6.Clinical Study and Therapeutic Experience in Tinea Versicolor ( 3 ).

BACKGROUND: Tinea versicolor may be treated by topical oroal antifungal drugs. OBJECTIVE: Ihe objertive of this study was to evaluate the their, peutic effect and safety of oral itraconazole(100-200mg/day), cloconazole cream, 20% sodium chiosulfate solution, sulconazole cream and 2% ketoconazole shampoo in Tinea versicolor. METHODS: Forty patients with Tinea versicolor were studys clinically and treated in five groups, with oral itraconazxale(100-200mg/day), cloconazole crean 20% sodium thiosulfate solution, sulconazole cream and 2% ketoconazole shampoo, from JuIIy 1991 to June, 1994. The writers also compared the results with the writers previous similar studies in 1986, 1990. RESULTS: The average age was 29.9 years, the oldest patient were 52 year-old and the youngest patient was a 14 year-old. The ratio of male to female a 1.2: 1. The distributions of lesions was most common in the axilla(26.9%), and the other sitsuere the anterior chest(19.2%), the back(17.3%), the neck(15.4%), the upper extremites(9.6%) the abdomen(7.7%) and the lower extremites(3.9%), in order. The incidence of hyperpigrietted lesions was 76.8% and of hypopigmented lesions was 23.2%. The average duration of treatment until cure was 3.00+0.55 weeks in the oral itraconaxile treated group, 3.31+1.18 weeks in the cloconazole cream group, 3. 25+1.03 weeks in the 20%, sodium thiosulfate solution group 3.45+0.52 weeks in the sulconazole cream group and 3.40+0.33 weeks in the 2% ketoconazole shapen group. Mere were no statistically significant differences of therapeutic efficacy in each the five groups(p>0.05). Compared with the study in 1990, the average age was higher from 5.7 to 29.9 years, and the male to female ratio was slightly less, showing a relative increase in male incidence. But, there was no statistical difference in therapeutic efficacy(p>0.05). CONCLUSION: This study was shown that the above five theraieric regimens can be used safely and simply, according to patients preferences.

7.Angiogenic factor-enriched platelet-rich plasma enhances in vivo bone formation around alloplastic graft material.

PURPOSE: Although most researchers agree that platelet-rich plasma (PRP) is a good source of autogenous growth factors, its effect on bone regeneration is still controversial. The purpose of this study was to evaluate whether increasing angiogenic factors in the human PRP to enhance new bone formation through rapid angiogenesis. MATERIAL AND METHODS: In vitro, the human platelets were activated with application of shear stress, 20 microgram/ml collagen, 2 mM CaCl2 and 10U thrombin/1 x 109 platelets. Level of vascular endothelial growth factor (VEGF) and platelet microparticle (PMP) in the activated platelets were checked. In the animal study, human angiogenic factors-enriched PRP was tested in 28 athymic rat's cranial critical bone defects with beta-TCP. Angiogenesis and osteogenesis were evaluated by laser Doppler perfusion imaging, histology, dual energy X-ray densinometry, and micro-computed tomography. RESULTS: In vitro, this human angiogenic factors-enriched PRP resulted in better cellular proliferation and osteogenic differentiation. In vivo, increasing angiogenic potential of the PRP showed significantly higher blood perfusion around the defect and enhanced new bone formation around acellular bone graft material. CONCLUSION: Angiogenic factor-enriched PRP leads to faster and more extensive new bone formation in the critical size bone defect. The results implicate that rapid angiogenesis in the initial healing period by PRP could be supposed as a way to overcome short term effect of the rapid angiogenesis.

9.Higher Cognitive Functions and Psychiatric Disorders.

Higher cognitive functions refer to the highest level of human intellectual functioning, including formation, reasoning, and executive functions. The executive functions can be conceptualized as having four components ; volition, planning, purposive action, and effective performance. Because higher cognitive functions represent the most advanced stages of intellectual development, they are often highly susceptible to the effects of brain injuries and mental disorders. The ability to perform effectively within the environment is determined in large part by an individual's adequacy in performing such higher-order function. Especially executive functions are necessary for appropriate, socially responsible, and effectively sell-serving adult conduct. Therefore, an assessment of the psychiatric patient's performance in these areas will provide useful diagnostic information, as well as information concerning social and vocational prognosis.

10.Surgical Treatments of the Acute Anterior Cruciate Ligament Injuries

Primary repair of anterior cruciate ligament is superior to late reconstruction procedures for two reasons; objective stability is more easily restored due to intact secondary restraints not subjected to chronic stress, and the quality of the joint is superior with greater chance of intact menisci and normal articular cartilage not yet damaged through recurrent subluxation. We present the result of thirty-eight kness in thirty-seven patients who had been treated with primary repair of augmented repair for acute ACL rupture from March, 1984 to August, 1990. Twenty-one knees were treated with primary repair and seventeen knees augmented repair. At an average 33 months follow up, the results obtained were as follows: 1. The patients were 23 males and 14 females with an average age of 27.1 years. 2. Twenty four knees were injuried during athletic endeavors, 9 knees by automobile accident, and 5 knees by landing from height. In 18 knees anterior cruciate ligament was ruptured at the proximal attachment, in 17 knees at the midsubstance tear and in 3 at the near distal attachment. 3. The average interval from injury to operation was 7.5 days (ranged from 1 to 14 days). 4. The results were evaluated by Hospital for Special Surgery knee rating scale (modified by Marshall) at an average 33 months (range from 13 months to 7 years and 6 months) follow-up study. 16 (76%) out of 21 knees treated with primary repair were rated as good or excellent. 12 (86%) out of 14 knees treated with augmented repair were rated as good or excellent. As the result of this study, acutely torn anterior cruciate ligaments appear to be successfully treated with primary repair or augmented repair according to torn site and shape of ligaments.
