5.Difference of the Accuracy between Autorefration and Subjective Refraction in Photorefractive Keratectomy(PRK) and LASIK-Treated Eyes.

Authors decided to test whether differences occurred in automated and subjective refraction in untreated, PRK, and LASIK-treated eyes. Ninety six eyes of 50 patients(96 eyes) who underwent PRK(57 eyes) or LASIK(39 eyes) for myopia and myopic astigmatism were routinely autorefracted with the CANON RK-3 before subjective refraction was done, using several parameters. This two procedures were done preoperation and 3 months after operation. In comparing the postoperative(PRK, LASIK) results, automated refraction showed more myopia and higher cylinder power, without significant difference in refractive axis, than those of subjective measurement. The difference of cylinder power were found in the cases of large ablation depth(above 100micrometer), high degree of eccentric ablation(above 0.25 mm), LASIK operation. The significant differences of cylinder power and axis were found in the cases of high degree of eccentric ablation(above 0.25 mm). So we recommend the method of subjective refraction before PRK and LASIK operation instead of using automated refraction. Postoperatvely, we also should evaluate the postoperative status of the patient by subjective refraction. And we should decide the amount of ablation in case of retreatment by subjective refraction, instead of autorefraction. We have to be cautious of evaluating the automated refraction results after operation, especially in cases of large ablation depth, high degree of eccentric ablation, and LASIK.

6.Two Cases of Venous Hemangioma.

Venous hemangioma is a vascular tumor that has been reported by such diverse names as; "cirsoid aneurysm", "arteriovenous hemangioma", "acral arteriovenous tumor" according to the author's opinions of its origin and histopathologic classification. It is benign and rarely biopsied, and it is also rarely reported in dermatology literature. We present two cases of venous hemangioma. The first case was a 64-year-old man who was presented with an elliptical dark-red plaque with overlying several grouped papules on the left periorbital area which had been present for 7 years. The second case was a 56-year-old man who was presented with a single red papule on the right postauricular area which had been present for 4 months. Histopathologically, there were a well-circumscribed proliferation of thick-walled muscle-containing blood vessels in the dermis. Intermingled with the thick-walled blood vessels, there were also thin-walled dilated blood vessels. In both cases, the thick-walled blood vessels resembled arteries, but they lacked elastic fibers on the Verhoeff van Gieson stain.

8.Deficit in Decision-Making in Chronic, Stable Schizophrenia: From a Reward and Punishment Perspective.

OBJECTIVE: We compared patients with chronic schizophrenia and normal controls with respect to decision-making ability. Measures were implemented to control for the participants' intelligence levels as well as to ensure to use of a moderate sample size. The goal of this study was to confirm inconsistent results from previous studies which had stemmed from too small of a sample size, highly variable performance of normal controls, and not controlling for intelligence as a confounding factor. METHODS: Fifty-two chronic stable schizophrenic inpatients and 55 healthy controls participated in the study. We controlled for intelligence by including subjects with intelligence quotient's (IQ) between 80 and 120, examining any differences in decision-making performance between groups on the Iowa Gambling Task (IGT). We also addressed several issues relating to performance on the IGT, such as working memory and clinical symptoms. RESULTS: Schizophrenic patients were found to perform poorly on the IGT relative to normal controls (F(1,105)=17.73, p<0.001); however, more importantly, they also displayed the slow yet profitable shift from disadvantageous decks to advantageous decks over time. We also found that when compared with healthy controls, schizophrenic patients showed a poorer performance on the Wisconsin Card Sorting Test (WCST)(t=-5.48, p<0.001 for perseverative error) which was not related to their performance on the IGT. CONCLUSION: Based on previous literature and the results of this study, impaired sensitivity to both reward and punishment might be a more plausible explanation for the poor performance on the IGT in the schizophrenic group. We speculated that this impairment seemed related more to the different responsiveness to the magnitude than to the frequency of punishment, and to the different interpretation of less informative verbal cues in the context of the reinforcing schedule.

9.The Application of Endoscopic Mucosectomy in Various Mucosal Lesioss of Upper Gastrointestinal Tract.

In case of submucosal tumor, adenoma and atypism, we can't frequently use the snare polypectomy for treatment of them. Adenoma and atypism are premalignant, so other therapeutic modalities are necessary for resection of them. We have perfortned endoscopic mueosectomy such as strip biopsy, and "O"ring ligation on various mucosal lesions of upper GI tract in 23 patients(11 men, l2 women) from June 1993 to December l994. Fourteen patients wbo had adenoma were followed up endoscopically for 6 months to 24 months(mean: 14 months). The results were as follows: 23 patients(27 lesions) were enrolled in this study. Mean age was 50.3(range 32-74 years). The method of mucosectomy was strip biopsy in 19 cases(21 lesions), and "O"ring ligation in 4 cases(6 lesions). The rate of complete resection was 88.9%. If adenomas were resected incompletely, we added a laser therapy. There was no recurrence except a case in which carcinoma was detected on the other site during follow-up. No serious compication occurred such as massive bleeding or perforation. Based on the above results, the endoscopic mucosectomy is considered to be a safe and effective modality in the treatment of the flat polyps of upper GI tract and could be used for prevention of gastric cancer.
