Chylothorax*

Epithelioid sarcoma(ES) is a very rare, unique soft tissue sarcoma, which was first fully described by Enzinger in 1970. The tumor chiefly occurs in young adults and affects the soft tissue of the distal extremity. A 46-year-old male patient had slowly growing masses on his left plantar surface, which were diagnosed as ES and mass excision and radiation therapy were done. Eight months later, about 1 × 1 cm sized, well defined multiple erythematous nodules on his scalp were discovered. Although he was treated with chemotherapy, no improvement was gained.

Phakomatosis pigmentovascularis was first reported in 1947 as adiease complex with a distinct association of pigmentary nevus such as nevus of Ota, nevus of Ito, bilon golian spots, nevus spilus and cutaneous hemangioma such as nevus flammeus, which wss assoeisted with systemic organ involvment, We report a case of phakomatosis pigmentovascularis in a 11-year-old girl who had nevus flammeus, nevus spilus, and n.evus of Ota over various parts of her bo by since birth. The antihypertensive effect of trimazosin was studied in 24 cases of essential hypertension, which include 9 cases with pretreatment diastolic pressure of 114mmHg or more, for a period of 4 weeks. The average pretreatment systolic and diastolic blood pressures were approximately 175mmHg and 114mmHg, respectively. The treatment was started with 100mg of trimazosin daily in 2 divided doses and and the drug was titrated upward at weekly interval by 100mg up to 400mg/day depending on the response of the blood pressures. Routine blood counts, urinalyses, liver and kidney function tests, electrolyte balance, total serum cholesterol and triglyceride were determined before and at the end of treatment. The diastolic blood pressure fell 10mmHg or more in 20 out of 24 cases(83.3%), and in 12 cases out of 20 favorable responders it fell to 90mmHg or below. The pretreatment diastolic blood pressure in 4 nonresponders was all 115mmHg or more. The antihypertensive effect appeared during the first week of therapy and progressively increased until the end of treatment week without causing postural hypotension. Unpleasant symptoms appeared in 12 cases during treatment, which include dizziness, headache, numbness in the extremities and tinnitus in the decreasing order of frequency. However, these symptoms were mild and transient in all cases disappearing spontaneously despite continued medication. No significant biochemical changes in the blood were recorded after treatment. We conclude that trimzosin seems to be a safe and effective antihypertensive drug particularly useful for the treatment of mild to moderate hypertension.

BACKGROUND AND OBJECTIVES:Traditionally, down beat nystagmus is regarded as a sign of central nervous system dysfunction. But, several years has passed since Herdman et al reported the down beat nystagmus developed during treatment maneuvers for posterior semicircular canal benign paroxysmal positional vertigo(BPPV). We undertook this study to evaluate the character and clinical analysis of the positional or positioning down beat nystagmus, to discuss the clinical significance of positional or positioning down beat nystagmus as a diagnostic criteria of superior semicircular canal BPPV, and to propose the new treatment method. MATERIALS AND METHOD:From November 1999 to March 2004, we sampled the 103 patients with positional or positioning down beat nystagmus. Of these patients, we selected 16 patients except for the patients with central nervous system dysfunction, nonspecific or artifact result, idiopathic origin. RESULTS:All of 16 patients had no sign and radiologic result of central nervous system disorder. 10 patients was reported or suspected the diagnosis of posterior semicircular canal BPPV. Fatigability was reported in 9 patients and reversibility was reported in 1 patient. Average latency was checked less than 2 seconds. CONCLUSION:Although the diagnostic criteria of superior semicircular canal BPPV that we reported was not controversial, we expect that this criteria is useful in diagnosis for patients with atypical positional or positioning down beat nystagmus. And the new treatment method that we reported will has the better results than previous method.

Cortical dysplasia is a cause of intractable epilepsy and a candidate for surgical resection to control epileptic attacks. The neuronal cytomegaly and balloon cell change are the diagnostic hallmarks of cortical dysplasia. Little research has been performed about the normal-sized dysplastic neuron which has complex arborizing dendrites and lacks in its polarity. The aim of this study was to define the histopathologic characteristics of the neurons in cortical dysplasia. Twelve cases of cortical dysplasia who underwent partial lobectomy for intractable seizures were selected and immunohistochemical staining for NF-M/H, MAP2, tau, and ubiquitin was performed. The perikarya and dendrite of dysplastic neurons were more intensely labeled with antibodies for the high and medium molecular weight neurofilament proteins (NF-M/H) than normal neurons. Immunoreactivity with the MAP2 antibody expressed mainly within the somatodendritic regions was present in the dysplastic or normal neurons without any significant difference in intensity. The complex arborizing dendrites of dysplastic neurons were easily identified due to pronounced immunoreactivity within the somatodendritic regions. Immunoreactivity with the primary antibody against tau and ubiquitin was present in the normal-looking neurons as well as the dysplastic neurons. This study suggests that the dysplastic neurons in cortical dysplasia are accompanied by changes of cytoskeletal neurofilaments, and the immunohistochemical stains for NF-M/H, MAP2, tau, and ubiquigin are useful to detect them.

The authors analysed the CT findings in 75 cases diagnosed as gastric carcinoma by U.G. I.S. or endoscopy in a 3)1, year period. The comparative analysis betw~en CT findings and surgical pathologic findings was done in 55 cases who underwent surgery. 20 cases could not be operated upon because of distant metastasis. The gastric carcinoma was devided into 4′ stages according to the staging system of the American joint Committee on Cancer. The CT findings were then compared and the following results noted : 1. The relationship with gastric wall thickening and penetratioil degree of tumori no thickening (Iesser than 1.0 cm) of 3 cases showed all of T1, 11 of localized thickening (1. 0.1 .5cm) showed 7 of T2, 38 of localized thickening (more than 1.5 cm) showed 37 of T3 and 3 of diffuse thickening showed all of T4a. 2. The accuracy of Iymph node detection by CT scan, 1) Regionaf tymph node showed 86% in operative f in ding and 73% in pathologic. 2) Extragastric Iymph node showed 87% in operative finding and 89% in pathologic. 3) Retro-peritoneallymph node showed 96% in operative finding and 95% in pathologic. ic. 3. In operated cases, there was one case of false positive distant metastasis and three false negative cases. 20 cases of non-operated cases showed distant metastasis such as liver, lung or kidney. 4. 55 cases were classified as stage 1 in 4 cases, 11 in 18, 111 in 23 and IV in 10 by pathologic findings, and 1 in 3, 11 in 24, 111 in 19 and IV in 9 by CT. The overall accuracy by CT stagi ng is 75%. 5. Among a total of 75 cases, there were 46 operable cases by CT finding an d 43 operabl e cases by pathologic staging. There we re 29 inoperable cases by CT finding and 27 cases by pathologic staging. Therefore, there was a 93% overall accuracy by CT stagi ng in terms of determination of operability. 6. The accuracy of adhesion into adjacent structure by CT finding was 69%. compared with operable finding.

BACKGROUND: The combination of psoralen and UVA (PUVA) as a model of photochemotherap, has been used in a wicle variety of cutaneous disorders such as psoriasis, vitiligo, mycosis fungoides and atopic dermatitis. The mechanism of PUVA of psoriasis is based on the fact. that PUVA causes photoconjugation of psoralens to DNA and a subsequent suppression of mitosis, DNA synthesis, and keratinocyte proliferation. Although PUVA apparently inhibits keratinocytie proliferation and is effective therefore in the treatment of psoriasis, PUVA increases pigmentations by stimulating melanocyte proliferation and melanin synthesis in vivo. OBJECTIVE: We tried to investigate the PUVA effects on the proliferation and different,iation in cultured human keratinoc tes and melanocytes. METHODS: We examined morphologic changes, and the number of the cultured human keratinocytes and melanoiytes and melanin contents in a control group and experimental group. (UVA group, 8-MOP group and PUVA group). i.e., UVA group was exposed to UVA at 60mJ, of. 8-MOP group was acJded at dose of 2 x 10 M to medium for 30 minutes. PUVA group was exposed to UVA at 60mJ, of after adding in 8 MOP at Zx 10 M for 30 minutes. RESULTS: 1. Morphologic changes of cultured human keratinocytes and melanocytes. There were no significent changes between the control group and the experimental groups in keratinocytes and melanocytes after 24, 48 and 72 hours culture. The number and length of meianocyte dendrites showecl no significant differences between the groups after 24, 48 and 72 hours culture(p>0.05). 2. Proliferation of cultured human keratinocytes and melanocytes 1) The number of keratinocytes in 8-MOP and PUVA groups decreased significantly more than in the control and LVA groups at 72nd hour after culture (p<0.01). 2) The number of melnocytes showed no differences between the groups at 72nd hour after culture (p>0.05). 3. Melanin contents in iultured human melanocytes The melanin contents increased significantly in the PUVA groups compared to that in the other groups at 72nd hour after culture (p<0.01). CONCLUSION: In culturel human keratinocytes, PUVA has no effect on the morphology and differentiation, hut inhibit proliferation. In cultured human melanocytes, PUVA has no effect on morphology and proliferation, but it increases the melanin contents.
