1.Antibacterial Activity of Ceftizoxime Against Gram Negative Enteric Bacteria in vitro and in vivo.

Ceftizoxime sodium is a new synthetic β-lactam antibiotic combining potent antibacterial activity with high stability to a wide range of bacterial β-lactamase. This experiment was achieved to evaluate the antibacterial activities of ceftizoxime sodium against. Gram negative enteric bacteria isolated from in outpatient visiting Yeungnam university hospital and to study the emergence of drug induced bacterial variants which resist to ceftizoxime in vitro. The antibacterial activity of the ceftizoxime was compared with that of antibiotics and its effect on population of normal intestinal flora in mice was observed. The results are summarized as follows: 1. Highly effective antibacterial activity of ceftizoxime against Gram negative enteric bacilli was demonstrated and this antibacterial activity was superior to that of ampicillin. 2. Several test strains shows multiple antibiotic resistance. Among 15 strains of Escherichia coli, 1 strain was resistant to ampicillin, cefadroxil, gentamicin, tetracycline, and 2 strains were resistant to ampicillin, cefadroxil, tetracycline, five strains of Escherichia coli and Enterobacter cloacae was resistant to ampicillin, tetracycline and Shigella dysenteriae was resistant to ampicillin, gentamicin, tetracycline. 3. The frequency of in vitro emergence of resistant variants among ceftizoxime sensitive bacteria in the presence of increasing concentrations of the compound was found to be low. 4. Plasmid was isolated in 6 of 9 strains (6 strains of Escherichia coli, Shigella dysenteriae, Enterobacter cloacae and Salmonella typhi). That showed different antibiotic resistance. They were 5 strains of Escherichia coli and 1 strain of Shigella dysenteriae. However, plasmid could not be considered as a hallmark for antibiotic resistance by this Further studies with curing experiment are to be accomplished for this purpose. 5. Changes in the bacterial count of normal intestinal flora following 25 mg/kg/day administration of ceftizoxime over 5 consecutive days were not significant. In conclusion, ceftizoxime appeared to be a drug of choice in the treatment of Gram negative enteric bacilli infection.

2.The cervical spinal fractures : comparison of the sites and incidences according to the causes and the types of the injuries.

The fractures of the cervical spine are relatively uncommon, but they may cause serious neurologic deficits temporarily or permanently. So, it is very important to treat the patients early by way of exact evaluation for the sites and the mechanisms of the injuries. The authors reviewed retrospectively 188 cervical spinal fractures in 100 patients from Sep. 1984 to Aug. 1990. Commonly involed levels were C5 and C6 in lower cervical level and C2 in upper cervical level and the sites in each spine were body; lamina and odontoid process. The hyperflexion injury was the most common type of the cervical spinal fractures occupying 53% of all cervical fractures and cause more multipe fractures(2.26 fractures/patient) than in hyperextension (1. 68 fractures/patient). In hyperflexion injuries, body, transverse and spinous process were commonly involved but lamina fracture was relatively common in hyperextension injury. The dislocations associated with fractures were developed most commonly in hyperflexion injury and 70% of these were anterior dislocation and the most commonly involved levels were C5-6 and C6-7. In conclusion, hyperflexion injury needs more close examination for the entire spinal levels than injuries of other mechanisms because it results in more severe fractures with or without dislocation and relatively frequent multiple fractures in different levels.

4.Complications of Central Venous Totally Implantable Access Port: Internal Jugular Versus Subclavian Access.

BACKGROUND: Totally implantable access port (TIAP) provides reliable, long term vascular access with minimal risk of infection and allows patients normal physical activity. With wide use of ports, new complications have been encountered. We analyzed TIAP related complications and evaluated the outcomes of two different percutaneous routes of access to superior vena cava. METHODS: All 172 patients who underwent port insertion with internal jugular approach (Group 1, n = 92) and subclavian approach (Group 2, n = 79) between August 2011 and May 2013 in a single center were analyzed, retrospectively. Medical records were analyzed to compare the outcomes and the occurrence of port related complications between two different percutaneous routes of access to superior vena cava. RESULTS: Median follow-up for TIAP was 278 days (range, 1-1868). Twenty four complications were occurred (14.0%), including pneumothorax (n = 1, 0.6%), migration/malposition (n = 4, 2.3%), pinch-off syndrome (n = 4, 2.3%), malfunction (n = 2, 1.1%), infection (n = 8, 4.7%), and venous thrombosis (n = 5, 2.9%). The overall incidence was 8.7% and 20.3% in each group (p = 0.030). Mechanical complications except infectious and thrombotic complications were more often occurred in group 2 (p = 0.033). The mechanical complication free probability is significantly higher in group 1 (p = 0.040). CONCLUSIONS: We suggest that the jugular access should be chosen in patients who need long term catheterization because of high incidence of mechanical complication, such as pinch-off syndrome.

5.The Relationship Between the Changes of the Corneal Curvatures and Histopathologic Findings in Pterygia.

The purpose of this study is to find out what changes occur in the corneal curatures before and after the operation for pterygium and the relationships between the histopathologic findings and the various factors concerned with pterygium. The results are as follows: 1. The degree of corneal invasion of the pterygium head is closely related to the difference between the horizontal and vertical corneal curvatures before operation. 2. The difference between the horizontal and vertical corneal curvatures decre ase after operation. 3. The changes in horizontal curvatures are remarkable in 3 weeks after surgery with a mean change of +0.41 D. 4. In the histopathologic findings, the duration of pterygium and the increased amount of collagen fiber are significantly related(p<0.01). 5. There is no relationship between the histopathologic findings and the changee in corneal curvatures analysed by the SPSS program (p>0.05).

6.Retinal Thickness After Cataract Surgery Measured by Optical Coherence Tomography.

PURPOSE: To report the early postoperative macular change in uncomplicated phacoemulsification cataract surgery. METHODS: A retrospective study of 36 consecutive eyes of 36 patients, who underwent phacoemulsification cataract surgery and OCT, preoperatively and postoperatively was made. In 16 eyes from group 1, nonproliferative diabetic retinopathy without macular edema was present before the cataract surgery. In the other 20 eyes (group 2), no systemic or ocular pathology was present . RESULTS: The mean +/- standard deviation (SD) follow up after cataract surgery was 4 +/- 1 weeks in each group. In group 1, the mean +/- SD retinal thickness increased from 201 +/- 14 micro meter preoperatively to 243 +/- 22 micro meter 3 hrs after cataract surgery (P=0.38). In group 2, the mean +/- SD retinal thickness increased from 212 +/- 10 micro meter preoperatively to 246 +/- 24 micro meter 3 hrs after cataract surgery (P=0.55). CONCLUSIONS: This study showed that uncomplicated phacoemulsification cataract surgery has no immediate influence on retinal thickness.

7.Radiologic Evaluation of Intraabdomenal Masses in Childhood.

The abdominal tumors in children are different from those of adult. These tumors are the third most common one, preceded by leukemia and brain tumors, in children under 15 years. X-ray examination is the most important method among diagnostic approaches. The role of diagnostic imaging is to identify the precise anatomic location and extent of pathologic process with the minimal number of imaging procedures. 23 cases of abdominal tumors were reviewed in respect of age incidence, site of origin, radiologic findings. The results are briefly summarized as follows: 1. Neuroblastoma was the most common (6 cases) and wilm's tumor (5 cases), choledocal cyst (4 cases), ovarian mass (3 cases), hydronephrosis (2 cases), were descending order in frequency. 2. The most common site was retroperitoneum (60%) Kidney was the single most common site of origin. 3. Radiologic findings. The most common findings of plain radiography was ill defined soft tissue mass and this method was helpful in the presence of calcification especially in neuroblastoma. Ultrasonographic pattern was anechoic (cystic), echoic or mixed pattern, but this method provide less precise anatomical details, nevertheless Ultrasonography was particularly useful imaging modality for the pediatric abdominal tumors. IVP findings were renal displacement, caliceopelvic system distortion or nonvisualization of kidney, these information was helpful in determining the location of tumors. CT scan showed homogenous or inhomogeneous, cystic or solid, mass with their anatomic location. 4. Ultrasonography was the most widely used specific diagnostic method, but had limited value in detecting the anatomic location of tumors. CT scan was superior to ultrasound for determining the extent of tumors.

8.Subjective Visual Vertical during Eccentric Rotation in Acute Vestibular Neuritis Patients.

BACKGROUND AND OBJECTIVE: By assessing unilateral utricular function at the acute unilateral vestinuloneuritis (Acute UVN), we sought to determine the ability of the subjective visual vertical (SVV) during eccentric rotation (dynamic SVV) in localizing the site of the lesion in unilateral vestibular neuritis (UVN). METHODS: The static SVV and dynamic SVV of fifteen patients diagnosed with acute UVN were enrolled within 10 days of onset (average 7 days). First, the static SVV was measured in a dark booth without rotation. The dynamic SVV was measured during rotation with an eccentric displacement of the head to 3.5 cm from the vertical rotation axis during a constant velocity of 300 degrees/s. RESULTS: In the acute stage of UVN, the static SVV showed an increase in deviation to the side of the lesion compared to those of normal subjects. Also, we found 73% of abnormal findings in Acute UVN patients by assessing static conventional SVV. The dynamic SVV had a statistically significant increase in deviation to the side of the lesion compared to those of normal subjects and 93% patients showed beyond normal range. CONCLUSIONS: The dynamic SVV would be an effective method in the diagnosis and localization of acute unilateral vestibularneuritis.

10.Diagnosis and Operation Results for Chronic Lateral Ankle Instability with Subtle Cavovarus Deformity and a Peek-A-Boo Heel Sign

Cavovarus deformity is considered an anatomical risk factor for chronic lateral ankle instability (CLAI). However, subtle deformity can be difficult to detect, and its correction is controversial. The current study aimed to evaluate clinical and radiographic outcomes of a modified Broström procedure (MBP) with additional procedures for CLAI with subtle cavovarus deformity and a positive peek-a-boo heel sign. We reviewed the records of 15 patients who underwent MBP with additional procedures for CLAI with a positive peek-a-boo heel sign between August 2009 and April 2015. Consecutive physical and radiographic examinations were performed. The visual analog scale (VAS) for pain, the American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot score, and the Karlsson-Peterson (KP) ankle score were applied to assess clinical outcomes. Weight bearing radiographs, hindfoot alignment view, and ankle stress radiographs were also examined. The mean follow-up period was 58.5 months. Calcaneal lateral closing wedge osteotomy was performed in seven patients to correct fixed hindfoot varus, and first metatarsal dorsiflexion osteotomy was performed in 11 patients to correct plantarflexion of the first ray. Three patients underwent both procedures. Mean VAS, AOFAS, and KP ankle scores improved significantly (p=0.001), and instability did not recur. Radiographically, all stress parameters improved significantly (p=0.007). Simultaneous correction of a positive peek-a-boo heel sign and cavovarus deformity with MBP for CLAI improves clinical outcomes and prevents recurrent instability. A comprehensive evaluation and cautious approach for subtle cavovarus deformity should be followed when treating patients with CLAI.
