2.The Clinical Review of Epithelial Tumors in the Lacrimal Gland.

We retrospectively reviewed clinical and radiologic findings of 19 patients who had been diagnosed as epithelial tumors of the lacrimal gland histopathologically after biopsy and surgery at Shinchon Severance Hospital from April, 1991 to July, 1998 and evaluated their correlation to the pathologic diagnosis, treatment and prognosis. There were 7 men and 12 women with the mean age of 41.0+/-13.3 years. Histopathologically, there were 7 cases of adenoid cystic carcinoma, 1 case of adenocarcinoma, 1 case of malignant mixed tumor, 9 cases of pleomorphic adenoma, and 1 case of benign oncocytoma. In comparison with benign epithelial tumor of the lacrimal gland, the characteristic clinical findings of malignant epithelial tumor of the lacrimal gland were pain, limitation of motion, and diplopia[44.4%]. The mean duration of the symptoms of malignant epithelial tumor was shorter[10.3+/-6.6 months] than that of benign epithelial tumor[19.5+/-14.4 months]. In radiologic CT and MRI findings, there were characteristic bony destructions in malignant epithelial tumors of the lacrimal gland[55.6%] compared with benign tumors and poorly marginated outline of the tumor was noted in malignant tumors[44.4%] but not in benign tumors. Therefore, clinical and radiologic findings of epithelial tumors of the lacrimal gland were well correlated with the pathologic diagnosis and would attribute to early diagnosis of malignant tumors.

3.Comparison of Hemodynamic and Oxygen Availability after Apnea between a Case with Anesthesia and a Case without Anesthesia.

BACKGROUND: Acutely induced hypoxia causes an increase in the mean arterial pressure, cardiac output and oxygen consumption. However, comparisons of hemodynamic changes and oxygen consumption between subjects with and without anesthesia are rare. The purpose of this study was to examine and compare the hemodynamic changes and oxygen availability after acute apnea between the dogs with and without anesthesia. METHODS: Apnea was induced to 19 healthy mongrel dogs. Group 1 (N=10) constituted dogs with anesthesia and group 2 (N=9) constituted dogs without anesthesia. Hemodynamic data and oxygen levels were rapidly measured with 1 minute intervals. RESULTS: The survival time of group 1 was longer than that of group 2. The mean arterial pressure increased in group 1. Although the cardiac outputs in both groups increased at the same time course in early phase of apnea, the preload values increased more rapidly in group 1. Systemic vascular resistance decreased at 2 and 3 minutes of apnea in group 1 but not in group 2. The oxygen extraction ratio increased significantly at 4 and 5 minutes of apnea in group 2. CONCLUSION: In this study the dogs with anesthesia had a prolonged survival time compared to the dogs without anesthesia. Most hemodynamic values did not differ significantly between the two groups, except for an increased mean arterial pressure and decreased systemic vascular resistance in the dogs with anesthesia. It was suggested that the cause of early death in the dogs without anesthesia was decreased oxygen delivery.

5.Coexisting Adenocarcinoma and Epithelioid Leiomyosarcoma of Stomach: A case report.

We report a case of malignant tumor of the stomach showing both carcinoma and sarcoma components in a 47 year-old woman. The resected stomach revealed two tumors which were in different location. The one mass was a 2 x I cm, poorly demarcated tumor at the body, along the lesser curvature and the other was a 6 x 7 cm, well demarcated polypoid tumor with a central depression at cardia of the posterior stomach wall. The tumor at the body was a poorly differentiated adenocarcinoma which showed strong reactivity for CEA. The tumor at the cardia was composed of epithelioid or spindle shaped cells with abundant eosinophilic cytoplasm and revealed 5-6 mitoses per ten high power field. The tumor at the cardia was diagnosed as epithelioid leiomyosarcoma and showed reactivity for vimentin but none for actin, desmin,keratin, and CEA. This is different from the usual carcinosarcoma.

7.A Clinical Study on the Incompetent Internal Os of the Cervix.

This study was undertaken for the clinical analysis and evaluation on 121 patients with incompetent internal os of the cervix, who were admitted and treated with McDonald operation or Shirodkar operation at the Soonchounhyang Medical Center from January 1991 to December 1995. The results of this study were as follows : 1. The incidence of this IIOC was 1.1% of 11,116 cases of total delivery. 2. The mean age of IIOC was 31.7 years old. 3. The average number of gravida was 3.2. 4. The most common contributary factor was previous history of artificial abortion (51.2 %), and midtrimester abortion (17.4 %), cervical laceration due to previous vaginal delivery (8.3 %) etc. was followed. 5. The success rate of operation was 76 %, and the highest success rate (85.7 %) was reveald with period from 15th weeks to 16th weeks of gestation. 6. When cervical dilatation was abscent or small, the success rate of operation was high. 7. The factors of failed operation were preterm labor (58.7 %), PROM (34.5 %), and PIH, bleeding. 8. The delivery method after operation was vaginal delivery in 83 cases (68.6%) and cesarean section in 38 cases (31.4 %).

8.The Level of Tuffier's Line in Korean Adults.

BACKGROUND: Tuffier's line, which connect the two iliac crests, is the often used landmark in determining the level for the needle insertion into lumbar intervertebral space for the spinal and/or epidural blocks. However, the level of Tuffier's line varies depending on the observer. METHODS: Two methods were used to measure the level of Tuffier's line was measured on a simple abdominal X-ray taken in the supine position (study 1). Second, the level of Tuffier's line was also measured on fluroscopy of students in the sitting position (study 2). RESULTS: Overall, the level of Tuffier's line was most frequently observed along the L4-5 intervertebral space and second most frequently observed along the lower one-third of L4. In study 1 using abdominal X-ray, the level of Tuffiers line was most frequently observed along the L4-5 intervertebral space, however, the second most frequently observed level varied depending on the ages of the volunteers. In study 2 using fluoroscopy, the level of Tuffier's line was most frequently observed along the L4-5 intervertebral space and second most frequently observed along the upper one-third of L5. CONCLUSIONS: The level of Tuffier's line was most frequently observed along the L4-5 intervertebral space both in the supine and sitting positions. Further, the level of Tuffier's line went higher with age for women but stayed constant independent of age for men in the supine position.

9.Hemodynamic Responses to Dobutamine, Hydralazine and Sodium Nitroprusside Following Pentastarch Infusion during Cardiac Tamponade in Dogs.

Background: Cardiac tamponade results in a hemodynamic disorder associated with decreased cardiac output and blood pressure. To improve cardiac output in a subject with cardiac tamponade, cardiotonic drugs and vasodilators with blood volume expander can be used. The purpose of this study was to observe the hemodynamic effects of cardiotonic drugs and vasodilators following administration of plasma expander in the dogs with cardiac tamponade. Method: Three groups of dogs were studied during the induced cardiac tamponade. Following infusion of pentastarch, group I received dobutamine by dripping of 10 microg/kg/min, followed by injection of 20 microg/kg/min, group II received hydralazine (20 mg, 40 mg) and group III received sodium nitroprusside (5 microg/kg/min, 10 microg/kg/min). The heart rate, blood pressure, cardiac output and pulmonary arterial occluded pressure were measured. The atrial transmural pressure was calculated by subtracting intrapericardial pressure from mean atrial pressure. Results: Cardiac output was increased in the groups I and II, but mean arterial pressure was increased in only the group I. Atrial transmural pressure was not changed in all three groups. Conclusion: The most pronounced hemodynamic improvements during the cardiac tamponade is observed in group I with pentastarch-dobutamine combination.
