1.Prevalence of Diabetes Mellitus and Glycosuria in School Children.

An epidemiological survey on the glycosuria was conducted on the senier students of middle and high school in Seoul, from March 1981 to March 1982. Selected examinee totalled to 254,835 of which 138,116 were male & 116,719 were female. The initial, second and third urine sugar test were done with Combur-test strip(r). Glucose tolerance test was performed on those who were found to have persistent glycosuria. The followings showed the results of the survey. Among the 254,835 examinees, 1,240 were found with initial positive glycosuria. The glycosuria positive rate revealed 0.52% for male, 0.44% for female. Its average was 0.49%. It also revealed that male students in high school had higher positive rate of glycosuria. 2) The persistent glycosuria positive rate revealed 11.58/100,000 for male, 15.42/100,000 for female and its average was 13.34/100,000. 3) Out of the 33 cases with the persistent glycosuria, 60.6% of them were diagnosed as diabetes mellitus. 4) The overall prevalence rate of diabetes was 7.85/100,000. The prevalence rate of male was5.79/100,000, and female was 10.28/100,000. Consequently the rate of male to female was 1:1.78, showing that rates were higher for females than males. The. survey also revealed that the prevalence rate of diabetes was increased with advancing grade. 5) The 70% of the total person who were diagnosed as diabetes, did not recognize of their disease by themselves till the survey was conducted. 6) The frequency of subjective diabetic symptoms were polydipsia, polyuria and polyphagia in order. *Combur-test(r)are registered trade marks of the Boehringer mannheim GmbH Company.

2.Elbow Arthroscopy for the Stiff Elbow

Twenty-five consecutive patients who had contracture of the elbow were treated by arthroscopy. The techniques were removal of loose bodies, removal of osteohyte, anterior capsular release, abrasional arthroplasty and excision of the radial head. The type of arthroscopic procedure was determined by the cause of limiting motion which was intra-articular(intrinsic). The mean preoperative arc of total motion was 92°(21°-113°). Re-examination of the elbows after anaverage follow-up of 19 months showed tbat the mean arc of total motion was 116°(14°-130°). Twenty-three out of twenty-five patients (92%) who were followed up were satisfied with the results of the procedure and exhibited improved ability in carrying out daily activities. In conclusion, arthroscopy of the elbow is an effective diagnostic procedure and is also effective in treating certain intra-articular problems with minimal morbidity and rapid recovery to function.

5.Arthroscopic Meniscal Repair.

Between December 1994 and December 1996, 42 patients underwent arthroscopic meniscal repair in our institute. Among them, 31 patients with a follow up more than one year were reviewed. Mean age was 24.4 years(range, 18-52 years) and mean interval between onset of symptom and opera- tion was 12.3 months(range, 3 days-6 years). The associated injuries were tibial plateau fracture(6 cases), anterior cruciate ligament injury(4 cases) and posterior cruciate ligament injury(2 cases). The type of tear were longitudinal in 28 cases(90%) and combined in 3 cases(10%). Most of the meniscal tear were encountered in posterior hom(12 cases, 39%) and middle portion(11 cases, 35%). Out side to in technique was used in anterior horn tears and inside to out technique in middle or posterior hom tears. In tibial plateau fractures, arthroscopic assisted reduction was performed with autogenous bone graft. Ligament reconstructions were performed in the cases of associated cruciate ligament injury. According to Ikeuchis grading system, the result was satisfactory(excellent or good) in 23 cases(74%), fair in 6 cases(19%) and poor in 2 cases(6%). But in the cases of isolated meniscal injury, the result was satisfactory in 95%. The result of arthroscopic repair in the meniscal tears associated with other intra-articular injuries seems to be worse than that of the isolated injuries.

6.Nephrotic syndrome under 2 years of age.

There had been total 20 patients with early onset(4 months~2 years) primary nephrotic syndrome in the Department of Pediatrics, Seoul National University Children's Hospital, during the period from March 1987 to February 1993. We analysed clinical courses, response to treatment, pathological findings and prognosis of the patients And the results were as follows; 1) The initial responders to steroid treatment were 10(50%), of whom 3 became late nonresponders. Of the 10 initial nonresponders, 8 revealed continuing nonresponsiveness. 2) Incidence of hypertension was significantly higher in the intial nonresponders than in the initial responders. 3) Renal biopsies were performed in 9 initial nonresponders and 2 late nonresponders. And the results were 6 with minimal change lesion, 4 with focal segmental glomerulosclerosis and 1 with mesangiocapillary glomerulonephritis. 4) Six continuing or late nonresponders received methylprednisolone pulse therapy or cyclophosphamide. And partial remission was induced in 4 of them. 5) The overall 3 year maintenance rate of normal renal function was 81%(100% in the initial responders and 64% in the initial nonresponders). In conclusion, the initial steroid responsiveness and prognosis were poorer in patients with early onset nephrotic syndrome than in patients with usual childhood nephrotic syndrome.

7.Analysis of the Gene Expression by Laser Captured Microdissection (I): Minimum Conditions Required for the RNA Extraction from Oocytes and Amplification for RT-PCR.

OBJECTIVE: Recently, microdissection of tissue sections has been used increasingly for the isolation of morphologically identified homogeneous cell populations, thus overcoming the obstacle of tissue complexity for the analysis cell-specific expression of macromolecules. The aim of the present study was to establish the minimal conditions required for the RNA extraction and amplification from the cells captured by the laser captured microdissection. METHODS: Mouse ovaries were fixed and cut into serial sections (7 micrometer thickness). Oocytes were captured by laser captured microdissection (LCM) method by using PixCell IITM system. The frozen sections were fixed in 70% ethanol and stained with hematoxylin and eosin, while the paraffin sections were stained with Multiple stain. Sections were dehydrated in graded alcohols followed by xylene and air-dried for 20 min prior to LCM. All reactions were performed in ribonuclease free solutions to prevent RNA degradation. After LCM, total RNA extraction from the captured oocytes was performed using the guanidinium isothiocyanate (GITC) solution, and subsequently evaluated by reverse transcriptase -polymerase chain reaction (RT-PCR) for glyceraldehyde-3-phosphate-dehydrogenase (GAPDH). RESULTS: With the frozen sections, detection of the GAPDH mRNA expression in the number of captured 25 oocytes were not repeatable, but the expression was always detectable from 50 oocytes. With 25 oocytes, at least 27 PCR cycles were required, whereas with 50 oocytes, 21 cycles were enough to detect GAPDH expression. Amount of the primary cDNA required for RT-PCR was reduced down to at least 0.25 microl with 50 oocytes, thus the resting 19.75 microl cDNA can be used for the testing other interested gene expression. Tissue-to-slide, tissue-to-tissue forces were very high in the paraffin sections, thus the greater number of cell procurement was required than the frozen sections. CONCLUSION: We have described a method for analyzing gene expression at the RNA level with the homogeneously microdissected cells from the small amount of tissues with complexity. We found that LCM coupled with RT-PCR could detect housekeeping gene expression in 50 oocytes captured. This technique can be easily applied for the study of gene expression with the small amount of tissues.

8.A Case of Transient Complete Atrioventricular Block in Acute Viral Myocarditis.

Arrythmias are often present and may lead to unexpected death in acute myocarditis. Occasionally, conduction disturbances are seen with varying degree of heart block yet complete atriove ntricular block is rare. We experienced a case of transient complete heart block caused by coxsackie B5myocarditis in a previously healthy 12-year-old girl. During the use of a transvenous temporary pacemaker for 5 days, the abnormal cardiac rhythm changed to sinus rhythm and clinical status improved progressively.

10.Fusarium Fruit Rot of Citrus in Jeju Island.

Twenty-three isolates of Fusarium spp. were obtained from decayed citrus fruits in the fields and storages in 1998-1999. Of them, six and five isolates belonged to F. proliferatum and F. moniliforme, respectively, which were the most common. F. solani and F. sambucinum had each two isolates, F. equiseti had one isolate and seven isolates were unidentified. They produced symptoms of two types in pathogenicity test: those with leathery, beige to light or dark brown, and sunken lesions without surface mycelium (type-1) and those with lesions covered with white, beige or pink surface mycelium (type-2). Four of six isolates identified to F. proliferatum and two unidentified isolates produced type-1 lesions, and all isolates identified to F. moniliforme, F. solani, F. sambucinum, F. equiseti and five unidentified isolates produced type-2 lesions.
