1.Study on the Healing and Correction of Rachitic Leg Deformity

Eighty three rickets could be diagnosed at our department during eight years from August of 1969 to July of 1977. Among them fourteen children, whose radiographs and clinical description were precise with well-documented follow-up study of more than one year, were subjected to this analysis. Evaluation of the healing and correction of the lower leg deformity in these rachitic children obtained the following results. 1. Among fourteen rachitic children, thirteen patients (26 legs) showed varus and one patient (2 legs) showed valgus deformity. 2. Mean mechanical tibio-femoral angle showed shift from the initial 16.1°±5.3° to 5.5°+4.0° at final visit, which indicated the gradual correction of the deformity with the advance of the ages of children. The follow-up study after three years in this series showed that the correction became almost complete. 3. At the epiphyseo-metaphyseal region of the distal femur, the initial angle showed two distinct trends of change. The initial varus angle more than 7 or 8 showed the decrease at the later follow-up study, while the initial angle less than 7 or 8 showed the increase. 4. The epiphyseo-metaphyseal region of the proximal tibia showed the minimum change of angle. Mean angle of deviation from the vertical line was 3.7°±2.9° at first visit and 2.6°± 2.4°at final visit. The least degree of varus and the trend toward valgus observed in this region may refiect its compensatory activity against the varus deformity of the leg as a whole. 5. The epiphyseo-metaphyseal region of the distal tibia showed the maximum degree of varus at both initial and final check although the greatest degree of correction was attained. The initial mean varus angle of 23.9°±9.2° became 12.9°±7.1° at final visit. 6. The results of this study have shown that the correct evaluation of the rcchitic leg deformity is possible with measurement of the mechanical tibio-femoral angle and that the deviation angle of each epipyseo-metaphyseal region is preferable.

3.Dystrophic Calcification Following Anterior Tibial Compartment Syndrome

Usually the histologic response of muscle to the ischemia range from mild, reversible change to extensive necrosis and fibrosis in the case of anterior tibial compartment syndrome. But dystmphic calcification in the late stages is very rare condition.Only three cases were previously reported by Gallie and Broder et al in the literature. We are reporting six cases of dystrophic calcification following anterior tibial compartment syndrome in late stages. Onsets of dystrophic calcifications after original injuries ranged from twelve years to thirty-two years. Two out of six cases revealed painless maas in the anterior tibial compartment and remaining four cases revealed painful aases. At surgery, tooth-paste like, calcified material was evacuated in one of five operated cases and yellowish grey, brittle materials were evacuated in the remaining four cases.

4.Immunohistochemical Study of the Ligamentum Flavum in the Lumbar Spinal Stenosis

Lumbar ligimentum flava obtained from 21 patients of spinal stenosis and 10 patients who underwent surgery for vertebral fracture(control group) were studied with hematoxylin-eosin stain, Verhoff s elastic stain, and immunohistochemical stains for S-100 protein ad fibronectin. The chondroid metaplasia of ligamentum flavum found more frequently and widely in the group of spinal stenosis compared with control group. The elastic fibers were decreased in number and irregu- larly arranged at the bone attached area in the group of spinal stenosis. There was tendency to increase the amount of fibronectin in the stroma of the ligamentum flavum in the group of spinal stenosis. Above findings suggest that chondroid metaplasia and changes of the elastic fiber in the peripheral area of the ligamentum flavum are able to contribute the pathogenesis of spinal stenosis.

5.Results of Treatment of Endophthalmitis with Ciprofloxacin Mixed Infusion Fluid.

PURPOSE: To investigate the effect of ciprofloxacin on the management of endophthalmitis, the anterior chamber irrigation or the vitrectomy with infusion fluid containing ciprofloxacin was performed. METHODS: We analyzed the records of 31 eyes of 31 patients who had been treated for endophthalmitis. Twenty cases had developed after cataract surgery, 3 cases were caused by infection of filtering bleb after trabeculectomy and 8 cases were following perforating ocular trauma. Culture and smear were performed with specimens with from conjunctival sac secretion, aqueous humor and vitreous. Ciprofloxacin(5micro gram/ml) in infusion fluid was used for anterior chamber irrigation or vitrectomy. In addition, 200micro gram/0.1ml ciprofloxacin was injected into the vitreal cavity, when vitrectomy was not performed. RESULTS: Causative organisms were identified. Staphylococcus epidermidis was cultured in 6 cases, Pseudomonas aeruginosa in 4 cases in 13 out of 31 eyes (42%), Staphylococcus aureus, viridans group streptococci, Streptococcus uberis in 1 case each. Except one case of Streptococcus uberis, all cultured organisms were sensitive to ciprofloxacin. Final visual acuity of 0.1 or better was achieved in 22 of 31 eyes after treatment with ciprofloxacin. Complications included 3 cases of retinal detachment, 2 cases of bullous keratopathy, 1 case of secondary glaucoma, 1case of optic nerve atrophy and 1 case of corneal opacity CONCLUSIONS: Our result suggests that application of ciprofloxacin by mixing in infusion fluid may be effective for the management of bacterial endophthalmitis

Herpes zoster is a disease caused by the varicella zoster virus which affects the sensory nerves or peripheral branches of the cranial nerves. Segmental motor paresis, one of the complications of herpes zoster, is a rare complication which can occur two to three weeks after the skin rash develops and involves the corresponding muscle segments to the skin lesions caused by herpes zoster. We treated a patient with segmental zoster paresis who had a disorder similar to herpes zoster.

7.Desaturation of Pulse Oximetry by Intraoperative Using of Subcutaneous Blue Dye: A case report.

Pulse oximetry is a noninvasive, reusable device that is being used routinely in anesthesia procedure for monitoring oxygen saturation. This is a very beneficial device that can quickly detect of hypoxia, the most common cause of anesthesia-related death. However, in clinical uses, false desaturation readings are frequently found to be caused by various factors. Such as by the use of intraoperative blue dye, which can falsely decrease oxygen saturation as determined by pulse oximetry. We therefore report our findings concerning prolonged decreased pulse oximetry readings due to the intraoperative use of blue dye subcutaneously.

9.STA-MCA Anastomosis with Gradual Ligation of ICA in the Management of Aneurysm, Cavernous Portion.

An aneurysm in the intracavernous portion was treated with EIAB and gradual ligation of ICA. Carotid ligation historically has been successful in controlling the mass effects from an aneurysm and in minimizing the risk of an acute, recurrent SAH. However, ICA ligation alone carries a higher risk, esp, ischemic complication. In an attempt to minimize these ischemic complications, EIAB procedure was added to the ICA ligation for the treatment of inoperable or inaccessible aneurysm.

10.Association between the Urine Cotinine Level and Blood Pressure in Korean Adults with Secondhand Smoke Exposure: Korea National Health and Nutritional Examination Survey 2016–2018

Background:
Similar to smoking, exposure to secondhand smoke is a risk factor for developing hypertension and cardiovascular diseases; however, there is no standardized method for measuring smoke exposure. Measuring urine cotinine levels is one possible means for determining the degree of exposure to secondhand smoke. This study investigated the association between urinary cotinine levels and blood pressure in Korean adults exposed to secondhand smoke.
Methods:
Data from the Korea National Health and Nutrition Examination Survey conducted between 2016 and 2018 were used. A total of 9,273 participants aged ≥19 years self-reported as current non-smokers, which was cotinine- verified. A complex sample general linear model regression analysis was performed to analyze the association between urine cotinine and blood pressure. A P-value of <0.05 was considered statistically significant.
Results:
Corrected urine cotinine levels were positively associated with systolic and diastolic blood pressure in female participants (P<0.001 and P=0.040, respectively). Furthermore, a 10-fold increase in the corrected urine cotinine level of those in contact with secondhand smoke was independently associated with 2.085 mm Hg and 0.575 mm Hg increases in systolic and diastolic blood pressure, respectively. However, there was no association between systolic and diastolic blood pressure in male participants (P=0.226 and P=0.256, respectively).
Conclusion
Urinary cotinine levels were positively associated with increased blood pressure in females exposed to secondhand smoke. Therefore, urinary cotinine may be used as an indicator to quantify and monitor the effects of blood pressure elevation in females exposed to secondhand smoke.
