1.The Study of the Quantitation for Immune Globulins ( I & G. I & A and I & M ) in Syphilis.

The sera of 86 patients with untreated secondary syphilis were anaIyzed by immunoelectrophoresis with the specific antisyphilitic rabbit serum. Twenty-seven out of the 86 sera (31. 4%) showed more prominent IgA precipitate arcs as compared to the precipitate arcs of the normal serum. Fourteen out of the 86 scra (16. 3%) showed more promincnt IgM precipitate arcs as compared to the normal. However there was no speciific abnormal precipitate line for the syphilis serum, Quantitative analyscs of the 86 sera from the syphilitics by the Lim's modification of the Oudin capillary tube method showed that the mean of the IgG level is 2.415+53.2mg/100ml, IgA 293+8.3mg/100mI and Igm 102+3.6mg/100ml. This result represents a statistically significant increases of immune globulin levels in the syphilitics as compared with the normal controls (p<0.05).

2.Malignant Acanthosis Nigricans: Report of a Case.

One case of malignant acanthosis nigicans associated with gastric adenocarcinoma (scirrhous type) and papilloma on the oral mucosa is presented. The patient, 36 year old Korean male, visited S.N.U.H, complaining of sore-mouth and sore-tongue of 20 days duration. He had been affected with severe papillomatosis on the oral mucosa and with hyperkeratosis on the periorbital area, the perioral area, the neck, the axillae, the palms, the umbilical area, the inguinal area, the anogenital area and the soles which were moderately hyperpigmented with grayish to brown color. He had not been complaining of G-I troubles except sore-mouth and sore-tongue, but he was diagnosed as stomach carcinoma by gastroendoscopy, He got an operation of extended total gastrectomy with isoperistaltic jejunal interposition. Post-operative histopathologic examination rvealed well differentiated adenocarcinoma of the stomach which had already metastasized to the neighboring lymph nodes. After operation the papillomatosis on the oral mucosa and skin eruptions had been improved but becarne aggravated again one and half month after discharge.

3.A Case of Lupus Vulgaris on the Patella Region of the Right Knee Joint.

A case of lupus vulgaris who has the typical skin lesion on the patella region of the right knee was presented. The patient is 9 years old, the generally healthy male, and is no special finding of chest X-ray but tuberculin test was strong positive. The skin lesion started about 2 years ago as baby's palm sized erythematous patch with apple jelly nodule on the right knee joint region. Diagnosis was made by characteristic of clinical appearance and histulagial finding including other laboratory data. The treatment was done with INAH, PAS, streptomycin and pyridoxine for 3 months. Now,The skin lesion is almost healed and still continue treatment and observation go on.

4.Xepoderma Pigmentosum Complicated by Squamous Cell Epithelioma and its Treatment with Bleomycin.

Xeroderma pigmentosum, a rare, genetically determined dermatosis, is often cornplicated by malignant chagnes of various types-most commonly, basal-cell epithelioma, squamous cell epithelioma, less frequently malignant meIanoma, keratoacanthoma, fibrosarcoma, and endothelioma have been reported. In Korea, no case has ever been reported in literature except four cases at a symposium for dermatology, two of them complicated by basal-cell epithelioma, and the others by squamous cell epithelioma. This case reported was complicated by squamous cell epithelioma. As one of the anticancer treatments, especially for skin cancer, Bleomycin is, now, drawing the attention for its high therapeutic index with only few side effects. For this case, we injected Rleomycin 15mg at once, twice a week, for 8 weeks, So total injection was 16 times and 240mg of Bleomycin was given in total. While this amount given for this case was not enough for the complete treatment of squamous cell epithelioma, we got the following results: 1. Clinically, the ulcerated lesion of the cancer on the right ala nasi had shrunken in size with reepithelialization at the margin showing clear surface, and 2. microscopically, the atypical cancer cell masses and individual cell keratinizations had been decreased in size and in number, and less remarkable comparing to those in untreated stage at his first visit to the hospital.

5.A Case of Hemolyic Disease of Newborn Caused by Anti-c and Anti-E Antibody.

Hemolytic disease of newborn due to Rhesus incompatibility occurs mostly by anti-Rh(D) antibodies. But recently, because of Rh(D) immune globuin prophylaxis, Rh(D) incompatibility is decreased and importance of minor group incompatibility is increasing. The majority of minor blood group incompatibilities are anti-c, anti-E or anti-Kell, but hemolytic diseases caused by combination of these antibodies are relatively rare. The 21 day-old male neonate was admitted because of pallor and poor feeding for 2 days. Laboratory data showed severe normochromic and normocytic anemia with increased reticulocyte and bilirubin. Patient's and his mothers blood type was B, Rh(D) positive. Minor blood groups of mother's and sons were different and anti-c and anti-E antibody were detected in mother's serum. After transfusions of two times with anti-c and anti-E free B, Rh(D) positive blood, the baby was discharged at 11th hospital day with good general condition.

6.An Epidemiological Evaluation on the Sexual Partners of Male Patients with Sexually Transmitted Diseases.

BACKGROUND: In the management of patients with sexually transmitted diseases(STD), contact tracing is fundamental to get the information of sexual partners to be responsible of STD transmission. OBJECTIVE: The aim of this study was to obtain statistical data of the sexual partners for more effective STD control. METHOD: In order to obtain statistical knowledge of the sexual partners, we retrospectively reviewed medical records of 11,333 male patients who visited the Venereal Disease Clinic of Choong-Ku Public Health Center in Seoul from January 1986 to December 1995. RESULTS: As the source of STD transmission for 11,333 male patients, the so called 'high risk groups' comprised 77.6% including prostitutes 38.3%, bar girl 18.9% and call girl 20.6%. In other groups, the so called 'low risk groups' are as follows; girl friends 10.4%, acquaintance 7.6% and wife 3.5%. In regard to prevalence of STD, nongonococcal urethritis and gonorrhea comprised 46.4% and 42.9%, respectively while chancroid, syphilis, herpes progenitalis and condyloma acuminatum were 2.2%, 0.9%, 0.9% and 1.0% respectively. CONCLUSION: We obtained the epidemiological data of the sexual partners of patients with STD. This will help us make STD prevention strategies.

7.A Clinical and Pathological Study of Recurrent Hematuria in Childhood.

The clinico-pathological observation was done on 28 children with recurrent hematuria, who had been admitted to the Pediatric ward of NMC from January 1981 to July 1991. The results were as follows; 1) Most of the children with recurrent hematuria were over 6 years of age(24 cases, 85.7%)and the sex ratio was about 2.5:1. 2) IgA nephropathy (17 cases, 60.7%) was the leading pathologic lesion and mild focal nonspecific glomerulonephritis (5 cases), membranous nephropathy (2 cases), minimal chage (2 cases), Alport syndrome (1case), abnormal glomerular basement membrane (1 case) followed. 3) Types of recurrent hematuria ware gross in 22 cases and microscopic in 6 cases. Flank pain was noted only 1 case. The prodromal events of hematuria were upper respiratory infection in 15 cases and severe exercise in 2 cases. 4) There were no significant differences in clinical features and laboratory findings between IgA nephropathy and other pathologic lesions, except for higher frequency of proteinuria in former group. 5) During the follow-up periods, three cases were resulted in chronic renal failure and they were two cases of IgA nephropathy Class IV and one case of Alport syndrome. And one case of IgA nephropathy was resulted in nephrotic syndrom.

8.Effect of Cognitive-Behavioral Therapy on the Delusion in Schizophrenic Patients.

OBJECTIVE: The effect of cognitive-behavioral therapy on the delusion in schizophrenic patients was evaluated. METHOD: The patients admitted to a psychiatric ward from September 1999 to June 2000 and diagnosed as schizophrenia, schizophreniform disorder, and schizoaffective disorder by DSM-IV were randomly assigned to cognitive-behavioral therapy(CBT) group(n=9) and supportive psychotherapy (ST) group(n=8). During the 10 weeks' treatment period, conviction, preoccupation, and anxiety on delusion, mode of explanation about symptom, and recovery style were regularly measured and compared between groups. RESULTS: 1) As for conviction and anxiety on delusion, the patients from both groups showed gradual reduction over time, but there was no significant difference between groups. 2) As for preoccupation of delusion, patients from both groups showed gradual reduction over time, and the patients on CBT group had a significantly more reduction than ST group. 3) As for mode of explanation and recovery style, CBT group had more marked positive changes than ST group did. CONCLUSION: Cognitive-behavioral therapy is more effective on preoccupation of delusion, explanatory mode about symptom, and recovery style than supportive psychotherapy.

9.Clinical Reseach for Sport Injures of Korean Disabled Athletes in Seoul Paralympics

We Studied the sport injuries of 135 Korean dissbled athletes who were going to Seoul Paralympics from 15th September 1988 to 30th September 1988. The results were as follows.1. The mean age was 23.3years, male and female ratio was 7 to l. 2. The type of disability were as follows ; Poliomyelitis, Cerebral Palsy and Blindness and transition rate of below 5 years of age was 71.6%. 3. The most common age of begining exercise of the disabled athletes were 16-20 years and composed of 77.4%. 4. The most common cause and process of athletic injuries were heavy training and excessive exercise. 5. The most common athletic injury was soft tissue injury and the most common site was hand and the most common type of injury was blindness and the most common causative sport was baseball and fencing. 6. The protects were used in 41% of athletes and they were globes and glassed as follows. 7. Over 77% of the disabled athletes were answered as “Necessary” to the question about scientification of disabled sport.

10.Experimental Study of Microvascular Anastomosis Using Biological Fibrin Glue.

Anastomosis of rat common carotid artery was performed by applying four crossed-fixing sutures and by mantling with human fibrinogen-thrombin glue. There was a 97% patency rate 24 hours after surgery, and the anastomosed vessels were still patent on the 7th and 30th postoperative days. Aneurysm formation was absent but stenotic change was seen to 1 case of 8 angiographic studies. Histopathological studies of microvascular anastomosis showed that this surgical technique of anastomosis has certain advantages, namely that it reduces the number of sutures needed, is less traumatic to the vascular wall, ensures elastic vascular junction, and the glue used possess high biological inertness that provides a better physiological connection of vessels than conventional suturing. The duration of the clipping and the operation is significantly reduced. A tensile strength test 24 hours following operation and 1 week later showed that this anastomosis was significantly better than that achieved with the usual manual suture method. The author therefore concludes that this new surgical technique of microvascular anastomosis may be valuable in microvascular neurosurgery.
