1.Relationship between Union of Grafted Autologous Bone and Clinical Results of Operative Treatment of Degenerative Spondylolisthesis by Posterolateral Fusion.

PURPOSE: Bone graft is essential for successful spinal fusion. So, we clinically assessed the effect of uniting grafted autologous bone. MATERIALS AND METHODS: Retrospective analysis is performed on 46 patients who had grade I or II, one segmental, degenerative spondyloiisthesis according to Meyerding classification and treated operatively by posterolateral fusion with posterior decompression and autologous iliac bone graft from January 1991 to June 1996. We got the data from simple anteroposterior, lateral, flexion- extension X-ray film at preoperative, postoperative and last follow-up period, and from clinical results at last OPD follow-up according to Kirkaldy-Willis criteria. We compared the union of grafted autologous bone with clinical results using X2-test. We also compared preoperative spinal instability, with/without instrumentation and intraoperative reduction with the union of grafted autologous bane and clinical results. RESULTS: There was high significant correlation (P=0.000) between the union of grafted autologous bone and clinical results statistically, CONCLUSIONS: This results suggest that the union of grafted autologous bone was an important factor in determing clinical results.

2.Cementless Bipolar Hemiarthroplasty for Treating Intertrochanteric Fracture in Elderly Patients.

PURPOSE: To evaluate clinicoradiological outcomes after cementless bipoloar hemiarthroplasty in elderly patients with femoral intertrochanteric fractures. MATERIALS AND METHODS: From March 2006 to February 2008, 28 patients-all in patients greater than 80 years of age, classified unstable intertrochanteric fractures in Evans classification-were followed for more than 1 year. 24 patients were women and 4 patients were men. The mean age of the patients was 84.6 years, the mean follow-up period was 16.3 months. Harris hip score, postoperative inguinal and thigh pain, Parker and Palmer mobility score were analyzed clinically. The radiological results were assessed using various radiological indicies including bone-union, fit and alignment change of femoral stem and vertical subsidence. RESULTS: The average Harris hip score was 82.9, Parker and Palmer mobility score preoperative 8.0 changed to 5.2 postoperatively. More than moderate pain was presented in 1 case. There were no cases of varus deformity or osteolysis. All stems were stable without significant alignment change or subsidence except 1 case of periprosthetic fracture. There were no dislocation, thromboembolism, death during operation or hospital days. CONCLUSION: In elderly patients, cementless bipolar hemiarthroplasty is good treatment method of unstable intertrochanteric fracture and short-term clinicoradiological outcomes proved to be satisfactory.

3.Comparative Estimation of Coverage between National Immunization Program Vaccines and Non-NIP Vaccines in Korea.

This study aimed to describe the differences in vaccination coverage between National Immunization Program (NIP) vaccines and non-NIP vaccines in Korea and to identify factors affecting the difference. Nationwide face-to-face interview-based questionnaire survey among randomly selected 4,374 participants aged 7-83 months was conducted. Vaccination coverage analyzed according to the birth cohorts, geographic areas, and socio-demographic characteristics. We found that NIP vaccines recorded higher primary vaccination coverage compared to non-NIP vaccines (95.9%-100% vs 30.7%-85.4%). The highest rate was Haemophilus influenzae type b (Hib) vaccine (85.4%), which was introduced in 1996, and the lowest rate was rotavirus vaccine (30.7%), which was introduced recently. On multivariate analysis, having a sibling were significantly associated with lower uptake of Hib vaccine, pneumococcal conjugate vaccine (PCV), and rotavirus vaccine; while, older mother's age and attendance to daycare center were significantly associated with lower uptake of PCV and rotavirus vaccine (P < 0.001). We found differences in the vaccine coverage rate between NIP vaccines and non-NIP vaccines; and the data suggests potential disparity in accessing non-NIP vaccines in Korea. Expansion of NIP to include non-NIP vaccines can provide better protection against the diseases through increased coverage.

4.Time-Lag between Subretinal Fluid and Pigment Epithelial Detachment Reduction after Polypoidal Choroidal Vasculopathy Treatment.

PURPOSE: The goal of the present research was to study post-treatment changes in polypoidal choroidal vasculopathy (PCV) shown by optical coherence tomography (OCT). METHODS: The study included 12 patients with naive PCV. Photodynamic therapy and 3 consecutive intravitreal bevacizumab injections at 6-week intervals were given. Best corrected visual acuity, subretinal fluid (SRF), pigment epithelium detachment (PED), central macular thickness (CMT), and total macular volume (TMV) were measured before and after treatment as assessed by Stratus OCT3. RESULTS: After treatment, the SRF height decreased earlier than the PED height. The SRF diameter decreased with statistical significance. However, the PED diameter did not show a statistically significant improvement, persisting at pre-treatment levels. Both CMT and TMV decreased significantly after treatment. CONCLUSIONS: After PCV treatment, SRF and PED stabilized, as shown by OCT. However, the PED treatment response was both delayed and refractory compared to the SRF response. The small change in post-treatment PED diameter may suggest the possibility of PCV recurrence.

5.Macular Gradient Measurement in Myopic Posterior Staphyloma Using Optical Coherence Tomography.

PURPOSE: To evaluate clinical characteristics and the macular gradient in myopic posterior staphyloma with time domain (TD) optical coherence tomography (OCT). METHODS: Sixty-four staphyloma eyes of 40 patients were examined. Macular gradient (tangent theta) and the location of staphyloma were assessed with OCT imaging. The macular gradient was measured at points 1 mm and 2 mm distant from the fovea. The relationships of the macular gradient with age, axial length, and spherical equivalent were analyzed. RESULTS: In 8 eyes (12.5%), the bottoms of the staphylomas were in the fovea, and there was no macular gradient. However, in the other 56 eyes (87.5%), the bottoms of the staphylomas were not in the foveal area, and macular gradients existed. Staphylomas were commonly located in the infero-nasal retinal area. The mean macular gradient (tangent theta) was 0.26 +/- 0.08 at 1 mm distance from the fovea and 0.28 +/- 0.10 at 2 mm. No significant relationships were observed between macular gradient and axial length, patient age, or spherical equivalent. CONCLUSIONS: TD OCT reveals staphyloma location. If the location is outside of the fovea, a macular gradient exists and can be measured by OCT. Axial length measurement error may occur in eyes with poor visual fixation and steep macular gradients.

Radiographic and endoscopic procedures established the diagnosis of a rectourethral fistula. 8.Change of the Lordosis on Cervical Spine after Anterior Interbody Fusion with Autogenous Iliac Strut Bone Graft.

PURPOSE: Various methods have been used to treat traumas and diseases of the cervical vertebrae. For fractures and diseases of the cervical vertebrae, The authors of this paper sought to evaluate the effectiveness of these different methods by checking for the correction of kyphosis, seeing whether bone graft was successful, making clinical evaluation, and checking for the appropriateness of size and shape of the graft. METHODS: Of the 193 patients who visited the Department of Orthopedics at Chungnam National University Hopspital from January, 1997, to March, 2000, and had operative treatments using the anterior approach or the Smith-Robinson technique, 161 cases that were possible for follow-up were chosen for this study. Among the cases chosen, 111 cases had cervical trauma, and 50 cases had disease of the cervical vertebrae. Simple X-ray was taken to measure the curvature of the affected area and also of the overall cervical curvature(lower end of the second cervical vertebra-lower end of the seventh cervical vertebra), and the healing of the bone was evaluated. RESULTS: Normal range of overall cervical curvature is 16~38 degrees of kyphosis; the curvature after operative treatments for diseased cervical vertebrae was corrected back to the normal range from the reduced angles that existed before the operation. The bone graft was healed in all cases except one(cervical disease), and 15 cases showed prolonged healing. For clinical evaluation using the Robinson scale, 109 cases were excellent, 42 cases were good, and 10 cases were fair. CONCLUSION: Anterior interbody fusion operation of the cervical vertebrae using anterior cervical plate fixation on cases of cervical vertebrate damage and diseases performed, and for the bone graft, using grafts with the anterior surface longer than the posterior surface to induce kyphosis of the cervical vertebrae seemed to be effective. Thus, it is believed that above method is effective in inducing anatomical and functional recovery of patients with trauma and diseases of the cervical vertebrae.

Eighteen patients(19 affected hips and 17 unaffected hips) with Legg-Galvé-Perthes' Disease(LCP) were reviewed to evaluate the relationship between medial joint space widening and lateral subluxation of the femoral head. MRI was used to evaluate components in widened medial joint space in radiographs. There was increased cartilage thickness of the femoral head and acetabulum and increased joint fluid which represented synovial hypertrophy in the widened medial joint space in radiographs. Increased cartilage thickness was found at the medial aspect of the femoral head and at the lateral and posterior walls of the acetabulum when compared to unaffected hips and normal control hips. Widening of the medial joing space was related to lateral subluxation of the femoral head during the fragmentation or remodeling stage, not always during the avascular stage, in LCP. Widening of the medial joint space did not change remarkably after a Salter osteotomy or femoral varus derotational osteotomy because of the remaining cartilage thickness.
