1.A study of artifacts in MR imaging induced by metalic aneurysm clips.

To investigate MR artifacts induced by metallic implants, scans were obtained using both the ferromagnetic Drake lip and the non ferromagnetic Yasargil clip. Scan were taken through the area of clips using geometrical phantom. The MRI was performed by spin echo technique and gradient echo technique on both 2.0T and 0.5T MR unit. The luthors evaluated the nature and differences of artifacts in each sequence and parameter. Artifacts induced by both lips were noted in the direction of frequency encoding gradient, and consisted of region of signal loss abutted in one side by survilinear region of bright signal. Geometric distortion of image was marked in the Drake clip, out was minimal in the Yasargil clip under both 2.0T and 0.5T. Artifacts were more pronounced in the gradient echo technique than those of the spin echo technique on both 2.0T and 0.5T. Although there were no differences n the nature of artifacts induced by the Drake clip among each parameter in the spin echo technique under 2.0T, artifacts were slightly more pronounced on T2 weighted image under the 0.5T field, but no differences were found in the nature of artifacts induced by the Yasargil clip in the spin echo technique under, both the 2.0T and 0.5T fields. Marked artifacts were induced through the small area of the Drake clip which were included in the scan plane, but induced artifacts were small when a small area of the Yasargil clip was included in the scan plane. It seemed likely hat artifacts were slightly more pronounced in the 2.0T than the 0.5T field in both clips, but objective evaluation of the difference was difficult, In conclusion, these results can be an essential basis for the interpretation of MR images or patents with metallic inplants.

3.Clinical application of absolute ethanol as an embolizing material

Transcatheter infusion of absolute ethanol was applied clinically in 3 cases of artificial embolization; 25yrsold female with bilateral renal angiomyolipoma, 19yrs old female with right paralumbar liposarcoma and 25 yrs oldmale with hypernephroma of right kidney. Selective or subselective manual infusion was made and 9 to 22cc of pureethanol was delivered in a speed of 1 to 2cc per second. The sclerosing effect of absolute ethanol was potent.However, all 3 cases revealed mild post-emboliaztion syndrome.

5.Diagnostic accuracy of beta-hCG discriminatory zone and vaginal ultrasound in abnormal early pregnancy.

OBJECTIVE: To evaluate the accuracy of combined transvaginal ultrasound and beta-hCG discriminatory zone for diagnosing intrauterine pregnancy, abortion, and ectopic pregnancy in early abnormal pregnancy. METHODS: Initial ultrasound findings and beta-hCG level were compared with final pregnancy outcome in 164 early pregnant women who visit our hospital with vaginal bleeding or abdominal pain. The sensitivity, specificity, and predictive value by the combination of two diagnostic tools were calculated. Statistic analysis of collected data used x2 of SPSS(9.0). RESULTS: Of 90 women with normal outcome, 64(71.1%) had a gestational sac 5mm, and in 47 cases, the hCG level was above 1,800 mIU/ml. Transvaginal ultrasound was non-diagnostic in 47(28.6%) of 164 women, and especially, 43(53.1%) of 81 cases with beta-hCG levels below 1,800 mIU/ml. The portion of accurate ultrasound diagnosis was significantly higher in women above 1,800 mIU/ml (85.5%, 71 of 83 cases) compard with levels below 1,800 mIU/ml (37.0%, 30 of 81 cases) : P < 0.001; Relative Risk(RR) 2.31; CI 95%. Sensitivity of transvaginal ultrasound diagnosis of intrauterine pregnancy, abortion, and ectopic pregnancy was 90.2%, 79.3%, and 66.7% in women who presented with beta-hCG levels above 1,800 mIU/ml, and 41.0%, 23.5%, and 75% below 1,800 mIU/ml, respectively. And, negative predictive value was 83.9%, 89.7%, and 98.8% in each of intrauterine pregnancy, abortion, and ectopic pregnancy above 1,800 mIU/ml, and 64.6%, 60%, and 97.3% below 1,800 mIU/ml, respectively. CONCLUSIONS: The sensitivity, specificity, and predictive value of transvaginal ultrasound for diagnosing abnormal early pregnancy were poor except cases of ectopic pregnancy when beta-hCG levels were low than discriminatory zone. Ultrasound impressions were well related with beta-hCG levels.

6.Acute Appendicitis Diagnosed by Colonoscopy.

Acute appendicitis is the most common operative disease in general surgery, but it is not also rate disease to see in other clinical department such as Internal Medicine and Obstetric and Gynecology, because it reqnires to differenciate from many other diseases which have similar symptoms. Expecially, because the clipical aymptoms of acute appendicitis are not typieal in children, elderly, and women, it is hard to be diagnosed as acute appendicitis and it is often misdiagnosed as other disease. We performed colonoscopy due to be interpretated aa one of the colon diseases rather than acute appendicitis. But in colonoscopic findings, there were hyperemie and edematous change on the appendiceal orifice of cecum(cherry or acorn shape). So we report 5 cases of acute appendicitis diagnosed br colonoscopy.

7.Histologic Study in Transplantation of Cultured Rabbit Retinal Pigment Epithelium.

The authors investigated the possibility of transplantion of cultured retinal pigment epithelial cell to normal pigmented rabbit retina. Focal retinal detachment was made in the pigment rabbit retina, and the cultured RPE cells were injected into subretinal space. The neural retina spontaneously rettached withim 6 days. At 4 weeks after tranplantation, eyes were enucleated and examined with light-microscopy and electron-microscopy. The transplanted RPE cells were proliferated with multilayer in electronmicroscopic finding, and the tight-junction was found between proliferative RPE cells. The outer segment and nucleus of photoreceptor cell were well preserved in microscopic finding. As a result, the cultured RPE cells can be sucessfully transplanted to normal pigmented rabbit retina, and photoreceptor cell was not changed after transplantation.

8.CHONDROBLSTOMA ON TEMPOROMANDIBULAR JOINT, A CASE REPORT.

Chondroblastoma is a rare primary bone tumor which originates from cartilage, and represents approximate 1% af all bone tumor. The chondroblastoma arises most frequently from the epiphysis of the long bones with the humerus being the commonest site. It afflicts usually the young under 25 years with greater incidence in male. As there is no cartilage cell on craniofacial bone which is mainly fromed by intramembranous ossification, the chondroblastoma on the craniofacial bone is extremely rare. But the chondroblastoma recurred frequently in craniofacial bone when the mass is excised incompletely or curretted and, as the tumor has the outstanding ability of local invasiveness, it destructs the adjacent bone. In addition, it is difficult to diagnose differentially from sarcoma or giant cell tumor histopathologically. Due to the entities described above, it is necessary to remove the entire tumor mass as complete as possible, to treat with radiation pre or postoperatively for preventing from recurrence, and to observe for a long time. The chondroblastoma on temporal bone is rare and is difficult to diagnose and treat successfully. So we'd like to present a case of chondroblastoma which was originated from temporal side of TMJ with literatural review.

10.A Case of Nongestational Ovarian Choriocarcinoma.

Pure ovarian choriocarcinoma of germ cell origin is exceedingly rare neoplasm, and even the presence of choriocarcinomatous elements admixed with other neoplastic germ cell elements is rare. In the most cases, the tumor is admixed with other neoplastic germ cell elemeats, and their presence is diagnostic of noagsatational choriocarcinoma, except for the remote of the tumor being a geatational choriocarcinoma metasttic to an ovarian germ cell tumor. We have experienced a case of nongestational ovarian choriocarcinoma in 10 year dld woman. So we report this case with a brief review of its literatures.
