1.The Effect of Subconjunctival Injection of Liposome Encapsulated Cytarabine on Proliferation of Fibroblasts.

To know the effect of subconjunctival injection of liposome encapsulated cytarabine on proliferation of conjunctival fibroblasts, the conjunctiva was isolated at 180 degrees from the injection site 3 days after subconjunctival injection of the normal saline (control), cytarabine, liposome encapsulated cytarabine, and 1 day after injection of cytarabine, and then those were inoculated in the culture media of fibroblasts. In the case of 3 days after injection of cytarabine, there was 49% and 42% inhibition of proliferation of conjunctival fibroblasts compared with the control respectively. Therefore, the authors concluded that the liposome encapsulated cytarabine is effective on inhibition of proliferation of conjunctival fibroblasts and reduces the frequencies of subconjunctival injection compared with the cytarabine itself.

2.Hyperkeratosis of the nipple and areola.

Hyperkeratosis of the nipple and areola is a rare condition. We report a case of hyperkeratosis of the nipple and areola occurrinp in man with no underlying endocrirop thy or synthetic estrogenic drug therapy. The patient was not associated with epidermal nevus orichthyosis and so fitted into the nevoid form of hyperkeratosis of the nipple and areola. The clinical appearance and histological findings of the lesion were the same as those reported before. He showed a slight improvement in the color and hyperkeratcsis of the nipple and areola with 6 months use of a topical keratolytic agent and corticosteroid crearn.

3.A Case of Molluscum Contagiosum Treated with Diphenylcyclopropenone Immunotherapy.

We are reporting a case of molluscum contagiosum in a 3-year-old boy who showed a good response to diphenylcyclopropenone (DPCP) immunotherapy. The lesions were multiple, 2 to 7mm in size, centrally umbilicated, normal skin-colored papules of 7 months' duration which were distributed on the penoscrotal area, extremities and trunk. In spite of several treatments using extraction and curettage, new lesions developed continuously. The patient was sensitized with 0.1ml of 1% DPCP solution in acetone on his right shoulder and there-after challenged with solution of varying concentrations (0.05 to 0.1%) on his left shoulder once or twice a week to maintain a mild eczema. Clinical improvement was noted 1 week after sensitization and almost all of the lesions cleared after 8 week's treatment.

4.A Case of Mycosis Fungoides Treated with Recombinant Interferon - alpha and PUVA.

We report a case of mycosis fungoides in a 28 year-old female patient who had multiple, bizzare, kidney or annular shaped, erythematous to brownishaly patches and plaques on the trunk, buttock, upper and lower extremities. Histopathologic finclingm showed epidermotropism and Pautrir microabscess in the epidermis and sssatypical lymphocytes infiltration in the dermis. T cell marker in leukocyte common antigen were positively stained in an immunohistochemical study. The skin lesions were reproved markedly after treatment it a total dose of 45 x 10 IU recombinant interferonaiv combination with a total dose of 385 iules/art PUVA.

5.Hemodynamic Responses and Oxygen Availability in Unanesthetized Dogs during Apnea.

BACKGROUND: This study examined hemodynamic variables, oxygen delivery, extraction, and consumption in response to acute progressive hypoxia and hypercarbia in the setting of apnea. METHODS: Apnea was induced in 9 healthy mongrel dogs by disconnecting animals from mechanical ventilation of 30 minutes with pure oxygen. Hemodynamic variables, oxygen transport, extraction, and consumption were rapidly and repeatedly measured using pulmonary arterial and arterial catheters until cardiac output was undetectable. RESULTS: The baseline PaO2, PaCO2, pH, base excess were 318 +/- 137 mm Hg, 36 +/- 3.5 mm Hg, 7.30 +/- 0.06, 6.81 +/- 2.65 mmol/l respectively. Hypercarbia and hypoxemia (76 +/- 33 mm Hg) was first noted at 1 and 4 minute respectively. Base excess was not changed. Indices of preload (PCWP and CVP) were increased early in the time course (p<0.05). In contrast, indices of afterload (SVR) increased later, just before cardiac decompensation began (p<0.05). No significant reduction of cardiac output, oxygen delivery, extraction, and consumption was detected just until abrupt cardiac decompensation started, 5 minute. CONCLUSIONS: These data suggest that the early increase in preload was primarily due to hypercarbia whereas the late increase in afterload was due to hypoxemia, but the main cause of acute cardiac decompensation was a critical decrease in arterial oxygen tension with some contribution of increased preload and afterload.

6.A Case of Hypereosinophilic Syndrome.

Hypereosinophilic syndrome represents a heterogenous group filisorders with the common features of idiopathic prolonged eosinophilia of undetectable cause and orgar, system dysfunction. We experienced a case af hypereosinophilic syndrome in a 58 year old man who has been suffered from fever, chilling, generelized exfoliative dermatitis, periorbital sngioedema and oral ulcer for 3 weeks duration. The blood eosinophil count was 7436/mm and skin, liver, bone marrow biopsy revealed marked infiltration of eosinophils. We treated him with co ticosteroid, but 5 months later, he died due to hepatoencephalopathy, hepatorenal syndrome, dissernimted intravascular coagulopathy and acute respiratory distress syndrome.

7.The Association between Unexplained Second-Trimester Human Chorionic Gonadotropin Elevations and Pregnancy Outcome.

We conducted this cohort analytic study to determine whether women with unexplained elevations of maternal serum hCG at 15-18 weeks' gestation are at increased risk for pregnancy complications and adverse perinatal outcomes. The inclusion criteria were a singleton gestation, a confirmed gestational age, and an hCG level greater than 2.0 multiples of the median (MoM). The exclusion criteria were fetal anomalies, an abnormal karyotype, molar pregnancy, and an MSAFP level greater than 2.5 multiples of the median (MoM). A group of randomly selected women with hCG levels under 2.0 MoM served as controls. Patients with elevated levels of hCG had a significantly higher risk for PIH (17.9% versus 4.5%; P <.05) and preterm delivery (17.9% versus 3.5%; P<, 05) than control. But no significant differences were observed in the incidence of intrauterine growth restriction and low birth weight and in the newborn weight. We suggested that pregnancies with unexplained elevated hCG levels should be regarded as high-risk pregnancies. And these patients require careful monitoring with adequate obstetric management.

8.VEGF Expression and Microvessel Density in Oral Squamous Cell Carcinomas.

Angiogenesis is an essential process in tumor growth and metastasis. VEGF has been considered a leading candidate inducing tumor angiogenesis. VEGF expression was significantly correlated with clinical stage, lymph node matastasis, and prognosis of cancers of various parts of body. However, little has been known about the correlation between VEGF expression and clinicopathologic parameters in oral squamous cell carcinoma. The aim of this study was to correlate VEGF expression with the clinicopathological parameters and microvessel density. Forty six oral squamous cell carcinomas were analyzed using immunohistochemical method with primary antibodies to VEGF and CD31. VEGF expression was detected in 33 (71.7%) of the 46 cases. The microvessel density was significantly correlated with VEGF expression (P=0.002). There was no correlation between microvessel density and tumour size, clinical stage, and lymph node metastasis, respectively. VEGF expression did not correlate with the histological grade of tumour differentiation, tumour size, and clinical stages. The VEGF-positive rate seemed to be higher in patients with cervical lymph nodal metastasis than in those without it, but it was not statistically significant. In conclusion, the overexpression of VEGF in the oral squamous cell carcinoma seemed to be associated with a more aggressive course of the disease. Further study is necessary to define the role of VEGF in oral squamous cell carcinoma.

9.Effects of Carbonated Water Intake on Constipation in Elderly Patients Following a Cerebrovascular Accident.

PURPOSE: This study was done to identify effects of carbonated water intake on constipation in elders who have experienced a cerebrovascular accident (CVA) and are bed-ridden. METHODS: Forty elderly patients with CVA were randomly assigned to one of two groups in a double-blind study. Patients in the experimental group drank carbonated water and those in the control group drank tap water for two weeks. Six patients dropped out during the study period. Data were analyzed by repeated measured ANCOVA and the covariance was the dose of laxatives used for the two weeks. RESULTS: Frequency of defecation increased significantly and symptoms of constipation decreased significantly for patients in the experimental group. CONCLUSION: The study results suggest that the intake of carbonated water is an effective method for the intervention of constipation in elderly patients with CVA.
