BACKGROUND: The aim of this study was to investigate the relationships between left ventricular ejection fraction (LVEF) and mortality and neurologic outcomes with post-cardiac arrest syndrome (PCAS) after out-of-hospital cardiac arrest (OHCA). METHODS: Patients with PCAS after OHCA admitted to the intensive care unit between January 2014 and December 2015 were analyzed retrospectively. RESULTS: total of 104 patients were enrolled in this study. The mean age was 54.4 ± 15.3 years, and 75 of the patients were male (72.1%). Arrest with a cardiac origin was found in 55 (52.9%). LVEF < 45%, 45-55%, and > 55% was measured in 39 (37.5%), 18 (17.3%), and 47 (45.2%) of patients, respectively. In multivariate analysis, severe LV dysfunction (LVEF < 45%) was significantly related to 7-day mortality (odds ratio 3.02, 95% Confidence Interval 1.01-9.0, p-value 0.047). CONCLUSIONS: In this study, moderate to severe LVEF within 48 hours after return of spontaneous circulation was significantly related to 7-day short-term mortality in patients with PCAS after OHCA. Clinicians should actively treat myocardial dysfunction, and further studies are needed.

PURPOSE: This study was performed for investigation of epidemiology, clinical characteristics, and serial value of cardiac troponin level of patients who had myocardial injury due to Carbon monoxide poisoning. METHODS: This study reviewed 98 cases of Carbon monoxide poisoning patients who visited Emergency Department from January 1, 2008 to October 31, 2013. We categorized them by two groups, one with elevation of cardiac troponin level and the other with normal level. We had comparison between two groups data using statistical analysis. RESULTS: Among 98 patients of Carbon monoxide poisoning who were admitted to hospital, 10 patients were excluded. 88 patients who were included to our study, 70 patients with normal value of Troponin, and 18 patients with elevated troponin level. Of all patients, Carbon monoxide inhalation due to suicided trial patients has more higher proportion in elevated troponin level group compared with normal group (40 (57.1%) vs 15 (83.3%), P=0.041). Furthermore, corrected QT interval, length of hospital stay, number of ICU admission, also were showed higher value in elevated troponin level group. CONCLUSION: Carbon monoxide induced myocardial injury is associated with subside trial, prolongation of correted QT interval, length of hospital stay, and number of ICU admission.

To investigate the relationship between insulin response and morphometric changes of the mitochondria of pancreatic beta-cell, this study was performed using hyperglycemia and streptozotocin as oxidative stresses. Adult and neonatal rats were used. Intravenous glucose tolerance test (IVGTT) and morphologic examination of pancreas using immunohistochemical stain, in situ end-labeling method and electron microscopic study were performed. Various mitochondrial parameters were measured by image analyzer. Immunohistochemical stain revealed a markedly reduced islet size and decreased number of beta-cells and the increased number of non-beta-cell in adult and neonatoal streptozotocin group, and the appearance of insulin positive cells throughout the exocrine parenchyma in neonatal streptozotocin group. Three days after injection of streptozotocin in adult streptozotocin group, TUNEL stain showed increased apoptotic cells in islets. Ultrastructurally, beta-cells in adult streptozotocin group showed increase in number and size of mitochondria, and disruption of mitochondrial structures. Hyperglycemic group and neonatal streptozotocin group showed preserved mitochondrial ultrastructure. Ultrastructural morphometric study revealed increase in size and number of mitochondria and decrease in mitochondrial contour index in adult streptozotocin-treated rats, which suggested mitochondrial degeneration. Hyperglycemic group showed mild increase in size of mitochondria. Increased number of mitochondria was also observed in neonatal streptozotocin group. IVGTT revealed marked decrease in insulin response in adult streptozotocin group, and non-insulin-dependent diabetes mellitus pattern in glucose and insulin response in neonatal streptozotocin group. Hyperglycemic group showed a glucose and insulin response similar to control group. The above results suggest that a severe oxidative injury may cause degeneration and disruption of mitochondria of pancreatic beta-cell, and may be associated with substantial apoptotic cell death. The changes in the morphology and the number of mitochondria may result from streptozotocin treatment within neonatal period and hyperglycemia treatment, which may be associated with changes in insulin response.

In this report, a case of a 70-year-old man with a large ganglion cyst formed after anterior transposition of the left ulnar nerve is presented. Three months after the index surgery, the patient presented with a painless superficial ovoid, soft mass measuring 5x4x2 cm in size located at the posteromedial aspect of the left elbow, the previously operated site. Magnetic resonance imaging showed a well demarcated cystic mass with a stalk connecting to the elbow joint. Excisional biopsy was performed and pathologic findings showed that the cystic wall had no definite lining cells with myxoid degeneration compatible with findings of ganglion cyst.

Statement of problem. Osseointegration is important mechanism of dental implant but it is not easy to evaluate. Indirect measurement is non-invasive and clinically applicable but they need more study about correlation between indirect values and degree of osseointegration. Purpose. The aims of this study were to evaluate the coefficient of correlation between indirect measurement and direct measurement under different healing time, and assessment of effect of initial insertion torque to the implant stability. Material and Methods. 20 rabbits received 3 implants on each side of tibia. Three kinds of implants (machined surface implant, Sandblasted with Large grit and Acid etched implant, Resorbable Blast Media treated implant) were used. During the surgery implant insertion torque were measured with OsseocareTM. After the 1, 4, 8, 12 weeks of healing time, animals were sacrificed and stability values (Implant Stability Quotient with OsstellTM, removal torque with torque gauge) were measured. Results. The Bone quality of rabbit tibia was classified into 2 groups according to the insertion torque. Resonance frequency analysis (ISQ) and removal torque showed positive correlation until 4th week (r=0.555, p=0.040). After 8th week (r=-0.011, p=0.970) the correlation became weak and it turned negative at 12th week (r=-0.074, p=0.801). Insertion torque and ISQ showed changing correlation upon the healing time (1st week: r=0.301, p=0.033, 4th week: r=-0.429, p=0.018, 8th week: r=0.032, p=0.865, 12th week: r=-0.398, p=0.029). Insertion torque and removal torque has positive correlation but it was not statistically significant (1st week: r=0.410, p=0.129, 4th week: r=-0.156, p=0.578, 8th week: r=0.236, p=0.398, 12th week: r=-0.260, p=0.350). Conclusion. In this study, bone quality may affect the degree of osseointegration positively during healing time and correlation between ISQ and degree of osseointegration can be different according to the healing time and bone quality.

Posterior root tears of the medial meniscus are radial tears, usually found in patients older than 50 years with degenerative articular cartilage. Partial menisectomy has been the conventional treatment for this condition. However, growing dissatisfaction with partial menisectomy for medial meniscus root tears has led to an increasing interest in meniscal repair. One method for medial meniscus root repairs involves suturing around the region of the tear where the sutures are pulled-out and fixed around the tibia. In another approach, a suture anchor was used for fixation of the medial meniscus root tear. Both methods improved functional outcome scores at least during a short-term follow-up period. However, complete healing of the repaired root seemed to be less predictable. Strict patient selection, good surgical skills and proper postoperative rehabilitation are the key factors for successful repairs of medial meniscus root tears.

Osteochondritis dissecans (OCD) is a pathologic condition of the subchondral bone which involves the articular cartilage. Unstable OCD can be fixed with various implants, including bioabsorbable materials. Bioabsorbable materials have several advantages. Removal of the implant is not required, and they have low interference on imaging studies, such as magnetic resonance imaging (MRI). This paper reports a case of surgical treatment using bioabsorbable pins for OCD lesion. MRI showed a displaced osteochondral lesion at medial femoral condyle. Arthroscopic reduction and fixation were attempted initially, but additional mini-open incision was needed to reduce the fibrotic fragment. The fragment was trimmed and fixed with two bioabsorbable pins. Union was achieved at the follow-up without complications.The use of bioabsorbable pins for OCD treatment may be one of successful treatment method.
