Primary malignant fibrous histocytoma (MFH) of pleura is very rare. Upon microscopic examination, the tumor was characterized by storiform pattern of growth and intense, diffuse immunostaining for alpha l-antichymotripsin. We report a case of primary malignant pleural fibers histiocytoma with extension to lung, which was managed by complete excision of pleural mass and lobectomy with thoracoplasty.

2.Treatment of the Infected Ununited Fractures of the Tibia by Posterior Bone Graft

In treatment of infected ununited fractures of the tibia, We have solved this disastrous problems by posterior bone graft through posterolateral approach. By this posterior bone graft, We can achieve both bone union and infection contral simultaneously. The results were as follows: 1. Total cases of the infected ununited fractures were 17 cases, among them 16 cases were open fracture initially. 2. They occured more commonly in young adult, 7 cases(41%) were in 4th decade, and all cases were male. 3. Tibial union was achieved in 16 cases and one patient can not walk without brace due to ain on fracture site. 4. Infection was cleared in 16 cases, but one case had intermittent drainage of pus. 5. The most common complication was stiffness of the ankle and tarsal joint. 6. There was no infection in grafted bone.

4.The Effects of High Glucose on Paraquat-induced Cell Injury in Human Retinal Pigment Epithelial Cells.

PURPOSE: Functional and structural alterations of retinal pigment epithelial cells were observed in experimental and clinical diabetes. To investigate the effects of high glucose on free radical-induced injury in retinal pigment epithelial cells, we determined the effects of high glucose on activities of antioxidant enzymes and paraquat-induced cytotoxicity in cultured human retinal pigment epithelial (HRPE) cells. METHODS: Human retinal pigment epithelial (HRPE) cell line (ATCC:CRL-2302) was cultured with high glucose (22.4 mM)-and normoglucose (5.6 mM)-contained DMEM for 3 days. The activities of superoxide dismutase (SOD), catalase and GSHPx were assayed by Crapo's method, Aebi's method and GUnzler's method, respectively. Paraquat-induced cytotoxicity was determined by 3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyl tetrazolium bromide (MTT) method. RESULTS: CuZn-SOD activity of HRPE cells was decreased by 32% in high glucose (25.4 mM) media compared to normoglucose (5.6 mM) media. But the activities of catalase and GSHPx were not changed by high glucose. The paraquat-induced HRPE cells toxicity was increased by high glucose. Diethydithiocarbamate (DDC), as inhibitor of CuZn-SOD, also potentiated paraquat-induced HRPE cell CONCLUSIONS: Paraquat-induced HEPE cells injury was potentiated by high glucose and decreased CuZn-SOD activity by high glucose may be some roles in free radical-induced HRPE cell injury.

5.A Case of Supravalvular Aortic Stenosis.

Supravalvular aortic stenosis may be defined as an obstructive congenital deformity of the ascending aorta whitch originates just distal to the level of the origins of the coronary arteries, and whitch includes a wide spectrum of pathologic changes. A 27 years-old-female was admitted because of further evaluation of known some heart disease. Clinical diagnosis of supravalvular aortic stenosis was made by echocardiography and angiography. We presented a case of supravalvular aortic stenosis with a review of literature.

6.Mid-term Result of Operations for Atrial Fibrillation.

BACKGROUND: Majority of patients with mitral valve disease and chronic atrial fibrillation can resume sinus rhythm with the maze procedure. The purpose of this study is to identify mid-term outcome and possibility of recurrent atrial fibrillation in the patients who underwent mitral valve operation and the maze procedure. MATERIAL AND METHOD: A retrospective study was undertaken on 11 consecutive patients(including 10 patients with rheumatic mitral valve disease) operated on for mitral valve disease and chronic atrial fibrillation. Maze II and its modified procedure were performed in 6 patients and maze III and its modified procedures in 5 patients. Postoperative left atrial contractility was compared between the patients that underwent mitral valve operation and maze procedure and the patients with mitral valve procedure and medication of anti-arrhythmic drugs. RESULT: Five(83.3%) of 6 patients with maze II procedure and all 5 patients(100%) with maze III procedure resumed sinus rhythm. In 1(20%) of 5 patients with sinus rhythm after maze II procedure and 2(40%) of 5 patients with sinus rhythm after maze III procedure, atrial fibrillation recurred 23, 2, and 13 months after the operations, respectively. None of three patients with the modified procedure had recurrence of atrial fibrillation. Of 3 patients with recurrent atrial fibrillation, 2 patients resumed sinus rhythm and 1 still had atrial fibrillation. After the maze procedures, only 4 (40%) of 10 patients with sinus rhythm presented left atrial contraction, and their mean velocity of mitral 'A' wave was lower than that of the patients who resumed sinus rhythm with only valve replacement and anti-arrhythmic drugs. CONCLUSION: Although the maj ority of patients with mitral valve disease and chronic atrial fibrillation can resume sinus rhythm with the maze procedure associated with mitral valve operation, atrial fibrillation may recur in mid-term. Also, the left atrial contractility may be very poor in patients who resumed sinus rhythm with maze procedure.

BACKGROUND/AIMS: Helicobacter pylori is found in more than 90% of patients with gastric mucosa-associated lymphoid tissue (MALT) lymphoma. Regression of lymphoma after H. pylori eradication was reported. Therefore, early detection of gastric MALT lymphoma is important. On reported several endoscopic findings, nonspecific gastritis with erosion and ulceration were common and there is no typical endoscopic findings. METHODS: Therefore we analyzed the endoscopic findings of 16 low-grade B-cell gastric MALT lymphoma confirmed by biopsy, PCR and immunostain and compared histologic change after eradicating H. pylori infection in patients who refused operation retrospectively from March 1995 to October 1996. RESULTS: The male: female ratio was 7:9 and mean age was 43,7 years ald (23-71 yr), On endoscopic findings of gastric MALT lymphoma, irregular shaped geographic superficial ulcer was 7 cases (43.7%), diffuse mucosal nodularity was 4 cases (25.0%) and erosion was 3 cases (18.7%). The most common site of MALT lymphoma was antrum and angle. H. pylori was positive in 87.5% (14/16 cases) and eradication of H. pylori was done in patients who refused operation and histologic improvement after H. pylori eradication was found in 62.5% (5/8 cases). CONCLUSIONS: The most common endoscopic findings of gastric MALT lymphoma was irregular geographic superficial ulcer but diffuse mucosal nodularity and erosions were also common. Therefore, biopsy must be taken, especially on the antrum and angle although nonspecific lesion may be suspected.

10.Clinical Observation on Ruptured Aneurysm of the Sinus of Valsalva.

From May, 1975 to August, 1986, we experienced 21 patients with ruptured aneurysm of the sinus of Valsalva. 1) Their ages ranged from 15 to 52 years with a mean age of 26.7 years, and 14 patients were male and 7 patients were female. Among 18 cases of which aneurysms occurred on the right sinus of Valsalva, 15 cases (83%) ruptured into the right ventricle, 1 case (6%) ruptured into the right atrium and 2 cases (11%) ruptured into the pulmonary artery. All 3 cases of which aneurysms occurred on the noncoronary sinus ruptured into the right atrium. 2) Clinical symptoms consisted of dyspnea (95%), palpitation (81%), chest pain (43%), orthopnea (33%), cough (14%), hemoptysis (5%), oliguria (5%), and fever (5%). In all patients continuous heart murmur was audible. Hepatomegaly (48%), pulmonary basal rale (14%), and pretibial pitting edema (19%) were also observed. 3) In all patients diagnosis was made with cardiac catheterization and angiography, and confirmed by operation excetpt one case. 4) Among 20 patients who had been performed corrective surgery, ventricular septal defect was observed in 16 patients (85%), aortic insufficiency in 5 patients (25%), pulmonary infundibular stenosis in 1 patient (5%), patent foramen ovale in 1 patient (5%), and tricuspid insufficiency in 1 patient (5%). Ventricular septal defect was observed in all 16 patients whose aneurysms of the right coronary sinus of Valsalva ruptured into the right ventricle and the pulmonary artery. 5) All patients survived operation, and no postoperative complication was found except 1 patient to whom reoperation was performed because of postoperative aortic insufficiency and detachment of the patch closure for ventricular septal defect. There was symptomatic improvement in all patients.
