1.A Clinical Observation on Antihypertensive Effect of Tripamide(Normonal(R)).

The antihypertensive effects of once-daily 15mg dose of tripamide(Normonal(R)) were observed in 30 cases of essential hypertension, and the results were as follows; 1) Mean drop in systolic and diastolic pressure were 30 mmHg and 16 mmHg, respectively. 2) In 76.7% of cases, good or fair control of blood pressure was resulted. 3) There was no significant difference in heart rate before and after treatment. 4) There was no significant difference in serum K, BUN, and FBS before and after treatment. 5) There was no any other side effect which required discontinuing the treatment.

2.The Role of Proteinase in Acanthamoeba keratitis and the Effect of Amniotic Membrane as a Proteinase Inhibitor.

This study was performed to investigate the biochemical properties of Acanthamoeba proteinase, its role in the pathogenesis of Acanthamoeba keratitis and the therapeutic effect of the homogenate of amniotic membrane as a proteinase inhibitors. Acanthamoeba castellanii isolated from the keratitis patient was cultured in PYG medium, in which the excretory and secretory products were analysed. The secretory proteinases of A. castellanii wre identified using in vitro azocasein assay, activity-PAGE, and various protein substrate degradation assays, and one of them was purified and characterized. The pruified secretory proteinase was a kind of serine proteinase. Its molecular weight was 105 kDa and optimal pH was 8.5. It was able to degrade the various protein substrates such as fibronectin, IgA, IgG, fibrinogen. The various proteinase ingibitors and the amniotic membrane homogenates were tested in vitro against the purified seirne proteinase. The amniotic membrane homegenates markedly showed the inhibitory effect against the enzyme activity and this inhibitory effect was also revealed in animal study. In vivo study, this purified proteinase was infected into 14 pigmented rabbit corneas, pretreated with steroids. The corneal lesions induced by both of the purified proteinase and A. castellanii, showed similar clinical findings each other, in which the stromal infiltration and opacity with epithelial defect was revealed. These corneal lesions were significantly inhibited without any side effects of the amniotic membrane homogenates. Conclusively, Acanthamoeba proteinase was closely associated with the pathogenesis of Acanthamoeba keratitis. This study provides a successful animal model of Acanthamoeba keratitis using pigmented rabbit. And the fact that Acanthamoeba-induced corneal lesions were inhibited by the amniotic membrane homogenate, suggested that the amniotic membrane homogenate have the ability of the serine protinase inhibition further investigative studies are also necessary.

8.Prosthetic Replacement for Severe Proximal Humeral Fractures

The authors have reviewed 11 cases of prosthetic replacement for severe proximal humeral fractures which were treated from March 1989 to December 1993, paying particular attention to the range of motion attained after surgery and the factors which influence the limitation of shoulder motion. The results of this study were as follows: l. Among the 11 cases, 10 cases showed sttisfactory pain relief. 2. Active motion of the shoulder averaged 91° of forward flexion, 72.8° of abduction and 35° of external rotation. 3. The humeral offset averaged 30.9mm in normal shoulder and 18.9mm in affected shoulder. 4. In 6 cases which the humeral offset measured over 20mm, active motion averaged 104° of forward flexion and 83° of abduction and 5 cases showed satisfactory results. 5. In 5 cases which the humeral offset measured under 20mm, active motion averaged 104° of forward flexion and 83 of abduction and 5 cases showed unsatisfactory results. 6. Prosthetic replacements were performed within 1 week after injury in 7 cases and among these 5 cases showed satisfactory results. The range of motion after prosthetic replacement for severe proximal humeral fracture was disappointing. We concluded that limited range of motion is mainly due to reduced humeral offset and restoration of humeral offset by appropriate reattachment of greater tuberosity will allow better clinical result.

9.Acetylcholine-Induced Endothelium-Dependent Responses in the Internal Mammary Artery and Grafted Saphenous Vein in Humans(in Vivo).

BACKGROUND: Coronary artery bypass grafts obtained from internal mammary artery(IMA) have a greater patency rate than do saphenous vein grafts(SVG). The release or production of endothelium-derived relaxing factor(EDRF), which regulates blood flow and inhibits platelets function, may contribute to the higher patency rate of IMA. In order to evaluate the difference between endothelium dependent relaxation in IMA and in grafted saphenous veins, we observed acetylcholine responses of IMAs and SVGs in humans. METHOD: Incremental dose if acetylcholine(Ach:A1-20, A2-50, A3-100microg) or single dose if acetylcholine(A3) were infused into the non-grafted IMA in 16 patients with different clinical presentation of coronary artery disease(3 with stable angina, 2 with acute myocardial infarction 4 with variant angina and atypical chest pain syndrome in 7). After the dose-response to Ach was completed nitroglycerine 200microg was injected into the IMA. Graded dose of Ach (A1, A2, A3) was also infused in 13 grafted saphenous veins(SVG). RESULTS: The overall vascular effects of Ach into the IMA were dilatory responses regardless of different clinical presentation. Two or 3 minutes after Ach injection, the diameter of IMA increased by 9.2%(A1) and 16. 7%(A3) respectively(p<0.01). Maximal dilatory response after A3 was comparable to that after nitroglycerin injection. Angiographically normal segements of SVGs also increased its diameter 2 minutes after Ach injection (7.2% with A1, 6.9% with A2 and 8.5% with A3). Thus the degree of relaxation of IMA was more prominent than that of SVGs. Angiographically dicreased segments of SVGs had no response after Ach injection. The overall responses of native coronary artery to Ach was dose dependant constriction. CONCLUSION: Injection of Ach to IMA and angiographically normal segments of SVGs caused vascular dilatation probably due to EDRF response. However the degree of relaxation was more prominent in IMA than in SVG. Angiographically diseased segments of SVGs had no response to Ach injection, suggesting that diseased SVG does not produce EDRF. However the finding of a consistent Ach-induced EDRF relaxation in the SVGs despite of constrictive response in the majority of native coronary arteries might suggest that the regenerated endothelial cell in the SVGs were probably of saphenous vein origin rather than derived from the coronary arterial endothelium.

10.A Study of Serum Lipid Levels in Normal Subjects and Various Diseases.

Total cholesterol and triglyceride were measured in sera of 40 cases of hypertension, 22 cases of myocardial infarction, 14 cases of cerebral thrombosis, 18 cases of subarachnoidal hemorrhage and 30 cases of normal control in Chungnam national University Hospital from October 1980 to September 1981, and the results are as follows; 1) The mean serum total cholesterol and triglyceride levels in normal control group are 153.57+/-40.60mg% and 150.82+/-51.76mg%. 2) The age incidence of these diseases were highest in 6th decade. 3) Serum cholesterol levels of myocardial infarction, hypertension, cerebral thrombosis and subarachinoidal hemorrhage group were higher than that of normal control group. 4) Serum triglyceride levels of acute myocardial infarction and hypertension were significantly higher than that of normal control group, but there is only slightly increases in cerebral thrombosis and subarachinoidal hemorrhage. 5) In general, serum lipid levels are higher in cardiovascular diseases(hypertension and myocardial infarction) than cerebrovascular diseases(cerebral thrombosis and subarachnoidal hemorrhage). 6) Serum Cholesterol and triglyceride levels decreased by administration of pancreatic mucopolysacharide for 1 or 2 months. In conclusion, it seems that high serum cholesterol and triglyceride level may play and important risk factor in development of hypertension and myocardial infarction.
