1.The Treatment of Acromioclavicular Seperation

There are many procedures described for the treatment of acromioclavicular seperation but there are still controversies concerning the best management of these injuries. Thirteen cases were operated on by technique of modified Phemister method and four cases by Bosworth method, Dept. of Orthopedic Surgery, Chonbuk National University Hospital from January 1980 to December 1984. The following results were obtained. The following results were obtained. 1. This injuries is more prevalent in male (76.4%) with peak incidence in the second, third and fourth decades(76.4%). 2. The most common causes of the injuries were traffic accidents and followed by falling from the height. 3. Fifteen patients were grade 3 by Allmans classification. 4. Operative method consists of modified Phemister method (76.4%) and Bosworth method (23.6%). 5. The operative procedures in Type 2 and Type 3 are good treatment of acromioclavicular seperation.

2.Clinical and Histo-Pathological Analysis for Recurrence after Curative Surgery of Esophageal Cancer.

BACKGROUND: Surgical resection remains the mainstay of treatment for esophageal cancer. Despite recent advances in surgical therapy, i.e. en bloc resection and extended lymphadenectomy, the overall long-term prognosis of patients with esophageal carcinoma has not, however, improved during the last decades. One of the major reasons in its relatively high recurrence rate. MATERIAL AND METHOD: A retrospective review of recurrent patterns of cancer in 42 patients who underwent curative surgery for primary esophageal cancer was performed clinically and histo-phthologically. RESULT: Nineteen patients had developed recurrece during the 18 to 52 months(mean 34.2 nonths), 8 had local recurrences, 1 had both, and 11 had systemic recurrences. Twelve patients(63%) had developed recurrence within 1 year, 5 patients(26%) between 1 year to 2 patients(11%) after 2 years. The recurrence rate according to growth pattern of tumor or presence of microinvasive findings was not statistically significant, but it increased significantly in clinical tumor stage III than stage IIA, B and in patients with the number of metastatic lymph node over ten. CONCLUSION: Post-operative recurrences of esophageal cancer appear as a high rate even though curative wide resection was done. Several clinical and histo-pathological factors correlate with the recurrence.

6.Pedal Indirect Lymphangiography.

PURPOSE: Recently, indirect lymphangiography has been developed as a relatively good and noninvasive imaging modality of the lymphatic system at extremities. But the disadvantage of the indirect lymphangiography is a low contrast ratio between the surrounding tissues and the contrast media in lymphatic vessels, because dimeric nonionic contrast media is water soluble and diluted in the proximal leg lymphatic vessels. We could have relatively better image than previously published images for the leg lymphatic system, when we injected contrast media with adequate high pressure in intradermal space of the interdigital areas at the foot dotsum. So, we would like to report the results. MATERIALS AND METHODS: We could study all 9 lymphedemas(primary :6, secondary: 3) from April 1990 to May 1993 on outpatient base. They were diagnosed as lymphedema clinically and radiologically. Ten ml of dimeric nonionic aget, iotrolan(Isovist 300 ) was injected into intradermal space with five 30-gauge needles The injection speed was more than 0.2mi/min. We have done one side pedal lymphangiogram in 30 minutes. The evaluation of the anterior superficial lymphatics was accordig to the criteria of the Weissleder(2). RESULTS: The results were as follows:1. All lymphatic vessels from foot to inguinal area could be visualized. 2. Two or three inferior inguinal lymph nodes could be visualized about 42%. 3. The most common abnormal finding of the lymphedma was the neovascularization of the lymphatics on indirect pedal lymphangiogram. CONCLUSION: If we use adequate technique relatively high pressure injection, correct intradermal needle insertion, adequate soft tissue exposure technique indirect lymphangiography is considered to be a safe and noninvasive imaging modality for the evaluation of the lymphedema of lower extremity lymphatics including inferior inguinal lymph nodes.

Two cases of cutaneous sporotrichosis, especially one of which was verrucous type in baby aged 4 months, were described. Throush the literatures the verrucous manifestation in baby was not yet reported. It is known that verrucous sporotrichosis is to be developed in individual who has had local and general resistance obtained by the previous cutaneous infection or by the infection of internal organ. But no such evidences were noticed in our case.

10.A Case perifolliculitis Capitis Abscedens et Suffodiens.

A case of 24 years old male with perifolliculitis capitis abscedens et suffodiens was reported This is, I presume the first case to be reparted in Korea. Clinically, the skin lesions were characterized by tbe multiple confluented indolent abscesses and hypertrophic scars which were developed on the vertex. On these lesions, there were also noticed complete alopecia and several dilated hair follicle orifices plugged by keratinous materials. The regional lymphnodes were not enlarged and the abscesses were proved bacteriologically sterile.
