1.A Statistical Study of Morgolian Spot.

This survey was made on 1462 cases in different age groups, from the neonate to the age of 15, to analyse the data statistically concerning the prevalence, size, shape, site and color of Mongolian spot. The results were as follows: 1) Of 1462 cases, Mongolian spot(s) was(were) present in 759(51. 9%) with the sex ratio of male to female, 1: l. 2. The prevalence could be divided into 3 stages, high(neonate to 3years), abruptly declined(4 to 9year), steadily declined (10 to 15years) 2) The mean and standard deviation of diameter represented 4. 56+2. 96cm(M +SD), ranging from 0. 5 to 21 cm. 3) The two most common shapes were ovoid(45. 5%,) and irregularly circular (31.4%). 4) The two most common sites were sacrococcygeal(80. 1%,) and gluteal(75. 1%). 5) The two most common colors were blue(75. 1 %) and purple-blue(13. 5%).

3.An implant-supported removable partial denture on milled bars to compromise the inadequate treatment plan: a clinical report.

Presurgical prosthetic treatment planning is critical for the success of the implant prosthesis. Inadequate treatment plan, due to insufficient discussion between prosthodontist, and surgeon, may result in poor prognosis. A 26-year-old male patient was referred for prosthodontic treatment after implant was placed in the area of teeth #17, 16, 22, 25 and 27, without adequate discussion nor the treatment planning between oral surgeon and prosthodontist. It was found that the patient had two hopeless teeth, and a severely resorbed alveolar ridge. Additional tooth extraction was needed and the type of definitive prosthesis was shifted from fixed type to removable one. Proper pre-surgical treatment planning is essential for the good prognosis. Implant-supported removable prosthesis on milled bars may be a useful treatment option in patients with incorrect angled placement on severely resorbed alveolar ridge.

6.Pituitary Stalk Transection Syndrome

We experienced one case of severe pituitary dwarfism in a 16 years old male boy that magnetic resonance image(MRI) revealed transection of the pituitary stalk with the ectopic location of posterior pituitary gland and hypoplastic anterior pituitary gland. The serum growth horrnone(GH) response to clonidine and L-dopa revealed severe GH deficiency. The patient revealed normal response in TSH, prolactin and LH, FSH to TRH and LHRH respectively. So far, the primary cause of idiopathic pituitary dwarfism in many patients was due to hypoxic injury to hypothalmus by perinatal insults. In this patients, there was no history of perinatal insults and postnatal head trauma. We report a case of severe dwarfism due to hypogenesis of anterior pituitary gland.

7.Radiographic Knee Dimensions in Discoid Lateral Meniscus - Compared with Normal Control.

We reviewed the knee radiographs of 68 cases of arthroscopically-proven discoid lateral meniscus to evaluate usefulness of plain radiographs in diagnosing discoid lateral meniscus. The subject included 42 cases of complete type and 26 cases of incomplete type discoid lateral meniscus. Knee radiographs of 70 cases which had no pathology of the meniscus in knee arthroscopy were used as controls. We excluded patients who had a significant history of trauma and who were under 16 years of age or over 60 to avoid measuring radiographs of skeletally immature or degenerative knees. The absolute values and relative normalized values of specific knee dimensions such as a squared-off appearance of the lateral femoral condyle, widened femorotibial joint space, cupping of the lateral tibial plateau, obliquity of the lateral tibial plateau articular surface, high fibular head and hypoplasia of the Iateral intercondylar spine in discoid lateral meniscus and normal controls were compared. Among these dimensions, high fibular head and widened lateral tibiofemoral joint space showed statistically significant difference between the discoid lateral meniscus group and normal control group. We suggest that these findings could be useful in screening lateral discoid meniscus in plain radiographs.

8.A Clinical Study on the Fractures of the Proximal Humerus in Children

Fractures of the proximal humerus in children are rare and according to Neer and Horwitz, fractures of the proximal humeral epiphysis constitute 3% of all epiphyseal injury. The most frequently reported complications are humeral shortening, varus angulation, and neurovascular complications. Closed anatomic reduction in frscture of the proximsl humerus is very difficult, so reported modes of treatment have ranged from aggressive attempts at reduction to simple neglect. Twenty nine fractures of the proximal humerus in children which were treated at Seve- rance Hospital, Yonsei University, College of Medicine from July 1982 to December 1986 were snalyzed both clinically and radiologically and the following results were obtained. 1. The age of the patients in this series ranged from 3 years to 18 years and the most frequently between the age of 12 years to maturity. And the oldest age was 18 years in male and 17 years in female. 2. Among the 29 frsctures, 21 cases were involving the proximal humeral epiphysis and all the injuries were Salter-Harris type Il epiphyse14 fracture. And 8 cases were fracture through the proximal humeral metaphysis not involving epiphysis. 3. Sixteen cases(55%) hsd associated injuries and the common associated injuries were fracture of the lower extremity, and fracture of same upper extremity were 5 cases(17%). 4. The fracture were graded according to their initial displacement(Neer snd Horwitz) and grade I, 6 cases, grade II, 3 csses, grade , 5 cases and grade IV were 15 cases. 5. Eight cases of grade I and II fractures were treated by simple immobilization. And among the twenty cases of grade III and 1V fractures, 12 cases were trested by closed reduction and immobilization and 9 cases, by open reduction. 6. Regardless of displacement of fractures and modes of treatment, all cases were united between six weeks to twelve weeks with full range of motion and no pain. 7. There were two cases of K-wire loosening, two cases of transient nerve palsy and one case of varus angulation.

9.Surgical Treatments of the Acute Anterior Cruciate Ligament Injuries

Primary repair of anterior cruciate ligament is superior to late reconstruction procedures for two reasons; objective stability is more easily restored due to intact secondary restraints not subjected to chronic stress, and the quality of the joint is superior with greater chance of intact menisci and normal articular cartilage not yet damaged through recurrent subluxation. We present the result of thirty-eight kness in thirty-seven patients who had been treated with primary repair of augmented repair for acute ACL rupture from March, 1984 to August, 1990. Twenty-one knees were treated with primary repair and seventeen knees augmented repair. At an average 33 months follow up, the results obtained were as follows: 1. The patients were 23 males and 14 females with an average age of 27.1 years. 2. Twenty four knees were injuried during athletic endeavors, 9 knees by automobile accident, and 5 knees by landing from height. In 18 knees anterior cruciate ligament was ruptured at the proximal attachment, in 17 knees at the midsubstance tear and in 3 at the near distal attachment. 3. The average interval from injury to operation was 7.5 days (ranged from 1 to 14 days). 4. The results were evaluated by Hospital for Special Surgery knee rating scale (modified by Marshall) at an average 33 months (range from 13 months to 7 years and 6 months) follow-up study. 16 (76%) out of 21 knees treated with primary repair were rated as good or excellent. 12 (86%) out of 14 knees treated with augmented repair were rated as good or excellent. As the result of this study, acutely torn anterior cruciate ligaments appear to be successfully treated with primary repair or augmented repair according to torn site and shape of ligaments.

10.Advantages of incremental dynamic CT in the evaluation of pancreatic and peripancreatic lesions.

Authors compared early phase scan of the IV bolus CT (two phase incremental bolus dynamic CT) with late enhanecd scan similar to the conventional contrast enhanced CT for evaluation of the advantages of the IV bolus CT with two viewpoints of the pancreatic or peripancreatic mass and peripancreatic lymphadenopathy in 68 patients-28 cases of the pancreatic cancer, 6 cases of the pancreatitis and 34 cases of the pancreatic or peripancreatic metastasis. On the diagnosis of the pancreatic or peripancreatic mass, IV bolus CT could show the lesion(s) more easily in 41% (Grade II; 13/31) and much more easily in 34% (Grade III; 10/31) when compared with conventional contrast CT scan. The diagnosis of the peripancreatic lymph node involvement was also easy in 51%(Grade II; 20/39) and much easier in 37% (Grade III; 14/39). We thought that these differences were originated from the increase of the contrast between the lesion and normal portion because the early enhanced scans reflected the active blood flow change more exactly. Therefore IV bolus CT had advantages in comparison with the conventional drip infusion contrast CT in the diagnosis of the presence and pathologic extension of the pancreatic and peripancreatic lesion.
