2.CT characterization of bile duct dilatation: differential disgnosis of obstructive jaundice.

Each Disease affecting the bile ducts tends to produce characteristic pattern of billiary dilatation: recurrent pyogenic cholangitis causes dilatation and straightening of the larger(central) intrahepatic ducts ; clonorchiasis causes dilatation of the smaller (peripheral) intraahepatic ducts; and carcinoma along the extrahepatic ducts causes (proportional) dilatation and tortuosity of both larger and smaller intrahepatic ducts. To evaluate the specificity of the pattern and morphology of the dilated biliary tree on CT scancs (CT characterization) three independent radiologists who were unfamiliar with the cases were asked to classify 62 CT scans in patients with obstructive jaundice. The case population consisted of 14 cases with recurrent pyogenic cholangitis, 18 cases with clonorchiasis and 30 cases with carcinoma along the extrahepatic ducts, which were intermixed randomly. Classification was made only on the basis of CT characterization: those scans showing primary lesions i.t., stone, aggregate of flukes, or tumor mass were excluded or masked. All the scans of every case showing the extrahepatic bile duct were masked. Radiologists correctly classified 54 of the 62 cases (87%): ten of the 14 patients with recurrent pyogenic cholangitis(71%), 17 of the 18 patients with clonorchiasis(94%) and 27 of the 30 patients with carcinoma along the extrahepatic bile cucts(90%). We believe that CT characterization of bile duct dilatation is useful in the differential diagnosis of obstructive jaundice, especially when a primary pathologic lesion is not depicted in CT scans.

3.Radiological diagnosis of islet cell tumor: Percutaneous transhepatic portal venous blood sampling.

Two patients with clinical and biochemical evidences of islet cell hyperfunction underwent percutaneous transhepatic portal and pancreatic vein catheterization and hormone assays. Local step-ups of insulin level in the protal venous system, found in two patients with insulinoma, corresponded to tumor sites at surgery. One patients had single insulinoma and the other had malignant insulinoma and liver metastasis. Percutaneous transhepatic protal and pancreatic vein catheterization with measurement of radioimmunoactive insulin concentration is a safe and reliable method, and may play an important role in the localization of adenoma in patients with hyperinsulinism.

5.Treatment of Displaced Fractures of the Femoral Neck by Muscle-Pedicle Bone Graft and Internal Fixation

The major blood supply to the femoral head is probably damaged by the fracture of the femoral neck especially when it is a displaced fracture. Complications such as delayed or nonuion of the fracture and avascular necrosis of the femoral head are common. Revascularization after reduction and internal fixation appears to be a slow process and in many patients late segmental collapse occurs. Various techniques have been proposed to restore the blood supply or to improve the circulation to the femoral head. Hereby we performed internal fixation and an autogenous muscle-bone pedicle graft composed of the quadriceps femoris muscle in 3 cases of old, neglected displaced femoral neck fracture. The results seemed to be good in regard to bony union in all 3 cases but it would be needed further follow up study for developing of any avascular necrosis.

6.A Clinical Observation of the Pelvic Bone Fracture

62 cases of the fracture of the pelvis admitted to the orthpedic department of Wonju Christian Hospital during past 5 years from 1975 to 1979 were reviewed and clinical results were obtained as follows: 1. Males were frequent than females and almost 3rd to 4th decades of life. 2. The causes of injury were traffic accident, coal minor accident, and falling. 3. Stable type is the most common in this series and among the unstable types, lateral compression type is the most common and then straddle type and vertical shear type. 4. The most common complication is uro-genital injury. 5. The residual complications at the end of treatment remained in some cases, Arthritis of sacroiliac joint, Impotence, Urethral stricture, Low back pain, etc. 6. All of the cases were treated conservatively and their results were somewhat good except 4 cases. 7. Surgical treatment would be considered, if necessory, to decrease the complications and reduce hospital days.

8.A Case of Traumatic Spinal Epidural Hematoma in Ankylosing Spondylitis.

A case of traumatic thoracolumbar epidural hemorrhage is presented in a 37-year-old man with ankylosing spondylitis. The patient complained neck and back pain right after the accident but paralegia and difficult urination followed 2 days later. Spine CT showed thoracolumbar epidural mass, which was confirmed to be hemorrhage associated with spine fracture and was solely responsible for paraplegia.

9.A Case of Cerebral Candidal Granuloma.

Candidosis, the most common fungal infection involving the central nervous system, is rarely diagnosed during life. The authors present a case of candidal granuloma of the caudate area in a 31-year-old, otherwise healthy man with episode of intermittent antibiotic therapy. There was no extracranial fungal lesions, Ventriculoperitoneal shunt and biopsy followed by treatment with Amphotericin-B resulted in a favorable outcome.

10.Double Stapling Technique in Low Anterior Resection for Rectal Cancer.

The double stapling technique has become an established reconstruction method for patients undergoing low anterior resection. We have used a modification of the conventional technique in which the lower rectal segment is closed with a linear stapler (TA-55) and the anastomosis is performed by using the circular EEA(CEEA) instrument across the linear staple line of the double stapling technique. The aim of this study was to evaluate the prophylactic effect of a loop ileostomy preventing anastomotic leakage. Stapled colorectal anastomosis and stapled coloanal anastomosis in 60 patients forms the basis for the report. The sixty patients were treated by using the double stapling technique either with or without a loop ileostomy. This review presents the advantages and disadvantages of a loop ileostomy for coloanal anastomosis. Postoperative anastomotic leakage in the double stapling technique group occurred in 5 (10.6%)cases of the total 31 cases while in the double stapling technique with loop ileostomy group, it allowed 1(3.4%) of the total 29 cases. This study suggests that the double stapling technique with a loop ileostomy is more effective than the double stapling technique without a loop ileostomy in preventing anastomotic leakage. The addition of a loop ileostomy to protect the low anastomosis might also be expected to influence anastomotic healing.
