004~2008).

BACKGROUND: There have been many reports that have focused on the incidence of skin diseases in big cities or provinces. However, there have been no reports concerning the data of a new town such as Goyang City. OBJECTIVE AND METHODS: To study the prevalence of common dermatoses in the new town and to compare this with the previous reports, we reviewed the records of 15,064 outpatients who visited the Department of Dermatology at Myongji Hospital from 2004 to 2008, and we statistically analyzed the findings. RESULTS: The study results are summarized as follows: Of the 15,064 outpatients, the total number of male patients was 6,779 (45.0%), and the number of female patients was 8,285 (55.0%). The most frequent age group was the fourth decade followed by the first decade, third decade and fifth decade, and these 4 age groups comprised 63.7% of the total outpatients. The 15 most common dermatoses were found to be viral warts, dermatophytosis, atopic dermatitis, seborrheic dermatitis, pruritus, urticaria, irritant contact dermatitis, herpes zoster, alopecia, allergic contact dermatitis, acne, other eczema, melanocytic nevus, molluscum contagiosum and postinflammatory hyperpigmentation. For the occurrence of dermatoses as disease groups, 1) viral diseases, 2) diseases resulting from fungi and yeasts, 3) atopic dermatitis, eczema and noninfectious immunodeficiency disorders, 4) contact dermatitis and drug eruptions, 5) pruritus and neurocutaneous dermatosis, 6) seborrheic dermatitis, psoriasis, recalcitrant palmoplantar eruptions, pustular dermatitisand erythroderma, 7) erythema and urticaria, 8) Melanocytic nevi and neoplasm, 9) disturbances of pigmentation, and 10) diseases of skin appendages constituted 82.2% of the total. CONCLUSION: The distinct characteristic of skin diseases of the new town Goyang City showed a high incidence of viral warts and viral skin diseases, as compared to the previous reports.

8.Cutaneous Metastasis on the Face from the Cardiac Angiosarcoma.

Angiosarcoma is a rare, aggressive vascular neoplasm of the endothelial cell origin. It is usually seen in male, elderly patients and generally has a poor prognosis due to its high potential for metastasis. They arise at various sites such as skin, soft tissue, heart and great vessels. However, the tumors have a predilection for skin and superficial soft tissue. Most angiosarcomas of the skin arise in the three clinical variant; primary angiosarcoma of the face and scalp in the elderly patients; following lymphedema, and post radiation or trauma. Although, this tumor tend to recur locally and metastasize to the viscera, especially in the lung, but cutaneous metastasis of angiosarcomas from the internal organ are uncommon. Herein, we report a patient with cutaneous metastasis from primary cardiac angiosarcoma in the right atrium.

9.Embolia Cutis Medicamentosa Following Intramuscular Injection of Diclofenac.

Nicolau syndrome or embolia cutis medicamentosa is a rare complication of a still largely unidentified pathogenesis at the site of intramuscular injections of various drugs. It is characterized by development of an acute, severe pain around the injection site followed by a localized erythema, a reticular rash, a hemorrhagic patch and varying degrees of tissue damage. The disease outcomes vary from atrophic ulcers and pigmentation to lower limb paralysis and limb amputation. We describe a 34-year-old woman with the diagnosis of this syndrome after she received an intramuscular diclofenac-beta-dimethyl-aminoethanol injection to the lateral aspect of the right upper buttock. The patient was treated by surgical debridement with concomitant primary closure.

10.Characteristics of Scabies Patients in Goyang City and Transmission Route.

BACKGROUND: Scabies is an ectoparasite caused by the mite Sarcoptes scabiei var. hominis, an obligate human parasite. Although its incidence has been decreasing, it is not a rare dermatologic disease and is difficult to diagnosis. OBJECTIVE: To evaluate the clinical characteristics of scabies patients and find out etiologic factors in transmission. METHODS: Two hundred and ninety-five patients who were diagnosed with scabies at the Dermatologic Department, Myongji Hospital, Goyang city, Korea were enrolled. Medical records were evaluated in a retrospective manner. RESULTS: We studied 295 scabies patients who were diagnosed by the mineral oil test or skin biopsy. The mean age of the patients was 58.2 (male: 56.7/female: 61.2), and 74% of patients were over 60 years. There was seasonal variation in occurrence, and scabies was found to be most common in winter. The majority of suspected routes of infection were transmission through convalescent hospitals (38.1%) and caregivers (15.2%). Nosocomial infection and communal living were the major causes. 73.8% of scabies patients had underlying medical disease, such as hypertension (32.2%) and dementia (11.9%). CONCLUSION: Currently, scabies prevalence is still increasing in Goyang city, because of the increased number of convalescent hospitals.
