4.Evaluation of Treatment Effect of Primary Hyperhidrosis using Skin Surface Hydrometer.

BACKGROUND: Primary hyperhidrosis is the disease of production of exessive sweat which is mainly localized in palm, sole, and axilla, treatment of the disease is usually accomplished with topical 20% aluminum chloride solution, an iontophoretic device, and surgery(sympathectomy) being reserved for recalcitrant cases. OBJECTIVE: We have compared the therapeutic effects with each treatments for hyperhidrosis using a skin surface hydrometer. METHODS: Thirty six patients(11 ; male, 25 ; femlale) with primary hyperhidrosis were evaluated in this study. Nineteen patients used topical 20% aluminum chloride solution, 10 patients used iontophoretic device, and 7 patients in recalcitrant cases. had sympathectomy. We have measured the conductance on the stratum corneum of the palm and sole suing skin surface hydrometer before and after treatment every weeks for a month. the control group were composed of healthy 10 males and 2 females. RESULTS: 1) In the patients of primary hyperhidroisis the conductances of palm, sloe and axilla were higher than that of control group(P<0.01). 2. There was a marked reduction of conductance after the treatment for one week by topical 20% aluminum chloride solution, an lontophoretic device, and sympathectomy(P<0.01). 3. After 2-4 week of treatment by the change obsetved after treatment for one week. 4. By iontophoretic device, there was a slight increasement of conductance of the palm after 2-3weeks of treatment. 5. We obseved a sustained decrease in the conductance by sympathectomy into 4 week. CONCLUSION: Topical 20% aluminum chloride solution, iontophoretic device, and sympathectomy were effective for treatment of hyperhirosis. Among hem sympathectomy showed the best effect. We observed that the skin surface hydrometer is useful to evaluate of the efficacy of the treatment.

5.Vesical Endometriosis : A Review and 1 Case Report.

The term endometriosis refers to ectopic endometriume. It most often involves the pelvic organs, including ovaries, broad and round ligaments, fallopian tubes, cervix, vagina and the pouch of Douglas. We report a case of vesical endometriosis in a 4- year old female patient and review literature. Segmental resection of this mass with involved bladder resulted in cure and this lesion was confirmed histologically as endometriosis of the bladder.

6.Decreased Basal Tear Production Associated with General Anesthesia.

Two types of tear secretion, basic and reflex, have been postulated. Basic secretion is thought to be a constant,slow secretion by the accessory lacrimal glands. Reflex secretion was defined as an increased rate of secretion caused by neural stimulation and thought to be secreted by the main lacrimal gland. The purpose of the present study is to measure the effect of general anesthesia and atropine on basal tear secretion in order to elucidate the association between basal tear secretion and autonomic nervous system. We studied the 84 cases of general operations that were performed between March 1998 and June 1998. Basal tear measurements were made before premedication, 5 minutes after injection of atropine, 10 minutes after general anesthesia and 1 hour after general anesthesia. Basal tear secretion was lower at 5 minutes after injection of atropine than before premedication and markedly decreased after induction of general anesthesia. Therefore, we think that basal tear secretion is closely related to the autonomic nervous system.

7.The Effect of Systemic Steroid Therapy on the Tuberculous Ureteral Stricture.

Ureteral stricture are not uncommon sequelae in renal tuberculosis. Oral steroid therapy has been tried as an alternative to surgery in some of these patient on several reports. Three ureteral stricture were effective in 5 patient by oral steroid therapy.

8.Malakoplakia Clinically Simulating Bladder Tumor - A Case Report -.

Malakoplakia of the bladder is an extremely rare condition in which there are multiple round or oval, plaque like or occasionally polypoid projections of the mucosa and submucosa which may be mistaken for cancer. It is believed to be infectious in origin, secondary to a deficiency of intracellular lysosomal digestion. There is often a clinical history of urinary tract infection, and the condition is considered to be an atypical form of granulomatous response to Gram negative coliform infection. We report a case of malakoplakia of the bladder clinically simulating malignant tumor, which was treated successfully with transurethral resection and antibiotics.

9.Sympatheic Skin Response and Nerve Conduction Studies in Diabetic Neuropathy.

Fifty eight diabetic patients and 20 normal controls were studied by the method of nerve conduction studies(NCS) and sudomotor skin reponse(SSR), and the following results were obtained. 1. SSR was easily obtainable in all normal control. 2. The patients with abnormal NCV showed more numbers of abnormal SSR than the pat ients with normal NCV. 3. The patients with abnormal SSR were significantly older than the patient with normal SSR, but duration of disease was not different between the patients with normal SSR and with abnormal SSR. 4. The levels of sugar at fasting and postprandial 2 hours, and level of HbAlC in serum were not different between the patients of norrnal SSR and of abnormal SSR. 5. The incidence of autonomic symptoms, such as syncope, sweating abnormality, impotence in male, urinary disturbance, postural hypotension, was not different between the patient of norrnal SSR and of abnormal SSR. 6. Among the patients of diabets, the number of abnormal nerves in terminal latency motor and sensory nerve condcution velocity, and amplitude of compound motor and nerve action potentiaL were significantly correlated with the number of abnormal nerves in SSR. 7. The absolute values of motor terminal latency, motor and sensory conduction velocity and amplitude of compound motor and sensory action potential in the patient of abnormal SSR were significanly decreased thai. The patients with norrnal SSR. The values of terminal latency in abnormal SSR were significanly prolonged than the patients with normal SSR We concluded that the SSR test is a valuabe one for early detection of sympathetic nerve invasion in the diabetes' patients.

10.Caliceal Anatomical Structure by Radiological Examination.

Developing the instruments in urological field, the urinary tract stones may be manipulated more easily and lesser invasively recently. The percutaneous nephrolithotripsy is one of the most widely using technique for the renal stone. But, the major portion of the failure through the manipulation of percutaneous nephrolithotripsy is the access to the correct, wanted calyx by puncture. So, we observed the 27 normal right kidneys and 26 left kidneys by abdominal computer tomogram scanning and excretory urography for the Korean caliceal anatomy. The following results were obtained 1. The average values of posterior renal rotation from the coronal plane of body were 31.0 degrees +/- 4.4 degrees in the right kidneys 32.4 degrees +/- 3.2 degrees in the left kidneys. 2. The average values of anterior and posterior caliceal angle were 58.8 degrees +/- 7.6 degrees and 20.7 degrees +/- 5.8 degrees in the right kidneys; 38.3 degrees +/- 6.2 degrees and 43.0 degrees +/- 5.2 degrees in the left kidneys. 3. The major type of classical system was Brodel type (74.l%) in the right kidney; Hodson type (53.8%) in the left kidney. 4. The anterior calyx is more laterally placed about 8l.8% in the excretory urography.
